    Outstanding rehab, dialysis, spotless facility

I had a very positive experience at Boca Circle. The facility is spotless and recently renovated, the nursing and rehab teams (PT/OT/ST) were outstanding and communicative, and onsite dialysis/therapy under one roof made care convenient. Staff from admissions to social work (Maria, Vanessa, Clement, Laurine, Sergio) were warm, professional, and responsive - my mom improved quickly and we felt well cared for.

    Summary of reviews

The reviews for Boca Circle Rehabilitation Center are strongly mixed, with a clear pattern: the facility is frequently praised for its rehabilitation and therapy capabilities and criticized for several operational and clinical-continuity issues. Many families highlight excellent therapy outcomes — physical, occupational, and speech therapy are repeatedly described as knowledgeable, effective, and instrumental in short-term recovery. The presence of an on-site dialysis center and integrated services is a distinct advantage for patients who need multiple therapies without repeated transport; several reviewers specifically cited the convenience of receiving dialysis, therapy, and medical care in one building.

Care quality and clinical oversight are areas of divergence between reviewers. Positive accounts describe compassionate nursing, engaged CNAs, and proactive social-work support that provide families with reassurance. Conversely, other accounts describe inconsistent nursing care quality, delays in medication administration, and limited physician availability. There are also multiple concerns about repositioning and wound-prevention practices and at least one serious wound-related outcome referenced; these indicate gaps in pressure-injury prevention protocols and routine turning. Reviews also raise concerns about emergency response and transfer processes following falls or acute events, and some mention equipment failures and unreliable call-button responsiveness.

Staffing, communication, and culture show variability across shifts. Several reviewers singled out individual staff members and managers (including social-work and therapy directors) for exemplary communication, advocacy, and responsiveness. At the same time, recurring themes include high staff workload, uneven performance between day and night shifts, and failures in message relay to families. These communication gaps extend to family notification about clinical changes and discharge planning. Medication-management lapses and inconsistent follow-through on care plans are additional operational weaknesses families should address proactively during admission and care-planning discussions.

Dining, activities, and facility environment are similarly mixed. The facility has undergone renovations and several reviewers describe clean, attractive common areas, a welcoming admissions process, and an active activities program. Some families praise the food quality, while others describe bland or poorly timed meals — indicating variable dining execution. Housekeeping and room-cleaning practices receive positive mentions from some families but also criticisms from others, including delayed linen changes and sanitation concerns; these appear to vary by unit and shift.

Administrative and billing issues were raised by multiple reviewers. Concerns include perceived lack of transparency around charges, pressure regarding payer source options, and charges for equipment rentals. Conversely, some families found administrators responsive and professional. There are also isolated but serious allegations concerning loss of personal property and problematic clinical outcomes; these items represent high-severity concerns that families should investigate directly with facility leadership and through appropriate oversight channels.

Overall, Boca Circle appears to offer strong, potentially high-value short-term rehabilitation and integrated dialysis services supported by capable therapists and some compassionate staff. However, variability in nursing consistency, staff levels, clinical oversight, communication with families, sanitation practices, and administrative transparency are recurring themes. Prospective residents and families should weigh the facility’s therapy strengths and on-site services against these operational risks, ask targeted questions during touring and admission (staffing ratios, wound-prevention protocols, emergency-transfer procedures, medication administration checks, billing practices), and request named contacts (therapy director, social worker, or administrator) to monitor care continuity if choosing this facility.

    About Boca Circle Rehabilitation Center

Boca Circle Rehabilitation Center cares for people who need short-term or long-term help, and you'll find the place welcomes everyone because they believe strongly in non-discrimination and equal opportunity for their workers and residents. They've got 120 certified beds and see about 107 people there on an average day, so you can tell the place stays active but not overcrowded, and you end up seeing staff around pretty much all day since the nurse-to-resident ratio is higher than many places, with nurses giving almost four hours daily per resident. The center is part of Excelsior Care Group, and what really stands out is the focus on rehabilitation and recovery, especially for folks who need help after a hospital stay. Specialized programs for pulmonary rehab and neuro-rehabilitation are part of their care, and they make sure to have physician services on hand, so there's medical oversight as well as nursing care. The staff gets called compassionate and experienced, but there's been some trouble with nurse turnover, at around sixty percent, which is worth knowing, and the place has had some infection-control problems in the past because those get written up in their inspections. There's skilled nursing care twenty-four hours a day for people who need lots of help, and you can also see the PASARR screening in place, which means they're prepared to assess and care for those with mental or intellectual disabilities. For your daily comfort, the place puts effort into a well-appointed setting, with amenities you can find listed on their website, and though details aren't all laid out, they talk about patient comfort and recovery, event spaces, and a tranquil atmosphere. Boca Circle asks visitors to follow a face coverings policy and has privacy and visitation rules that get explained if you ask. You can take a personal tour, look through photos and videos online, and see for yourself what the center looks like and how it feels. They bring together lots of clinical services-rehab, nursing, restorative care, and physician oversight-with a patient-centered way of treating folks, and they keep quality of life and resident rights in focus while they go about daily care.
