    Caring professional staff, excellent rehab

I'm grateful for the caring, professional team-Isabel, Ashley, Victor and Kimberly stood out-who treated my relative with dignity and kindness. Nursing and rehab were excellent; we saw real recovery and increased independence. Meals felt personal and customizable, communication was strong, and staff accommodated special requests. The facility was clean, welcoming and felt like home. I would gladly recommend York and would return if needed.

    Summary of reviews

The reviews for York Healthcare & Wellness Centre describe a facility with clear strengths in person-centered caregiving and rehabilitation, coupled with recurring operational inconsistencies. Many families and visitors praise the compassionate attitudes of nurses, CNAs, therapists, and specific administrative staff; reviewers frequently credit the physical-therapy program with producing measurable gains and note case managers who facilitate discharges and Medicaid guidance. The facilityโ€™s outdoor patio, routine housekeeping in many areas, personalized meal accommodations, and availability of social activities are also positive elements that repeatedly surface.

At the same time, a number of reviewers describe systemic variability in care from shift to shift and between admissions. Common operational concerns include slow responses to assistance calls, uneven nursing attention to routine tasks (bathing, turning, incontinence care), and delays or inconsistencies in medication timing. Several accounts indicate difficulty getting timely clinical escalation for acute symptoms, and a few describe coordination delays when residents required transport or advanced medical procedures. These items suggest staffing and supervision gaps that affect both routine comfort and clinical safety.

Dining and hospitality are reported unevenly. Some reviewers experienced high-quality, restaurant-style meals and appreciated individualized meal plans; others described cold, bland, or stale food and instances where special-diet instructions were not consistently followed. Activity programming exists and is meaningful for some residents (bingo, crafts, church services), but several reviewers noted limited offerings or inconsistent availability of organized engagement.

Facility environment and amenities draw mixed commentary. Positive notes include a clean, welcoming atmosphere in many communal areas, a pleasant patio, and efficient first-visit intake. Conversely, there are sanitation concerns in particular areas and reports of inadequate temperature control on hot days. Language-access strengths (Spanish-speaking staff) are balanced by reports of communication barriers with some aides. Management and leadership receive both praise and criticism: specific administrators and the director of nursing are lauded for responsiveness and advocacy, yet other comments point to supervision shortfalls, inconsistent shift oversight, and frustration with unclear or retroactive billing practices.

Overall pattern: reviewers portray York as a facility where individual staff membersโ€”particularly therapists, certain nurses, and administratorsโ€”can deliver high-quality, compassionate care and good rehabilitation outcomes, but where operational variability undermines consistency. Prospective residents and families may benefit from meeting key staff (nursing leadership, case manager, dietitian), clarifying medication and escalation protocols, asking about staffing levels for the planned unit and shift, reviewing billing and admission paperwork in advance, and observing mealtime and evening-shift routines to gauge fit and consistency.

About York Healthcare & Wellness Centre

York Healthcare & Wellness Centre sits in Los Angeles, right in the heart of Highland Park, and it's got 107 certified beds, with about 92 residents each day. The centre is licensed by the California Department of Health Care Services and takes both Medicaid and Medicare. Folks here can get nursing care, physical therapy, occupational therapy, speech therapy, and medical social services, and there's around-the-clock care with help for both short-term rehab and long-term stays, plus plenty of attention for each person. The rooms are big, recently remodeled, and have private bedrooms and personal flat-screen TVs. There's complimentary valet parking, restaurant-style dining, and menus that the dietician approves, plus special therapeutic diets.

You'll find things to do like arts and crafts, live music, gardening, and exercise programs, and there are trips out to restaurants, malls, and museums for those who want to get out a bit. Services also cover medication management, wound care, orthopedic and dialysis care, and you'll see a team that tries to keep things peaceful and compassionate for everyone. The spaces are bright and cheerful, with dรฉcor and amenities meant to feel comfortable and clean.

Now, York Healthcare & Wellness Centre is a for-profit limited liability company run by Shlomo Rechnitz since June 2012. It gets a slightly below-average rating from CMS. It's had 56 deficiencies on inspection reports, including five infection control issues and two recent complaint reports, and there have been citations over pressure ulcer care and residents' rights about coming back after hospital stays. Nurse turnover is high at nearly 50%, above the state average, but nurse staffing hours are about the same as other places in California. The centre does offer skilled rehab, treatment plans for mobility and strength, and a mix of activities for both mental and spiritual well-being. Overall, it's got a close-knit team trying to cover a wide range of elder care needs in a setting that feels open, relaxed, and as comfortable as possible.
