    Caring staff and clean facility

I'm very pleased with the clean, attractive facility and genuinely caring, professional staff. Nurses, CNAs, PT/OT and housekeeping consistently went above and beyond; meals were tasty and activities plentiful. Management was proactive and attentive, and communication felt personalized so I was comfortable entrusting my loved one. I highly recommend this place.

    Summary of reviews

Stonebrook Health and Rehabilitation elicits strongly polarized impressions: many reviewers describe a clean, modern facility with an effective, well-resourced rehabilitation program, while others raise operational and clinical concerns that affected their experience. Strengths commonly cited include high-quality physical and occupational therapy (with purpose-built training areas), engaging activities, helpful front-desk and records staff, and several examples of compassionate nursing and CNA care. The building, landscaping, and onsite amenities such as a hair salon and outdoor sitting areas are frequently described as updated and well maintained.

Clinical care and staffing present a mixed picture. Positive accounts highlight therapists who provide individualized instruction and measurable functional gains, and families who felt supported by attentive frontline staff. Conversely, recurring themes include inconsistent staffing levels or skill mix, delays in medication administration and call response at times, and variability in hands-on personal care (for example, assistance with bathing or feeding). Several reviews also mentioned limited physician presence and difficulty obtaining timely clinical communication, which contributed to family frustration in discharge planning and care coordination.

Dining and activities are another area of contrast. Multiple reviewers praised engaging activities and some dietary accommodations, including favorable special-diet offerings and generous portions. At the same time, there are numerous comments about inconsistent meal quality, portioning, and the nutritional adequacy of meals for some residents. Families should inquire about menu planning, special-diet implementation, and monitoring of weight and intake during stays.

Facility operations and environment show strengths and weaknesses. Many describe a tidy, odor-free campus and comfortable rooms; others report temperature-control problems, noisy hallways, and inconsistencies around room assignments and privacy (shared versus private rooms). Housekeeping standards appear uneven across units, producing sanitation concerns in some accounts. Safety and security processes were questioned in isolated instances (for example, unlocked doors and access control lapses), suggesting a need for stronger consistency in facility procedures.

Management, communication, and administrative matters are common sources of dissatisfaction. Families praised individual staff members and noted instances where leadership intervened effectively, but there are frequent complaints about poor phone responsiveness, a lack of timely callbacks, unclear answers about appointments and discharges, and contentious billing or Medicare-authorization interactions. Several reviewers described prolonged or poorly coordinated discharge processes tied to insurance authorization or billing, which affected transitions home or to other settings.

Overall pattern and advice: Stonebrook appears capable of delivering strong rehabilitation outcomes and offers a pleasant physical environment, but operational variability — particularly in staffing consistency, clinical communication, discharge planning, and housekeeping — leads to uneven experiences. Prospective residents and families should ask targeted questions before admission: confirm physician coverage and how families will be updated, review the facility’s discharge-planning and billing processes, ask about typical call-light response times and staffing ratios, verify room-assignment policies and privacy options, and request specific information about meal plans and therapeutic diet implementation. These steps can help match expectations to the aspects of care the facility performs most consistently well.

    About Stonebrook Health and Rehabilitation

Stonebrook Health and Rehabilitation sits in the Los Gatos, CA area and offers a range of care for seniors who need extra help or are recovering from surgery, illness, or falls, and the place is open around the clock with nursing care, so someone's always there to help. The staff includes CNAs with rates starting at $25.30 an hour, licensed vocational nurses starting at $40 an hour, and occupational therapists who start at $55 an hour, with part-time roles for Certified Occupational Therapist Assistants open for those interested. The facility takes Medicaid, Medicare, and long-term care insurance to help with costs. People can count on Stonebrook for nursing services, physical therapy, and occupational therapy, and therapists like Christian and Brian work with patients to help them regain their independence and get well enough to go home.

The living spaces have private bathrooms, air conditioning, kitchenettes, phones, Wi-Fi, cable TV, and each room comes with individual TVs, fresh water, and Ensure. There are washers and dryers at the facility, and every room has safety and handicap features like sprinkler systems and accessible bathrooms. Stonebrook has a fitness center, a library, and paths for walking, plus a salon and barber shop, and guests can enjoy programs in the activity room, arts room, a game room, a movie theater, and even a community garden. There's a dining room with restaurant-style service, allergy-sensitive food options, diets for diabetes, and meals by a professional chef. Housekeeping, maintenance, laundry, and dry cleaning come included to keep things comfortable and safe.

People living there get help with bathing, dressing, toileting, and moving around, including for those who use wheelchairs. Nurses manage medicine, wounds, diabetes (including insulin), and provide care for Parkinson's or mild cognitive impairment. They also provide hospice waiver services and transfers, and the podiatrist visits for foot care. Stonebrook brings in community services, transportation, parking, and family support, plus education classes and arts or social events, with plenty of activities like movie nights, music, and fitness programs to keep everyone engaged.

Business office help is part of their service, and the facility uses up-to-date systems for HR, payroll, and hiring, making things smoother for everyone who works there. All their licensing, certification, and inspection files stay updated for anyone who wants to check, and there are always about 6 out of 73 beds open in June 2025, so people can schedule tours and virtual visits to see if it fits their needs. The staff gets a lot of good words for being attentive, with names like Nejat providing elder care and nurse supervisor Hiruth called a real asset to the team, which makes the place feel safe, clean, and welcoming. The whole setup feels comfortable and looks pleasant too, which many find helps them thrive and feel at home.
