    Attentive staff helped me recover

I spent two months recovering from a mini-stroke and received attentive, professional care - nurses like Emily and therapists helped me walk again. Admissions and long-tenured leadership made the smooth transition easy, staff (Lucy, Yasmin, Marcos and team) were friendly, responsive and genuinely caring, and my bright, clean room with outdoor spaces made visits pleasant. Overall a very positive, well-managed place I'd recommend, with a little room to keep getting even better.

    Summary of reviews

Village Square Healthcare Center elicits highly polarized experiences. Many families and residents describe strong clinical strengths in rehabilitation and individualized caregiving: reviewers frequently highlight an experienced geriatric and orthopedic therapy team, effective physical and occupational therapy programs, and nurses and aides who provide compassionate, patient-centered attention. Admissions staff and social work are often characterized as helpful and responsive, and the physical environment โ€” bright, large rooms with terrace access and walking areas โ€” is commonly praised. The facilityโ€™s activities program, monthly outings, and dining structure are strengths for residents seeking engagement and rehabilitation-focused stays.

Conversely, a substantial pattern of operational concerns recurs across reviews. Staffing inconsistencyโ€”particularly on nights and weekendsโ€”appears to affect response times to assistance requests and call lights, and several accounts describe delays in medication or therapy administration. These issues are linked to family concerns about timely clinical oversight, including delayed provider contact and slow recognition of acute changes in some residents. Sanitation and odor concerns in particular areas, plus intermittent facility maintenance problems (for example, elevator or phone reliability), are noted as detractors from an otherwise well-appointed physical plant.

Communication and care transitions are another recurring theme. Families report variable transparency and follow-through during discharge planning, handoffs, and post-acute coordination with physicians and outside providers. Leadership stability also appears uneven: some reviewers describe visible improvement efforts under new management, while others cite turnover and inconsistent oversight. There are isolated but serious claims involving staff conduct and financial or safety incidents; these individual allegations should prompt prospective families to seek documentation, ask about incident investigation processes, and review state inspection and ombudsman records.

In practical terms, Village Square can offer high-quality rehabilitation outcomes and attentive caregiving in many cases, but reliability of routine operationsโ€”staffing, medication/treatment timing, sanitation, maintenance, and family communicationโ€”appears inconsistent. Prospective residents and families should perform an in-person tour, ask specific questions about staffing ratios and weekend coverage, request protocols for medication administration and response-time expectations, review infection-control and sanitation practices, confirm cohorting policies for cognitive impairment versus rehab populations, and inquire about recent quality metrics, leadership tenure, and any regulatory actions. Those steps will help weigh the facilityโ€™s clinical strengths against the operational risks identified by multiple families.

    About Village Square Healthcare Center

Village Square Healthcare Center sits in San Marcos and has 118 certified beds, though on average about 112 residents stay there each day. The place offers a mixture of independent and assisted living, along with both short-term rehabilitation and long-term skilled nursing care. Residents can get help with bathing, dressing, eating, walking, transferring, toileting, and moving around in a wheelchair, plus there's mobility assistance and personal care like grooming, and meals with dietary plans managed by registered dieticians following doctor's orders. The rooms feel pretty spacious, with both private and semi-private options available, and there's a resident and family area for people to gather and visit. For specialty medical needs, they have in-house wound care, pain management, enteral therapy, and intravenous therapy, all managed by skilled nurses 24 hours a day, every day, under the supervision of in-house physicians.

Staff includes caregivers, support staff, medical specialists, nurses, nutritionists, dietitians, and social workers, all with background checks, drug screening, immunizations, and reference checks. Residents get psychological services, get to meet with social workers, and can join in social and recreational activities, since there's a calendar filled with things to help people stay active and connected. They have in-house transportation with a friendly driver. For folks who want to stay at home, Village Square sends staff to help with light housekeeping and daily support.

Village Square Healthcare Center is for-profit, operated by a limited liability company, and has been affiliated with Mariner Health Care. It's certified for both Medicare and Medi-Cal, and contracts with most managed care insurance groups, though it hasn't shared service or amenity details beyond the basics. Total reported nurse staffing hours sit at 4.02 per resident per day, under the state average of 4.5, with a nurse turnover rate of 42.4%, a bit higher than the state's 40% average. The state reports show 54 total deficiencies, including 4 tied to infection control, with issues noted for failure to keep residents and families informed of injuries or decline, problems protecting resident dignity and self-determination, and violations of infection control standards. Inspections have sometimes been delayed, with the last standard inspection happening over two years ago, and there's no public info about management control or who manages the staff.

The center takes infection control seriously, with a special COVID unit for anyone suspected of or confirmed to have COVID-19, plus strong policies using personal protective equipment, cleaning, visitor and employee screening, and social distancing. They closely watch residents and staff for symptoms and adjust visitor policies as guidance changes. Village Square also provides in-house rehabilitation services like physical, occupational, and speech therapy from trained rehab workers, plus dietary support, psychological services, and restorative nursing. Social services help with emotional, spiritual, recreational, and social needs, with activities adjusted to suit individual interests. Residents get comprehensive care, with detailed treatment plans shaped by each person's needs.

No extra details are available about other amenities, special features, or extra care programs, but the center is part of an upscale senior living campus and aims to foster a supportive place for both residents and employees.
