    Compassionate, professional rehab; highly recommended

I placed my mom here for rehab and was very pleased - compassionate, attentive and professional staff gave excellent hands-on therapy and medical care, kept us informed, and made visits comfortable. The facility is spotless, welcoming, and well-run; I highly recommend it and would return.

    Summary of reviews

The review set for Grand Terrace Health Care Center is highly polarized, with two dominant experience patterns. Many families and clinical partners describe a rehabilitation-focused facility with strong therapy outcomes, attentive caregiving, and a generally clean, well-maintained environment. At the same time, a subset of accounts raises operational and clinical-safety concerns that significantly affect perceived quality of care for some residents.

Care quality and clinical services: Physical and occupational therapy are consistently highlighted as a major strength โ€” reviewers attribute functional gains, improved mobility, and attentive, outcome-focused therapy to the therapy teams. Several accounts praise nursing and clinician bedside manner and describe reassuring, hands-on physician involvement. Conversely, other accounts detail delays in clinical response (including long waits for medication or vitals to be taken), inconsistent pain-management practices, and gaps in staff clinical judgment on certain shifts. A small number of narratives mention regulatory or protective-service involvement; these appear alongside more typical rehabilitation success stories, indicating variability in clinical reliability.

Staff, teamwork, and professionalism: Staff receive mixed but generally positive marks for compassion and individualized attention. Numerous caregivers, CNAs, therapists, housekeeping staff, and administrators are named positively, and many families report clear, frequent communication and accessible department heads. However, reviewers also describe inconsistent conduct and communication tone from some employees, occasional retaliatory or unprofessional behavior, and coordination breakdowns between departments. Staffing shortages and variable shift coverage are recurring themes that reviewers link to slower response times and uneven care consistency.

Dining and nutrition: Dining experiences vary. Several reviewers praise meals and snacks and note good dietary support, while others describe cold or unappetizing food, unmet menu requests, and weight-change concerns. There are specific operational notes about temporary kitchen arrangements and at least one kitchen-related incident; these items suggest occasional interruptions to normal meal-service procedures and the need to confirm current meal oversight and accommodation capabilities.

Activities, amenities, and facilities: The facility is frequently described as clean, with active programming, outdoor visiting spaces, a spa/gym, and a generally welcoming atmosphere. Rehabilitation amenities and engaging activities are strengths for many residents. At the same time, some reviewers note discrepancies between online photographs and actual room condition, occasional odor or sanitation concerns in common areas, and variability in room quality. Maintenance and housekeeping receive strong praise in many accounts.

Management and operations: Leadership is often described as approachable and hands-on, with specific administrators and department heads receiving direct commendation. Yet a subset of reviewers report poor follow-through, slow phone responsiveness, and inconsistent case management. Notably, there are allegations related to lost or mishandled personal items and one or more claims of theft or property mismanagement; these are serious individual concerns that some families raised alongside other operational criticisms.

Notable patterns and considerations for prospective families: The facility appears to deliver very good rehabilitative care for many residents and to employ numerous committed, skilled staff. However, variability in staffing levels, responsiveness, medication handling, and certain operational controls creates a mixed quality profile. Prospective residents and families should weigh the strong therapy and activity programs against the reported inconsistencies in night/evening coverage, medication administration, dietary fulfillment, and personal-property safeguards. An in-person tour, direct questions about weekend and evening clinical coverage, medication protocols, dietary accommodations, and property-loss safeguards is advisable to determine whether the facilityโ€™s current operations match an individual residentโ€™s needs.

About Grand Terrace Health Care Center

Grand Terrace Health Care Center sits at 12000 Mt Vernon Ave, Grand Terrace, California, and you'll find it in a peaceful residential area where the building's all on one story and has 59 beds with private and semi-private rooms, some of them even set up as studio or companion apartments, and the grounds have a nice patio and gardens so people can go outside when the weather's good, while inside there are walk-in showers, worship services, dining options with nutritious meals, and both housekeeping and laundry services, so you don't have to worry about those chores. They provide a wide range of care, covering things like skilled nursing, long-term care, rehab, wound care, hospice, and there are therapy services in-house, like physical, occupational, and speech therapy, that use hands-on approaches for folks recovering from things like a stroke or surgery. There's a team of professional, caring, and friendly staff-nurses and in-house doctors give care 24/7, and the nurse staffing level is a bit above the state average at 4.60 nurse hours per resident per day, while the nurse turnover rate is about 34.4%, which comes in under the California average.

This place is well-equipped, with special areas for Cardiac ICU, Medical Surgical ICU, Pediatric ICU, Neonatal ICU, burn care, psychiatric care, and medical rehabilitation, and hospice, which means there's a focus on handling complex health needs and emergencies, as well as wellness with fitness programs, art and movie nights, social events, and outings into the community. There's lots of support with bathing, dressing, wound care, medication safety, infection control, and dietary management, though there have been inspection reports noting deficiencies, including 3 that were infection-related and problems like drugs not labeled or stored right and food not always from approved sources, for a total of 30 deficiencies recently. The facility is classified as "Other institution service type," is owned by Flagstone Healthcare Central LLC, with Brian Haroldsen managing since February 2023, and it's affiliated with The Ensign Group; it has a for-profit status, and typically around 52 residents day-to-day, with 59 certified beds available.

Alongside care, you'll find a range of amenities, such as exercise programs, housekeeping, laundry, nutritious meals, movies, community outings, and an activity schedule designed to help residents stay busy and active, plus there are religious services, transport for trips, and various support services, including opioid resources, suicide prevention, disaster mental health, elder abuse, fall prevention, and there's access to a health library, symptom checker, recovery centers, a care guide, and hospital ratings. The place keeps up a warm and supportive atmosphere, with rooms described as cozy and well-appointed, plus a rehab team many say is the best around, all aiming to help folks regain strength and mobility while feeling at home amidst beautiful, landscaped grounds.
