    Warm attentive staff, excellent care

I placed a family member here and have been very pleased - the staff are caring, attentive and responsive, nurses and rehab teams are strong, and activities are meaningful. Meals are tasty and generous, the facility is clean and hotel-like, and front desk/social services are helpful and communicative. Overall, a warm, well-run place I'd recommend.

    Summary of reviews

The reviews present a mixed but coherent portrait of Rosecrans Care Center. Strengths cited repeatedly include a caring clinical team, a strong rehabilitation program, and an active social-services/admissions presence. Many families praised individual nurses, CNAs, the rehab team (PT/OT/Speech), and specific staff members in housekeeping and front-desk roles. The facility’s activity calendar—holiday events, religious services, daily social-area programming, and therapeutic activities—was frequently noted as engaging and beneficial to residents’ recovery and well-being. Dining quality is often described positively, with generous portions and tasty meals contributing to a hotel-like, family-oriented atmosphere in many areas of the building.

At the same time, reviews identify several operational weaknesses that prospective families should consider. There is a notable pattern of inconsistent clinical responsiveness: some families experienced prompt, attentive care and proactive updates, while others described delays in medication administration, slow response times to care needs, and variability in staff communication tone. Related to clinical care, there are serious individual allegations concerning wound management and infection outcomes; these raise concerns about wound-care protocols and infection-prevention practices and warrant targeted questions during a visit.

Safety and equipment maintenance emerge as another mixed area. While some reviewers praised fall-risk management and successful rehabilitation outcomes, others described falls and problems with mobility equipment, indicating potential gaps in transfer-safety processes and preventive equipment oversight. Facility maintenance and cleanliness are generally viewed positively at the building level—many reviewers called the environment clean and well-maintained—but there are inconsistent reports about room-level cleaning, occasional water/heating issues, and variable housekeeping thoroughness across units.

Communication and administrative responsiveness also show variability. Several families highlighted strong social-work involvement, helpful admission assistance, and proactive family updates; conversely, others reported lapses in post-hospitalization follow-up or difficulties getting timely information. Language barriers and inconsistent staff availability were mentioned as impairing some resident interactions. Activity programming is widely regarded as a strength, though a few comments suggest noise-management could be improved during certain events.

In summary, Rosecrans Care Center demonstrates clear strengths in rehabilitation, engagement programming, and many aspects of day-to-day resident support, with individual staff frequently singled out for compassionate care. However, recurring operational concerns—particularly inconsistent clinical responsiveness, wound-care and infection-prevention gaps, equipment and transfer-safety issues, and variability in room-level maintenance and communication—suggest areas for further inquiry. Families considering this facility should tour in person, ask about wound-care protocols, staffing ratios and shift coverage, medication administration procedures, equipment-maintenance practices, and policies for post-hospitalization communication to better understand how the facility addresses the variability noted across reviews.

    About Rosecrans Care Center

Rosecrans Care Center stands as a well-kept healthcare facility that offers rehabilitation, nursing care, and assisted living, and there are dedicated wings for skilled nursing and rehabilitation services where residents can recover from surgery or manage long-term health problems in a clean, bright environment, and the rooms open up to cheerful activity spaces and an outdoor patio where annual events like a luau take place, while the kitchen sends the warm smell of fresh meals drifting through the halls every day. Nurses and care staff spend an average of 1.54 hours per resident each day, including 0.63 hours from Registered Nurses, 0.91 from LPNs, and 2.59 from CNAs, and the nurse-to-patient ratio helps residents get personalized attention and treatment plans tailored for recovery, long-term care, or Alzheimer's support, while a team of social workers gives help with information, family needs, and discharge planning. The staff is also trained for medically complex care, and therapy programs are made to fit the individual, giving people a chance to regain skills or adjust to new needs, and respite stay options are available.

The building keeps residents safe with a full sprinkler system, security guards, event security, and mobile and plain clothes patrols. Rosecrans Care Center maintains a 92% occupancy rate, with 99 certified beds and space for 91 residents, though there are 113 Skilled Nursing Facility beds in total as reported. They take part in Medicare and Medicaid, and they're a for-profit organization, but they're not part of a hospital or a big group of nursing homes, nor are they a continuing care retirement community or a special focus facility. There are both resident and family councils, and a social services team is there for help with planning and requests. Amenities include a variety of common areas, activity rooms, dining rooms, and bright communal spaces, plus special activities are matched to each person's abilities, and friendly staff lead social programs.

About health quality, 25% of short-stay residents have had pressure sores, and 4% experience moderate to severe pain, but there are no reports of short-stay delirium. Vaccination rates for short-stay folks are lower, with 43% getting the pneumococcal and 48% the influenza shot, but long-term residents have higher rates-63% for flu and 74% for pneumococcal vaccination. Among long-stay residents, 7% have lost weight, 5% have had a urinary tract infection, and 4% have worsened movement ability; 8% spend most of the day in bed or chair, while 2% have a catheter in place, and 63% of low-risk residents lose bowel or bladder control. Two percent are more depressed or anxious, and 19% are physically restrained. Long-stay folks also have an 8% rate of needing daily living help, 2% have moderate to severe pain, and 22% have pressure sores. The center has received some deficiencies, mainly around food safety and resident rights, but these caused only minimal or potential harm.

Rosecrans Care Center is nonprofit, focusing on providing skilled nursing, post-acute care, and tailored rehabilitation, and the environment's designed to support seniors with individual needs and foster a sense of well-being. There are opportunities for outdoor activities and various events throughout the year, and the photo gallery shows modern hospital rooms and facilities. The website has more about services, amenities, and community trust that's been built up over time, with a reputation for quality care.
