I moved my mother to TreVista Concord and I'm very satisfied. The staff are caring, professional, and responsive - Lori Thames, Silvia Arriaga and Stephanie Encinas stood out - leadership communicates clearly and acts promptly. The community is warm, clean, safe and well-maintained, with great activities and attentive memory-care support. Meals are healthy and enjoyable, maintenance is excellent, and overall it feels trustworthy and welcoming; I'd recommend it to other families.

Summary of reviews
Reviews of Todos Santos Assisted Living and Memory Care describe a facility with clear strengths alongside persistent operational weaknesses. Positively, many families praise frontline caregivers as compassionate and attentive, and individual staff members and activity leaders are frequently singled out for strong, hands-on engagement. The activities program—highlighting music therapy, outings, bingo, and social events—receives consistent commendation. Several accounts note a refreshed, remodeled interior with patio access and responsive maintenance, and some families cite included services and occasional high-quality dining events as value-added features.
Care-quality impressions are mixed. Families report attentive, personalized interactions from many caregivers, routine checks for residents, and successful hospice transitions in some cases. However, a pattern of clinical process issues emerges across reviews: lapses in medication administration, missed or delayed hygiene assistance until prompted, and multiple accounts of falls and emergency visits. These operational problems are especially problematic for residents with increased medical or memory-care needs; reviewers indicate the facility may be better suited to lower-acuity residents unless clinical oversight and staffing are explicitly confirmed.
Staff and management commentary shows a split experience. Several reviewers describe excellent, communicative directors and front-desk staff, and note improvements after leadership changes. Conversely, others describe high staff turnover, inconsistent training—particularly for dementia care—and periods of unprofessional communication. Some reviews reference regulatory concerns and licensing irregularities, including state inspection items and an expired certification associated with leadership, and frequent ownership or management transitions that families felt undermined accountability.
Dining and daily life are similarly variable. The activities calendar and social programming are frequently highlighted as strengths that contribute to a warm community atmosphere. Meal quality, however, ranges from praise for special dinners to criticism of repetitive or sandwich-heavy menus and limited variety; dining comfort has been affected at times by HVAC failures in the dining room. Prospective families should observe typical meal service and ask about menu rotation and therapeutic diets when applicable.
Facility and safety observations are mixed. The physical plant benefits from recent remodeling, clean resident rooms, single-level design, and outdoor access. Maintenance responsiveness is often noted. Counterbalancing this, reviews describe sanitation and odor concerns in specific areas, capacity increases and shared rooms that have strained staff resources, unreliable climate control, and operational safety gaps—such as exit-alarm programming and supervision practices—that can compromise dementia care if not addressed.
Recommendation for prospective families: Todos Santos presents a combination of strong interpersonal caregiving and activity programming alongside operational risks that merit targeted inquiry. Families should verify current staffing ratios and turnover, medication-administration protocols, dementia-safety measures (exit alarms, supervised corridors), licensing and inspection status, HVAC reliability, and how the facility manages higher-acuity needs. Visiting during mealtime and an activities period, speaking directly with current residents and staff, and asking for recent state inspection results will give a clearer sense of whether the facility’s strengths align with a given resident’s care requirements.

About Todos Santos Assisted Living and Memory Care
Todos Santos Assisted Living and Memory Care, which used to be called TreVista Concord Senior Living and Memory Care, sits in California and is family-owned and operated, treating residents like family and focusing on each person's needs with care and attention, and the building itself has gone through an extensive renovation to better serve everyone living there. The community offers both assisted living and memory care, providing support for older adults who need help with daily activities or those living with Alzheimer's or other types of dementia, and there are specialized services including memory-enhancing activities, personalized guidance, and 24-hour support from staff. The memory care units are secure and access-controlled, with semi-private rooms, and the courtyard and walking paths inside the secured area give residents quiet places to relax and walk safely. The staff creates care plans after thorough nurse assessments, helping each resident get the right level of support, and the team includes highly-trained caregivers who offer personalized supervision in a structured setting. People living there get a chance to engage in a wide mix of programs, including fitness classes, art workshops, and social events, and there are frequent outings, holiday celebrations, and opportunities for spiritual, physical, social, and intellectual activity. The dining room serves meals restaurant-style, and there's also an on-site salon for haircuts and pampering. Each resident has a spacious room with a private bathroom, lots of closet space, and climate control, and the community puts safety first with smoke detectors and 24-hour emergency response systems throughout. Medication management is provided, and the care team is always available to help with daily living needs, whether residents are there for long-term assisted living, memory care, a short respite stay, or hospice support. Todos Santos is part of the Agemark family of senior living communities and offers resources and materials for residents and their families, including free events, e-books, and blogs, and if someone's looking for work, there's a range of job opportunities on the care, food service, housekeeping, administration, activities, and marketing teams. The atmosphere is warm and homelike, and the goal is always to help each person be as independent as they want while staying active, safe, and connected to others.
