    Caring professional staff; smooth transition

I placed my mom here and was very pleased with the professional, caring staff - RNs/CNAs, aides, PT/OT (Caleb) and wound-care nurses provided attentive, timely care and helped with daily tasks and a smooth transition home. The facility is clean, well-run, offers excellent activities (Ballroom Dance/A Dance To Remember), varied dining and 24/7 responsive administration. Overall a very good, comforting experience I would recommend.

    Summary of reviews

Overall impression: Reviews of Petaluma Post-Acute Rehab are predominantly positive about the clinical and rehabilitative care, with frequent praise for the nursing staff, aides, and therapy teams. Many families highlighted strong outcomes from physical and occupational therapy, including specific positive mentions of individual therapists, and noted successful transitions back to home. The facility is commonly described as clean and well maintained, with a convenient location and an administrative team that can support admissions and discharge planning.

Care and staff: Reviewers frequently described staff as professional, attentive, and caring. Registered nurses, CNAs, aides, and therapists are characterized as competent and hardworking; wound-care nursing and pain-management were singled out as strengths. Several accounts emphasized personalized assistance with activities of daily living (laundry, showering, meal help), and the overall sense that staff made residents comfortable. At the same time, some families raised concerns about inconsistent staff responsiveness (for example, delayed responses to call systems) and uneven communication or tone from certain employees. These issues appear to be operational rather than universal, and some reviews indicate improvements under newer leadership.

Dining and activities: Food and dietary service elicit mixed feedback. Multiple reviewers praised the kitchen for variety and accommodation of special diets, including snacks between meals, while others described the dining experience as unsatisfactory. Prospective residents should expect variability in meal quality and may wish to sample menus or ask about current culinary staffing. Activities programming is a consistent positive: the facility offers an active calendar with organized social events, and specific programs such as a ballroom-dance class were highlighted as enjoyable and well run.

Facilities and operations: The physical plant is generally described as clean and attractive. A few reviewers requested more private-room options, indicating limited private-room availability at times. Operational weaknesses cited across reviews include discharge/transportation coordination gaps and variability in responsiveness across shifts or units. There are also references to past regulatory or safety-code citations and to isolated allegations of care-delivery lapses; some families noted that new leadership has addressed problems and produced positive changes. It is reasonable for prospective residents to ask for the facility’s most recent inspection history and to inquire about average response times to call systems and discharge logistics.

Bottom line: Petaluma Post-Acute Rehab appears to offer strong clinical and rehabilitative services, supported by an engaged staff and active programming. The main caveats are operational consistency — especially around dining, call-response reliability, discharge transportation, and private-room availability — and an intermittent history of regulatory attention noted by reviewers. When evaluating the facility, families should prioritize direct observation of staff–resident interactions, ask about current management initiatives, review recent compliance records, and clarify discharge and transportation arrangements.
