Vineyard Post Acute sits at 101 Monroe Street in Petaluma, California, and offers post-acute rehabilitation for seniors who need help after illness or big medical events, with a focus on safety, healing, and wellness that comes from careful, hands-on care by licensed experts who know how to handle even high-acuity cases and infectious diseases like COVID-19, thanks to infection prevention rules and steady use of PPE, which keeps things safer for everybody, and the staff go out of their way to stay attentive, kind, and knowledgeable, making sure each person's medical and personal needs are carefully met through individualized care plans, with skilled nursing services, medical supervision, and full rehabilitative therapy-including physical, occupational, and speech therapy that starts within 24 hours after admission and keeps going seven days a week, so no one misses a day of progress. There are 99 beds at Vineyard Post Acute, with single and double rooms that have enough space for residents to bring in their family photos and favorite blankets to make things feel more familiar, plus a patio in the back-some call it sunny and shady-where folks can sit outside, get some air, and join activities if they feel like it, which adds a touch of comfort and helps with healing. Meals come in a restaurant-style setting, with a full-time Registered Dietitian making sure any dietary needs get handled, and an off-site pharmacy handles medicines that the facility stores securely. People say the staff are exceptional and compassionate and that there's a warm approach to care, though some mention that certain areas beyond the lobby can get a little dirty and might not be as well-lit as they could be, and bathroom cleanliness sometimes gets comments, which might matter if that's something you care about. Residents stay engaged with daily activities, structured social programs, and a music program offered every day, and you'll always find the activity calendar posted in rooms, giving plenty of ways to spend the day and meet others. There's a lobby, a safe environment, and a front desk open Monday through Friday from 8 am to 7 pm and on weekends from 9 am to 6:30 pm, and with 24/7 admission support, folks who have questions about insurance or how to get started can expect help any time they need it. Vineyard welcomes residents with Alzheimer's disease, offers complex medical help, and works to promote independence through amenities like the patio, comfortable rooms for rest, and staff who focus on what each individual actually needs instead of following a one-size-fits-all plan. There are virtual photo tours and overview videos for people wanting a better look before they visit, and the facility holds a CMS 5-star rating, which some families might find reassuring, but as with any place, it's a good idea to take a tour yourself and see if the atmosphere and the daily life fit what you're hoping for.

Excellent therapy, but staffing issues
I received excellent rehab - therapists and many nurses/CNAs were skilled, caring and helped me make real progress. The facility is clean, bright and homey, administration and some staff were communicative and engaging. That said, chronic understaffing led to long waits, call-light delays, missed or delayed meds and occasional rude or neglectful behavior. Food and portions were inconsistent, and record-keeping/communication (discharge papers, med changes) was hit-or-miss. Overall I felt confident in the therapy and many caregivers and would recommend it for rehab - but be prepared to advocate for safety and timely care.
