    Compassionate staff, clean facility, recommended

I'm very satisfied with the care my mother received - compassionate, competent staff (nurses, CNAs, PTs and doctors) who were attentive, quick to spot issues, and open to family meetings. The facility is clean, bright and well-run with daily activities, good therapy, helpful customer service and convenient amenities; overall a very good place I'd recommend.

    Summary of reviews

Reviewer feedback for Skyline Health Care Center paints a mixed picture. Many families and visitors highlight compassionate direct-care staff — CNAs, nurses, and select managers receive repeated praise for being caring, professional, and responsive. Several accounts describe knowledgeable RNs, coordinated interdisciplinary teams (including an effective case manager and therapists), and positive rehabilitation outcomes when therapy was consistently provided. Common positive notes include bright, well-lit communal spaces, daily room cleaning, active group activities, flexible visiting hours, and functional amenities such as on-site transportation and ample parking.

However, significant operational weaknesses appear across multiple reviews. Staffing is described as inconsistent, with notable differences between day and night shifts and between regular and agency staff. This inconsistency is linked to delays in responding to call bells, missed or delayed medication administration, and variable wound and incontinence care practices. Reviewers cite gaps in medication management, including timing errors and documentation problems, as well as delays in escalation to primary providers. These issues contribute to family concerns about clinical reliability and continuity of care.

Facility operations and environment concerns recur as well. Several reviewers report sanitation and pest-control concerns in specific areas, shortages or delays in linens and essential supplies, and maintenance items left unresolved for extended periods. Safety systems such as call buttons and some safety mattresses or bed alarms were described as unreliable in certain accounts, and overcrowded rooming (including small shared rooms) was identified as a limitation for privacy and family visits. Infection-control issues, including accounts tied to outbreak events, increased family concern about the facility's ability to prevent and manage communicable disease spread.

Communication and conduct issues are another consistent theme. Language barriers between some direct-care staff and residents/families were noted, which affected care interactions and understanding of instructions. Several reviewers described difficulties obtaining timely, clear updates from nursing or management; some families found follow-up after serious events, including end-of-life situations, to be inadequate. There are also multiple descriptions of variable staff conduct and tone; while many families praised respectful interactions, others reported instances where staff communication felt dismissive or antagonistic, particularly on night shifts or with registry personnel.

Dining, activities, and amenities draw mixed reviews. Food quality is generally described as adequate but not exceptional, with limited diabetic-diet options noted by some families. The activities program and communal offerings receive positive comments for engagement and variety, though a subset of residents did not receive planned therapy services consistently. The building’s character — older construction with skylights, a piano, and aviary features — is seen as pleasant by some and dated by others; physical condition and cleanliness appear to vary by unit.

Taken together, the pattern is one of variability: the facility can provide attentive, coordinated care and a warm social environment when staffing and operational systems function well, but recurring weaknesses in staffing consistency, medication and wound-care processes, sanitation and maintenance responsiveness, and family communication create meaningful risks and dissatisfaction for other families. Prospective residents and families should consider visiting at different times (including nights and weekends), assess staffing ratios and language support, review medication- and wound-care protocols, examine common-area cleanliness and pest-control practices, and ask for documented therapy and infection-control policies to better gauge whether the facility’s strengths align with their needs.
