    Good care, but serious lapses

I liked the clean, well-run facility, very good food, excellent PT/OT/Speech and many truly caring, helpful staff (Vivian and Kristian stood out) - my dad received good quality care. However, I also saw serious lapses: unanswered call lights, ignored residents, rude front desk/poor communication and one instance of neglect, so stay vigilant and advocate.

    Summary review

Overall sentiment across the review summaries is mixed but leans positive for clinical care and therapies, with recurring praise for compassionate caregivers and effective rehabilitation services. Many reviewers explicitly commend the nursing and therapy teams, calling out individual staff members (Vivian and occupational therapist Kristian) for exceptional care. Multiple summaries describe the PT/OT/Speech therapy services as well-trained and effective, and several say the facility delivered 'best care' or 'excellent experience.' Reviewers frequently note that staff go above and beyond, that the facility is well-run, and that outside professionals (an ER doctor) have trusted the facility — all strong indicators of solid clinical capability.

Cleanliness and the physical environment receive mostly favorable comments: several reviewers say the interior is extremely clean and well-maintained, even if the building looks older from the outside. Dining is another consistent positive — food is described as good or very good in multiple summaries. Social programming and activities are also cited as strengths, with reviewers mentioning great social programs and expressing satisfaction with resident engagement opportunities.

However, there are significant and recurring negative themes that should not be overlooked. A number of reviews report troubling instances of poor care or neglect: long delays in responding to call lights (one reported as long as 35 minutes), CNAs allegedly ignoring patients, and at least one report of a resident being left in a wet diaper all day. Some reviewers describe dirty conditions and an unhelpful or dismissive director of nursing who reacted inappropriately (laughing), which raises concerns about supervision and complaint handling. There are also reports of rude front desk staff and lack of phone access for patients, which affect family communication and resident autonomy. Notably, one nurse (named Bella) is described as handling a patient aggressively, using a mocking tone, and ignoring family requests — illustrating that negative experiences are sometimes tied to specific staff behavior.

A clear pattern emerges of overall strong clinical and rehabilitative services complemented by caring staff, but with occasional, sometimes serious lapses in basic caregiving, communication, and front-line professionalism. This inconsistency suggests variability across shifts, units, or individual staff members rather than uniformly poor or uniformly excellent performance. The recurring positive comments about therapy, nursing excellence, cleanliness, and food indicate many residents receive high-quality care; concurrently, the reported incidents of neglect, poor communication, and dismissive management responses point to gaps in training, supervision, or staffing levels that the facility should address.

For prospective residents and families: the facility shows many strengths worth considering—especially for rehabilitation and therapy needs—but you should investigate those inconsistent areas during a visit. Ask about call-light response protocols and averages, staff training and supervision, complaint escalation processes, and how the facility addresses reported neglect or staff misconduct. Observe meal service and an activity, request to meet the therapy team and director of nursing, and inquire directly about phone access for residents. For the facility: the reviews suggest prioritizing consistent staff training on empathy and communication, reviewing call-response times and CNA practices, improving front-desk customer service, and ensuring management takes family complaints seriously and documents corrective actions.

In summary, Providence St. Elizabeth Care Center - North Hollywood receives many positive remarks for its compassionate caregivers, effective therapy programs, clean interior spaces, and good food, making it a strong option for rehabilitation and attentive nursing care. At the same time, several serious negative reports—ranging from neglectful caregiving and slow call responses to poor communication and isolated instances of abusive or dismissive staff behavior—indicate variability in resident experiences. These mixed signals mean the facility can provide excellent care, but families should perform targeted due diligence and the facility should act on the shortcomings reported to ensure consistently high standards for all residents.
