I felt my loved one received compassionate, attentive care from a dedicated, patient staff-nurses, CNAs, social worker, activity and housekeeping teams were kind and thoughtful. The warm community, crafts, music and supportive management made transitions peaceful. Overall a very caring, well-run place I would recommend.

    Summary of reviews

Reviews of Sierra Post Acute present a mixed picture with clear strengths in hands-on caregiving and community life alongside several operational concerns. Many accounts highlight compassionate, attentive staff — nurses, CNAs, physicians and social workers are frequently described as caring, helpful and supportive. Families report coordinated hospice transitions and peaceful end-of-life experiences when those services were involved. The facility’s activity program, including crafts, music and group gatherings, and consistent housekeeping contribute to a sense of community for long-term residents.

At the same time, multiple comments suggest variability in how those positive attributes are delivered. Some reviewers described inconsistent staff conduct and communication tone, and there are indications of uneven responsiveness and timeliness of care in certain situations. These patterns suggest that day-to-day resident experience may depend on staffing, shift coverage or individual team members.

Safety and facility operations were another recurring theme. Reviewers raised concerns about the lack of a visible security presence, neighborhood safety, and instances of residents accessing outdoor areas without close supervision. These observations point to potential gaps in elopement-prevention measures and perimeter/security monitoring that prospective families will likely want to evaluate directly.

Management and administrative processes receive mixed feedback. Several notes praise strong leadership and a recent management team change, yet others cite formulaic public responses and perceived prioritization of billing or value over care. There are also specific concerns about how end-of-life administrative steps and family communication are handled, including delays in postmortem transitions; these indicate opportunities to strengthen transparency and process consistency.

Information about dining quality is limited in the available summaries; housekeeping is mentioned positively. Given the split between strong, personalized caregiving reports and serious operational concerns, prospective residents and families should arrange an in-person visit, meet the nursing and administrative teams, and ask targeted questions about staffing ratios, security/elopement safeguards, end-of-life procedures, billing practices, and recent management changes. Direct observation and specific written policies will help clarify whether the facility’s strengths align with a family’s priorities and whether noted operational gaps have been addressed.

    About Sierra Post Acute

Sierra Post Acute sits on Depew Street in Lakewood, Colorado, offering skilled nursing home care in a place that tries to feel like home, and they really do like to treat people with kindness and respect, even when things get hard, because the staff knows that the work isn't always easy but it matters. The facility provides care for people who need help after a hospital stay or who have complex health problems, and they're well set up for both short-term rehab and longer-term care, with an expansive rehabilitation gym that's newer and well-equipped, and licensed therapists who guide physical, occupational, and speech therapy every day of the week. Nurses are on staff 24 hours, and there's always someone at the front desk during the week, with admission help available any time, which is helpful for families who need to ask questions about what kind of care or insurance is available, and everybody-from nurses to administration-works together to support residents safely and comfortably.

The place has assistive devices, secure medication storage, and on-site laundry so residents can keep their clothes and personal things clean and close, and they really do encourage folks to bring family photos and blankets to make rooms feel comfortable, which makes a difference when you're staying somewhere unfamiliar. The courtyards are inviting, and there are daily activities like arts and crafts, music, and even community trips, with a calendar posted in every room so no one misses out, and the dining program serves meals planned by a Registered Dietitian, adjusting for special diets as needed.

Sierra Post Acute takes safety seriously-there's a dedicated infection prevention team with a nurse, and they're able to handle high-acuity patients and those dealing with infectious diseases like COVID-19 while following proper PPE and protocols. The staff, including doctors, therapists, and dietitians, meet weekly to review each resident's progress, and doctors usually see new patients within 24 hours of arrival, which can bring peace of mind to families. They cover health topics with an online library, symptom checker, and mental health resources, and help connect families to community resources on opioid use, elder abuse, fall prevention, and more.

They focus on culturally responsive, trauma-informed, and person-centered care, aiming to create an inclusive and comfortable environment for all residents and families. Sierra Post Acute is part of the Colorado Health Care Association District I and works with HRSA-supported health centers and keeps a hand in health care policy, which shows a broader commitment to the community and the field. The building's always clean and well run, and visiting is pretty easy, with open hours every day and some flexibility for special circumstances, making it possible for loved ones to come by when it matters most.
