    Found during an annual infection-control review that the home was clean and in good repair, with installed smoke detectors, fire extinguishers present and charged, adequate food, PPE, cleaners and other supplies, and medications centrally stored and locked. Found to be in substantial compliance with regulations, and an exit interview was conducted.

    Found an infection control review conducted with pre-screening, masking, and hand hygiene observed, and that copies of the administrator certificate, liability insurance, and current staff schedule were requested. Noted a Technical Advisory regarding N-95 fit testing, with efforts to obtain a resource, and that the site appeared to be in substantial compliance at the time.

    Found gaps in daily medication administration records from Oct 1–4, 2024, and a chair near a resident’s door; staff explained the chair wasn’t blocking the doorway. MARs were completed with a double-check system and initials added, a MAR record-keeping concern was noted, and no deficiency was issued today.

Found that the home did not complete the required 20 hours of annual CEU training in 2023, though some training occurred in 2022 and January 2024, and that a resident's medication bottle lacked a specific dosage despite the MAR indicating daily use.
