    Caring attentive staff enabled recovery

I had a very positive experience: caring, attentive staff and nurses (Michelle, Andrew, Ms. Keishell, RN Patricia) made the fast hospital-to-facility transition easy, the therapy with Amy and team was excellent and helped me/get my sister walking again, and the facility was clean and home-like. Admissions and case management were proactive and responsive - a couple small areas could improve, but overall I strongly recommend their rehab and nursing care.

    Summary of reviews

The reviews for Grand Avenue Healthcare & Wellness Centre show a mixed profile: the facility receives frequent praise for its rehabilitation program, several individual clinical staff, and a generally welcoming admissions experience, but there are recurring operational concerns that families should evaluate carefully.

Care quality and clinical services: Many families and patients described strong rehabilitation outcomes and attentive therapy teams who provide individualized exercise instruction and extra time during sessions. Nursing staff are often described as compassionate, responsive, and respectful; some named case managers and nurses were highlighted for clear communication and proactive coordination. However, there are multiple accounts suggesting inconsistencies in clinical practice โ€” specifically concerns about medication-administration oversight, possible inappropriate sedation, and delays in attending to urgent needs. These patterns point to variability in clinical monitoring and medication processes that prospective families should query directly.

Staff and culture: Staff behavior receives polarized feedback. Numerous comments praise caring, attentive caregivers and helpful administrative personnel who facilitate smooth transitions from hospital to facility. Conversely, other comments raise concerns about communication tone, staff conduct, and workplace culture; there are also serious claims regarding discriminatory behavior that warrant careful inquiry. Staffing levels were also cited as contributing to delayed responses to call lights and requests, indicating potential gaps between staffing needs and coverage at times.

Safety, belongings, and infection control: Safety practices were described inconsistently. Some reviewers reported attentive fall-prevention care, while others raised concerns about side-rail use and overall adherence to safety protocols. Several reviewers reported problems with handling residents' personal property and belongings, suggesting weaknesses in property-control processes. Infection-control practices were noted positively in parts โ€” with strict PPE use mentioned โ€” but at least one COVID outbreak and variable outbreak communication were also reported, suggesting uneven outbreak management and family notification procedures.

Facilities, environment, and amenities: Many accounts describe a clean, well-kept, and hospitable facility with good interior maintenance and a home-like atmosphere. At the same time, some reviewers identified sanitation and laundry issues and raised concerns about environmental controls, including air-conditioning reliability during warm periods. Dining and activity programs were not a major focus in most reviews; where activities or programming were mentioned, feedback tended to be positive about the rehabilitative and social supports available.

Management, administration, and billing: Admissions coordination and transition support are clear strengths for some families, with staff described as helpful and efficient. Yet administrative inconsistencies were also reported โ€” including name confusion, frequent administrative changes, and concerns about opaque ancillary charges (transportation fees and other billing practices). These mixed signals suggest the facility may have effective frontline coordinators but could improve consistency in broader administrative transparency.

Notable patterns and recommendations: The dominant positive theme is effective rehabilitation and several highly regarded clinical staff members; the dominant negative themes are variability in staffing and communication, procedural gaps around medication and property management, and inconsistent safety and environmental controls. Prospective residents and families should visit in person, ask detailed questions about staffing ratios, medication administration protocols, fall-prevention procedures, property-handling policies, billing transparency, and outbreak-management plans, and request to meet the therapy and nursing leads. Doing so will help determine whether the facilityโ€™s strengths align with the residentโ€™s needs and whether operational concerns have been addressed.
