    Caring staff excellent rehab welcoming

I felt my loved one was in genuinely caring, professional hands-nurses and CNAs treated us like family, therapy and rehab were excellent, and staff were attentive and communicative. The facility is clean, welcoming, pet-friendly, with great food and engaging activities; overall a highly satisfying place for recovery.

    Summary of reviews

La Jolla Nursing and Rehabilitation Center elicits strongly mixed impressions. Many families and residents praise its rehabilitative services, describing physical, occupational, and speech therapy as professional, effective, and recovery-focused. A substantial number of reviewers highlight compassionate front-line caregivers — CNAs and nurses who are attentive, patient, and engaged in daily care tasks — and they credit hospice teams for positive end-of-life support. The facility’s physical environment receives frequent compliments: an open, well-lit lobby, a sizable courtyard with ocean breezes, spacious dining areas, and generally clean public spaces create a welcoming first impression.

Dining and activities are commonly cited strengths. Several reviewers note a varied menu with healthy options, custom omelet preparation, baked goods, and a range of recreational programming (bingo, chair yoga, karaoke, holiday events, and special luncheons). Admissions, case management, and discharge coordination are often described as organized and helpful, and the facility’s location near the beach, its pet-friendly policies, and outdoor amenities are additional positives.

At the same time, a consistent pattern of operational concerns appears across reviews. Staffing levels and shift coverage are uneven, which reviewers connect to delayed call-bell responses, long waits for assistance, and inconsistent bathing and toileting schedules. Medication-administration and pharmacy-supply gaps are reported, including late doses and occasional shortages; these describe system-level risks rather than isolated impressions. Housekeeping and sanitation practices are inconsistent: while some areas and shifts are noted as thorough, other reports describe lapses in timely linen changes, room cleaning, and odor control. Equipment maintenance and transfer-safety practices are also an area of concern, with reviewers reporting broken beds, missing or inadequate rails, and fall incidents that point to weaknesses in routine safety checks and transfer protocols.

Management and communication present a mixed picture. Several reviewers praise individual leaders, admissions staff, and specific nurses for clear communication and advocacy. Conversely, there are repeated observations of high staff turnover, low morale, unprofessional conduct in isolated instances, and gaps in family notification and incident response. Additional operational pain points include parking limitations and external noise, inconsistent meal quality and adherence to dietary instructions, occasional loss or mismanagement of personal belongings, and variable visitation policies. These patterns suggest that care quality can be quite good under certain teams and shifts but uneven across the facility.

For prospective residents and families, the dominant theme is variability: strong rehabilitation services, welcoming public spaces, and many dedicated staff members coexist with recurring issues around staffing consistency, timely assistance, sanitation practices, and equipment maintenance. An on-site visit that includes conversations about staff-to-resident ratios by shift, medication-administration safeguards, housekeeping schedules, safety and transfer protocols, and family-communication procedures will help clarify whether current operational practices match your priorities.

About La Jolla Nursing and Rehabilitation Center

La Jolla Nursing and Rehabilitation Center sits in La Jolla, California, just minutes from the beach, where residents can catch ocean views and the ocean breeze, and the place offers 161 certified beds for both short-term and long-term stays, with a strong focus on skilled nursing and rehabilitation services, and they've got nurses, doctors, physical therapists, occupational therapists, respiratory therapists, and activity directors working together, plus private and semi-private rooms with Wi-Fi, pain management, and hospice care available if needed. The staff there tries to help each resident reach the highest level of independence they can, using state-of-the-art medical equipment and carrying out lots of therapy-physical, occupational, and speech therapies are all offered-while the recreational program brings movies, teaching classes, religious programs, and arts and crafts to keep people active and engaged, and you'll find caregiver training, discharge planning, infusion therapy, and 24-hour skilled nursing care, with a daily schedule that tries to fit each resident's needs. The center is managed by Lance Hassell, has been under his control since May 2018, and runs under Covenant Care California, LLC, but has several other owners too, including Centre Capital Investors V, LP and Covenant Care, LLC, and it operates as a privately held place with 51 to 200 employees on staff. Even though they try for high quality and compassion, the facility has seen 48 total documented deficiencies including infection and resident rights issues, and even had delays in inspections-its last standard one happened over two years ago instead of the usual every 9 to 15 months. The nurse hours per resident per day sit at 3.75, which is below the state average of 4.5, and the nurse turnover rate is at 49.0%, higher than the state's 40.0%, and there have been citations for not always respecting residents' rights or pharmacy standards. The team says they're dedicated to providing a safe, respectful, and supportive place, serving fresh meals in an open dining room, and greeting residents with kindness, focusing on both recovery and comfort, and everything's done with the motto "Family Serving Families," aiming to meet the needs of each individual living there.
