    Improved care, attentive and grateful

I've come back since my last visit and it's noticeably improved. The staff are professional, warm, and attentive - nurses like Razma, Alvard Sargisyan and Maricel went the extra mile and my mom was genuinely cared for. The facility is very clean, homey and well decorated with courtyards and good meals; rehab and the team helped my mom heal and concerns were handled quickly, so I'm grateful and confident in their care.

    Summary of reviews

Reviewer experiences of California Healthcare & Rehabilitation Center are mixed, with clear patterns of both strong clinical and hospitality elements and significant operational shortfalls. Positive comments emphasize attentive nursing, an engaged rehabilitation team, a generally clean and well-decorated environment, and supportive social work and administrative help. Several families credited the facility with meaningful rehab progress, appreciated the dining options and accommodation of special requests, and described a warm, family-like atmosphere in parts of the center.

Care quality shows substantial variability. Many accounts describe compassionate, responsive nursing and effective therapy that supported recovery and discharge goals. Concurrently, other accounts describe delays in responding to call lights, concerns about monitoring wounds and injuries, and questions about medication practices that may have affected alertness. These differences suggest inconsistent clinical oversight and documentation practices across shifts or units.

Staffing and communication are recurring themes. Positive reports note helpful, friendly staff and individual caregivers who go "the extra mile." In contrast, other reports raise concerns about staff conduct, tone of communication with families, and variable supervision. Families also described difficulty obtaining clear answers during escalations and challenges with transfer or discharge coordination, which points to gaps in transparency and grievance handling.

Facility operations and physical plant observations are similarly mixed. Many reviewers praised cleanliness, regular linen changes, and well-kept common areas including courtyards and dining halls. However, there are repeated operational issues: malfunctioning equipment (beds, IV stands), pest-control concerns in some areas, and inconsistencies in laundry and personal-property handling โ€” including allegations of missing items. These operational weaknesses can materially affect resident comfort and family trust.

Dining and activities received both praise and criticism. Several families enjoyed meals and found staff accommodating with special requests; others described slow service or unsatisfactory activity programming. Rehabilitation services were often highlighted as a strength when well-executed, but some families found therapy insufficiently individualized to the residentโ€™s specific injuries or goals.

Taken together, the pattern suggests that individual staff members and specific units deliver high-quality, compassionate care, while system-level inconsistencies undermine reliability. Prospective families should weigh the positive reports of rehabilitation success, cleanliness, and supportive staff against documented operational risks: inconsistent responsiveness, clinical monitoring gaps, equipment maintenance issues, personal-property management, pest-control concerns, and variable visitation/communication policies. When considering this facility, families should ask targeted questions about staffing levels and supervision, clinical monitoring protocols (wound care, medication review), equipment maintenance programs, pest-control measures, property-management practices, and the facilityโ€™s formal escalation and transfer procedures. Touring the specific unit, meeting the nurse manager and therapy team, and clarifying visitation and escalation processes will help assess whether the centerโ€™s strengths align with a prospective residentโ€™s needs.

About California Healthcare & Rehabilitation Center

California Healthcare & Rehabilitation Center sits under the management of Longwood Management Corporation and runs a range of care programs to help meet different needs, so folks find both rehabilitation and skilled nursing here, as well as sub-acute services and respite stays for those who need short-term help, which works out for families who are looking for temporary care options, and with 201 certified beds and about 190 residents on average each day, there's always something going on but it doesn't feel crowded. Nurses put in about 4.66 hours of care per resident daily, and while the nurse turnover rate sits at 36.1%, there is always a professional and caring staff on hand, available 24 hours a day, seven days a week, and they do try to promote as much independence as possible so folks can keep up their quality of life. Residents get tailored therapy services, including rehabilitation therapies designed to help each person reach their highest possible level of living and comfort, and the staff also covers comprehensive healthcare, so medical needs get managed right there on site. Amenities and facilities are set up for comfort and convenience, which helps people feel a bit more at home while they're working on recovery or just settling in for ongoing care. Inspection reports are made available, so you can look into the details of any past problems; there have been some deficiencies, like issues with pressure ulcer care and proper assessment and planning for residents, and some inspections noted deficiencies with potential for harm, so it's important to stay aware of those. Even with these concerns, the center keeps a focus on helping everyone recover well, live with dignity, and have access to care that matches their needs, and all the basic comforts are there so folks can spend their days in as much comfort as possible.
