    Truly cared for during rehab

I stayed here for rehab and felt truly cared for - management and staff were outstanding. CNAs like Rebecca, Maria and Anthony (and LVN Clarissa) were kind and professional, scheduling (Dede, Mimi) was helpful, and admin (Jae, Ed, Daniel) was accessible. Therapy was excellent - Alice, Jackie, Kevin, Laura and PT Abby got me back on my feet in a welcoming, motivating gym. The facility is immaculate, privacy respected, meals and activities enjoyable; I would definitely recommend this center for rehab and long-term care.

    Summary of reviews

The reviews for Highland Palms Healthcare Center are strongly polarized but point to several consistent strengths. Many families and residents describe a clean, recently updated facility with attentive housekeeping and laundry services, private-room options, and a well-equipped therapy gym. The rehabilitation department (physical and occupational therapy) is repeatedly highlighted as a major strength: therapists are described as knowledgeable, encouraging, and effective in helping residents regain mobility and independence. Nursing staff and CNAs receive frequent praise for compassion, attentiveness, and hands-on care, and administrative leadership is often characterized as accessible and involved.

At the same time, there are repeated operational concerns that prospective residents should consider. Several reviews describe gaps in staffing continuity—notably on night and weekend shifts—which reviewers link to slower responses to call lights and inconsistent assistance. Family communication and notification during care transitions or clinical events is another recurring issue; reviewers describe delays or insufficient updates when residents required higher-level care or hospital transfer. These reports suggest underlying process weaknesses in incident reporting and family outreach protocols.

Clinical-timeliness concerns appear in a number of reviews: delayed diagnostic imaging or transfers, inconsistent medication/treatment timeliness, and variability in incontinence and hygiene assistance. A few reviews allege mishandling of dementia care and raise concerns following a resident’s death; these are serious individual claims that underscore the importance of confirming the facility’s dementia-care procedures, transfer protocols, and incident documentation during a tour or intake discussion.

Dining and activities receive mostly positive comments—many reviewers mention appetizing meals, themed events (for example, candlelight dinners), Bible studies, and social programs—but there are isolated complaints about meal temperature, occasional spoiled items, and insufficient daily-activity options for some residents. The facility’s social-services staff and therapy teams are often credited with advocacy and care planning, though a subset of reviews documents disputes over insurance, VA funding, and discharge timing, which can complicate transitions and financial arrangements.

In summary, Highland Palms demonstrates notable strengths in rehabilitation services, facility cleanliness, and many aspects of nursing and administrative leadership. However, patterns of inconsistent staffing coverage, communication lapses with families, occasional delays in clinical response, and operational disputes around billing and discharge appear frequently enough to merit careful pre-admission inquiry. Prospective residents and families should verify staffing patterns (including night/weekend coverage), ask for the facility’s protocols for family notification and emergency transfer, review visitation and infection-control policies, and confirm how insurance or VA funding issues will be handled to ensure the facility’s operational practices match their expectations.

    About Highland Palms Healthcare Center

Highland Palms Healthcare Center takes care of up to 99 residents in Highland, California, and most days there are about 89 people staying there, and boy, it really is a big place with a lot going on since there's a full inspection report that says inspectors found 34 deficiencies in different areas, and while Medicare has given the place a 5-Star Rating, the same report says there were 7 deficiencies in the most recent check, with 3 of them related to infections, and one about infection standards being violated, and then there was another one about how they use and care for feeding tubes, and another about not having a full care plan for every resident, and even a concern about assessing people when their condition changes, so they're trying to keep up with all these rules and care standards. The building is run as a for-profit limited liability company, and Providence Group Nh, LLC owns it all, but it's tied in with Pacs Group and Pacs Holdings, LLC too, and the whole place is listed as having a 45.8% nurse turnover rate, meaning a lot of the nurses end up leaving and being replaced, which sometimes happens in these sort of facilities. Residents here get about 3.95 nurse hours per day, and there are licensed nurses on duty 24 hours per day, and the staff tries to answer any questions about admissions, insurance, or the kind of care people might need, and there's always someone ready to talk through these needs. Highland Palms offers gentle physical therapy in a large rehab gym, along with activities, meals, and help with medical needs. They try to make people comfortable with friendly staff members and large courtyards, and they keep things going all hours of the day, with 24/7 admission help available, and a front desk open from morning into the evening most days. The place tries to be family-friendly, offering different spaces for people to gather, and keeping the rooms and common areas spacious, and they make it easy to pay online, or to stay in touch over Facebook or get a look at the place with online tours. People can stay for short or long periods, and some residents have been there for a few months at a time, with the team working on safety, wellness, and healing, but like any healthcare center, there are things they need to keep improving, especially when it comes to infection control and making sure every resident gets a complete care plan that fits their needs.
