    Compassionate attentive care, comfortable community

I'm very pleased with the compassionate, attentive staff and strong therapy team - they treated my mom like family, helped her regain mobility, and coordinated care smoothly (including VA/respite needs). The facility is clean and home-like with beautiful views, lively/varied activities (seasonal events, baking, piano), spacious dining, and generally good meals. Admissions and administration were responsive and welcoming; overall a comfortable, caring community I'd recommend.

    Summary of reviews

Reviewer feedback for Silver Heights Skilled Nursing and Rehabilitation describes a facility with clear strengths in frontline caregiving and rehabilitation, alongside operational and administrative inconsistencies. Many families and residents praise the direct-care staff โ€” CNAs and nurses are frequently described as compassionate, attentive, and instrumental to positive clinical and functional outcomes. Several accounts describe meaningful therapy progress, successful short-term rehabilitation stays, and helpful coordination with the VA for veteran respite and procedures.

Activity and social programming are commonly identified as strengths. Reviewers note a broad mix of daily activities, seasonal programming (for example, holiday events), and opportunities for in-room engagement such as baking. Communal areas โ€” including a spacious dining room with a piano and pleasant views โ€” contribute to a generally welcoming atmosphere when the facility is performing well. Admissions staff and some administrative team members are singled out for warm and helpful interactions, and multiple accounts indicate an improved culture and environment under recent management changes.

At the same time, reviews reveal recurring operational challenges. Staffing levels are a frequent concern: reliance on temporary agency staff, understaffing during shifts, and short CNA-to-resident coverage are linked to delays in toileting, bathing, and other direct-care tasks. Leadership instability (notably frequent changes in director-level positions) is repeatedly cited and appears to affect continuity of care, communication, and staff morale. Reviewers also describe inconsistent therapy scheduling and variability in the rehabilitation experience depending on staffing and the assigned therapists.

Facility-level systems and infrastructure issues emerge in several areas. Cleanliness and odor concerns in some units, inconsistent housekeeping (for example, floor cleaning and linen changes), and pest-control process gaps were reported. Dining receives mixed reviews: some meals are praised and there are flexible menu options, while others describe inconsistent preparation, texture problems, and reduced flavor associated with special-diet controls. Personal-property management and tracking also show weaknesses, with instances of misplaced items prompting concerns about storage and accountability processes. Safety checks and equipment oversight were raised as operational gaps in a few accounts (for example, unsecured tubing or oxygen equipment), indicating areas for process reinforcement.

Administrative and financial communication is another theme. Some reviews describe improved transparency under new management, while others document significant breakdowns in billing communication, Medicaid coordination, and staff human-resources practices โ€” including allegations of aggressive billing actions and confidentiality concerns. These are serious points for prospective families to clarify during intake. Overall, the pattern of feedback is polarized: many families report excellent, compassionate care and positive rehabilitation outcomes, while others experienced notable lapses tied to staffing shortages, cleanliness, and administrative dysfunction. Prospective residents and families may benefit from direct questions about current staffing ratios, leadership stability, property- and billing-controls, therapy schedules, and housekeeping protocols when evaluating the facility.

About Silver Heights Skilled Nursing and Rehabilitation

Silver Heights Skilled Nursing and Rehabilitation sits in Castle Rock, Colorado, right near the community called Castle Rock Care Center, and you'll find it's managed by Madison Care Partners and associated with Genesis Health Care, which means it's got a lot of healthcare experience behind it. The building has handicap accessible spaces, safety features like sprinkler systems, and secure guest parking, so getting around's not too hard for folks who need a little extra help or want to feel safe. In the rooms, there are kitchenettes for those who prefer making a snack or simple meals, with housekeeping, washers and dryers, and ongoing maintenance all included, making daily life easier for everyone, whether they're there for respite care or a longer stay.

Residents get help with things like bathing, dressing, grooming, walking, and using wheelchairs, so daily routines aren't as stressful, and there are personal care assistants around for those who need a hand. There are nurses and medical support day and night, wound care, medication help, and access to specialized medical services like podiatry, dialysis, and IV therapy, which is good for folks who need regular treatment. Skilled nursing teams develop care plans for each resident, involving nurses, social services, activities, dietary staff, and sometimes doctors, so everyone's needs get attention. Specialized programs are available for those living with neurological conditions, memory issues, joint replacements, amputations, speech and swallowing disorders, pain, and other health issues-there's gait and mobility training, aphasia treatment, help with feeding tubes, and pulmonary care.

Social life matters too, so residents get to join in on lots of activities, from arts and crafts to fitness, health and wellness programs, and games in the activities room, plus there's a dining room, salon/barbershop for haircuts, cable TV, and the whole place has WiFi for keeping families connected. Pet lovers don't have to leave their animals behind, as Silver Heights is pet-friendly, and for those who like to feel the fresh air, there are nice views around the building as well. There's transportation for appointments or trips and therapy teams for full-time physical, occupational, and speech therapy, with group or individual counseling (including for PTSD and addiction) for those who need mental health support.

Silver Heights covers a wide range of care, including assisted living, independent living, memory care, skilled nursing, hospice, and short-term rehab for people recovering from surgery, stroke, or injury, always aiming to help each person regain as much independence as possible. The place accepts long-term care insurance, Medicare, and Medicaid, and runs programs to make sure no one's left out, focusing on health services that promote independence and fairness for people of every background. There's a focus here on dignity, safety, and supporting everyday living, whether you're staying short-term or need 24-hour nursing care, and visitors are welcome to schedule a tour and meet the staff and residents to get a better feel for how Silver Heights operates and what it can offer for everyday life.
