    Compassionate staff, spotless facility, reassuring

I'm very pleased with Vermont Care Center - compassionate, attentive staff (nurses, CNAs, night nurses and outstanding PTs) helped my mom regain strength and made us feel reassured. The facility is spotless and welcoming, meals and communication were excellent, and the social worker and office team were supportive and clear throughout the stay.

    Summary of reviews

Overall impression The Vermont Health Care Center elicits a broadly mixed-to-positive picture. Many families and residents praise the facility for its compassionate direct care, strong rehabilitation services, and a clean, wellโ€‘organized environment. At the same time, there are recurring operational concernsโ€”chiefly around staffing consistency, medication and painโ€‘management timeliness, and certain communication gapsโ€”that prospective residents and families should consider alongside the facilityโ€™s strengths.

Care and staff Clinical care and frontline staff receive frequent positive mention: nurses, CNAs, night staff and therapists are commonly described as kind, attentive and skillful. Physical and occupational therapy are repeatedly highlighted as effective in helping residents regain strength and function. The social work team is a clear strength in many accounts, characterized as proactive, compassionate and helpful during admissions and discharge planning. However, some reviewers describe variability in staff conduct and bedside responsiveness; this appears linked in part to shift staffing levels and to perceptions of changing staff experience after an ownership/staff turnover.

Dining and activities Dining receives largely favorable commentsโ€”many reviewers praise meal quality and the kitchenโ€™s willingness to accommodate preferencesโ€”though a minority note inconsistent meal quality across different stays. Activity programming is present (singing groups, religious services, social interaction), and some residents find a warm, familyโ€‘like atmosphere. Still, several families expressed that activity offerings did not sufficiently engage every resident or match individual interests, indicating unevenness in programming or participation.

Facilities and cleanliness The physical plant is consistently described as clean, organized and easy to navigate. Many reviewers noted good upkeep, a pleasant environment, and effective maintenance. A small number of accounts raise sanitation or belongingsโ€‘handling concerns following adverse outcomes; these reports appear exceptional but are serious enough that families noted regulatory followโ€‘up in at least one instance.

Management, communication and transitions Communication is a mixed area: some families praise clear, direct communication during admissions and good ongoing updates, while others report unanswered phone calls, slow responses, and difficulty reaching clinicians. Transition logistics also show variabilityโ€”examples include medication availability at admission and coordination between the sending hospital and facility. Several reviewers perceive a decline in consistency and quality following staffing or ownership changes, which suggests operational variability over time.

Notable patterns and considerations Strengths most consistently cited are compassionate direct care, effective rehabilitation, strong socialโ€‘work support, and a clean, wellโ€‘kept facility. The principal operational weaknesses are inconsistent staffing levels (affecting response times), uneven medication and painโ€‘management timeliness, variable physician availability, and communication gaps with families. Prospective residents and family members should weigh the facilityโ€™s solid rehabilitation and socialโ€‘work capabilities and its generally positive care culture against these operational inconsistencies. When evaluating the facility in person, advisable focal points include: staffing ratios and typical response times, medicationโ€‘management protocols, physician coverage, activity schedules tailored to the resident, and specific examples of how the facility handles admissions and transitions.

    About Vermont Health Care Center

Vermont Healthcare Center is a large, for-profit skilled nursing facility that operates with a capacity of 200 beds. It provides both short-term rehabilitation and long-term care services, participating in Medicare and Medicaid programs. As a standalone community, it does not form part of a continuing care retirement community. Residents benefit from an average of 4 hours and 43 minutes of nurse staffing per resident each day, with consistent staffing levels maintained throughout the weekdays and weekends. The facility places an emphasis on maintaining comprehensive care, aiming to assist residents not only with medical needs such as medication administration, but also with daily living tasks including eating, dressing, and mobility support.

A significant portion of residents at Vermont Healthcare Center are able to return home following short-term rehabilitation, reflecting the centerโ€™s focus on post-acute care for those recovering from hospital stays after illnesses, surgeries, or injuries including stroke and heart attack. The proportion of residents who required emergency room visits during their rehabilitative stay was 9.7%, while 7.1% experienced infections that necessitated hospitalization. Additionally, the center fosters a preventive care environment, exemplified by a 97.9% flu vaccination rate among residents, and notable success with 94% of long-term care residents maintaining self-care abilities for common activities.

Facilities at Vermont Healthcare Center provide for the needs of individuals with daily and ongoing medical care requirements, and there are systems in place to manage aspects such as pain control, infection prevention, and nutritional support, with a qualified dietician involved in food and nutrition services. The center offers routine and emergency dental care to support residentsโ€™ overall well-being. Furthermore, it endeavors to maintain safe and dignified living conditions for all residents, making ongoing efforts to protect individualsโ€™ rights, provide opportunities for self-determination, and ensure the least restrictive environment appropriate to each residentโ€™s care needs.

Periodic health, fire, and safety inspections are conducted at Vermont Healthcare Center, with attention paid to areas such as infection control, prevention of abuse, accident prevention, medication management, and maintenance of resident records. Staff members receive training on a range of topics pertinent to elder care, including dementia care and the protocols surrounding reporting suspected abuse or neglect. Over recent years, the facility has addressed a range of areas identified through inspection processes, demonstrating a commitment to developing and implementing care plans tailored to the specific needs of each resident. The center also employs or obtains services from licensed pharmacists to oversee the safe storage and usage of medications within the facility.

Nurse staffing is supplemented by structured routines designed to support activities of daily living, range of motion, continence care, and pain management. Vermont Healthcare Center focuses on minimizing fall risks and accident hazards by adequate supervision and maintaining a monitored environment. The facility seeks to provide as much support as necessary to help residents retain their independence and dignity while ensuring their health and safety are prioritized through skilled, responsive care.
