    Caring staff and excellent rehab

I placed my loved one here and found the staff genuinely caring, professional and responsive - nursing and physical therapy were outstanding and helped restore mobility. The facility is clean, airy, dog-friendly, with a pleasant courtyard/patio, varied activities and generally well-prepared, timely meals (sometimes a bit bland). Night staffing felt thin and I had concerns about billing and transportation arrangements and a few administrative lapses. Overall we're grateful for the excellent rehab and compassionate staff - our family member is doing much better and I would recommend the place.

    Summary review

Overall sentiment across the review summaries is predominantly positive, with particular strengths in clinical care and rehabilitation. Multiple reviewers praise the nursing staff as caring, gentle, and professional; nurse aides are repeatedly described as kind and attentive. The rehabilitation department (physical and occupational therapy) is a consistent highlight — therapy staff are characterized as efficient, knowledgeable, and instrumental in helping patients regain ambulatory function and transition to lower levels of care such as assisted living or home. Several accounts describe excellent medical outcomes and gratitude toward management and staff for strong clinical care and coordination.

Facility features and atmosphere receive mixed but largely favorable comments. Many reviewers note that the center is clean, airy, and well organized, with appealing mountain views and easy courtyard access via sliding glass doors. Outdoor spaces, an activity room, accessible patio, and a dog-friendly policy are frequently cited as positives that enhance residents’ quality of life. Activities appear varied and social — examples include birthday parties, karaoke, arts and crafts, and movie nights — which several reviewers appreciated. Management and administration are often described as exemplary, responsive, and generous; the facility’s community outreach, including COVID-19 vaccination efforts, also draws praise.

Dining is reported in both positive and negative terms. Several reviewers praise well-prepared, varied meals that are delivered promptly, respect dietary restrictions, and come in ample portions. At the same time, a noticeable number of reviewers find the food bland or otherwise unsatisfactory. This split suggests that culinary experiences may be subjective or variable depending on the kitchen rotation or the individual resident’s tastes.

However, there are important and recurring concerns that prospective residents and families should investigate further. Multiple reviewers raise staffing issues — described as understaffed and thin coverage during night hours — and at least one account alleges staff refusal to assist and an apparent lack of compassion, with further criticism of a nurse and administrator for inaction. Safety and infection control worries appear in a few strong negative reports: one reviewer witnessed a serious accident, and another reported their mother was hospitalized due to a severe infection and described dirty conditions and bad smells. These serious negative anecdotes contrast sharply with the many positive testimonials and point to variability in care or isolated lapses.

Operational and administrative red flags were also mentioned by a few reviewers. Concerns include questionable billing practices, an odd cash-only transportation arrangement (and at least one canceled transport), and an instance where glasses were never delivered after a fitting. While many reviewers commend management as exemplary and responsive, these reports indicate potential inconsistencies in ancillary services and administrative follow-through.

In summary, La Crescenta Health Care Center appears to deliver strong clinical and rehabilitative care with many satisfied residents and families, emphasizing compassionate bedside care, a robust rehab program, and pleasant indoor/outdoor amenities. At the same time, reviews reveal variability: some families report poor cleanliness, staffing shortfalls (particularly at night), occasional lapses in compassion or responsiveness, and administrative inconsistencies relating to billing and transportation. Prospective residents and families should weigh the facility’s demonstrated strengths in therapy, nursing, and activities against isolated but serious reports of safety, infection, and operational concerns; recommended due diligence includes asking about current staffing ratios (including night coverage), infection-control practices, dining menus, recent inspection results, transportation procedures, and how the facility handles complaints and incidents.

About La Crescenta Health Care Center

La Crescenta Health Care Center sits as a for-profit, 92-bed skilled nursing facility, serving older adults who need help after a hospital stay, surgery, or illness, and it's got everything from medication management and help with bathing, dressing, and transfers, to 24-hour supervision and 12-16 hour nursing care each day, and with furnished rooms that come with Wi-Fi, air conditioning, private bathrooms, cable TV, telephones, and even a kitchenette, it tries to make day-to-day living comfortable. Residents find several community features-an arts room, library, fitness room, walking paths, movie theater, garden, and business room, along with daily scheduled activities, and there's a focus on keeping folks busy with movie nights, music, resident-run programs, and a variety of activities tailored to individual interests, all managed by a staff that includes a professional chef who prepares restaurant-style meals with options for allergies and diets like diabetes, with a registered dietitian on hand working with doctors for nutrition planning. The place takes both Medicare and Medicaid, has 88 average residents per day, and maintains four or so nurse hours per resident every day, so people who need help with personal care like bathing, dressing, and eating, or more advanced needs like wound care, intravenous therapy, pain management, enteral therapy, and both short and long-term care, can get it, and there's also a dedicated COVID unit with "dedicated heroes" staff for infection control, not to mention a strong emphasis on safety and communication with public health officials, and they've put in place visitation rules following health guidelines. All rooms have emergency alert systems just in case, and there's transportation, parking, housekeeping, laundry, and dry cleaning available, plus a Rehabilitation Therapy Department that tackles everything from basic tasks like cooking and cleaning, to dedicated occupational, physical, and speech therapies, and pain management. Inspectors found 63 deficiencies in recent reports, including 6 related to infection, and as of June 2025, 3 beds remain available; the most recent report on April 25, 2025, cited 12 deficiencies including issues with infection control and garbage disposal, so there are things to consider on that front. Staff work directly with residents and their physicians to create care plans, and offer psychological services, social and emotional support, restorative nursing, feeding assistance, and retraining when needed. La Crescenta Health Care Center does not operate inside a Continuing Care Retirement Community and is affiliated with Mariner Health Care, and while it provides a wide range of care and activities, it's wise to look closely at its inspection history and nurse turnover rate (37.3%) before making any decisions. Everything is delivered in English, and the center offers both non-ambulatory care and individual support with daily living, plus special assistance programs, a music program, a game room, fitness sessions, wellness and spa services, a community garden, and outdoor programs, so residents find both medical support and things to keep them engaged each day.
