    Trusted small facility, attentive staff

I trust this smaller, well-staffed facility with Mom and Dad - clean, recently renovated rooms with fresh paint and new flooring. Staff are attentive on my unannounced visits, the staff-to-resident ratio is strong, and activities (bingo, music, activity board) keep residents engaged. I'm very pleased with the care and would recommend it.

    Summary of reviews

The reviews present a mixed but actionable picture. Positive comments emphasize day-to-day caregiving strengths: staff are described as attentive and involved, with a favorable staff-to-resident ratio and consistent shift coverage. Families commonly cite trust in the facility’s ability to care for loved ones. The facility runs an active recreation schedule — frequent activities such as bingo, music, and an activity board are highlighted — and several rooms have been recently renovated with new paint and flooring, contributing to a cleaner, updated appearance in resident rooms. The smaller scale of the home is noted as contributing to a more personal, home-like atmosphere.

Counterbalancing those positives are operational concerns that warrant attention. Multiple accounts indicate odor concerns in some common areas and what reviewers characterize as lapses in incontinence care; these items suggest opportunities to strengthen housekeeping and personal-care routines. An unannounced visit described limited staff presence on arrival and observers reported residents appearing restrained, which raises questions about on-floor supervision and the facility’s behavior-management and medication oversight practices. Together these items point to gaps in responsiveness and clinical oversight rather than isolated maintenance issues alone.

Other patterns include variability in cleanliness between different observations and an older overall building infrastructure despite room-level renovations. Dining is not a dominant theme in the available feedback; it was not widely discussed, which suggests prospective families should obtain specific information about meal quality and nutritional oversight during a visit. Activities and staff engagement are consistent strengths, but management-level practices around staffing deployment, incontinence care, and behavioral/medication protocols appear to be unevenly executed based on observer accounts.

For prospective residents and families: arrange both announced and at least one unannounced visit, tour common areas and several resident rooms, and ask management for documentation on staffing ratios, incontinence-care protocols, restraint and behavior-management policies, medication-administration oversight, cleaning schedules, and recent inspection records. Request references from current families and clarify how the facility handles overnight supervision and unexpected clinical events. These steps will help verify whether the facility’s everyday operations align with the positive descriptions of staff engagement and recent room improvements, and whether the noted operational weaknesses have been addressed.

    About Fountains Nursing Home

Fountains Nursing Home sits at 3800 North Federal Highway in Boca Raton, Florida, and has been serving seniors for over 51 years with a range of care options including assisted living, skilled nursing, home care, respite care, hospice care, and memory care, with a special focus on seniors with dementia or Alzheimer's. The facility has 46 units with 46 beds and offers both fully furnished studios and in-room amenities like kitchenettes, cable TV, safety fixtures, and emergency call systems, and the building is fully sprinklered for fire safety. Staff at Fountains Nursing Home provide around-the-clock care, with RNs, LPNs, and CNAs meeting state staffing requirements, and they help with daily needs like bathing, dressing, medication management, feeding, light housekeeping, and meal preparation, while a nurse is always on-site, and there's specialized support for those needing IV therapy, wound care, speech therapy, heart care, physical or occupational therapy, mental health support, dental, podiatry, hearing, and vision care, plus outpatient rehab. Services also extend to in-home care for those who need it, so care professionals visit residents in their own homes to help with non-medical assistance.

Residents can bring small pets or enjoy pet visits, and there are outside gardens, a salon and barbershop, stores, and social activity spaces, along with WiFi throughout the common areas. Meals are served in a restaurant-style dining room or can be brought to rooms, with options for special diets such as gluten-free, diabetic, low sodium, vegetarian, kosher, and snacks always available. Activities include field trips, arts and crafts, daily exercise, religious or spiritual programs, and education groups, with both resident and family councils providing input. Family members can join in through outreach and education services, and counseling is available. Staff are known for being helpful and friendly, working to create a warm, welcoming space, and the facility maintains a commitment to giving each person respect with a focus on quality of life, using a holistic approach to care.

The facility accepts several payment options including private pay, cash, check, private insurance, Medicare, Medicaid, and VA benefits, and is certified for both Medicare and Medicaid, with a Medicare rating of 4.2 out of 5 stars and an average rating of 6.7 out of 10. While Fountains Nursing Home has a record of no penalties, fines, or payment denials, some quality care deficiencies have been noted related to professional standards and laws, and the facility currently has a Special Focus Facility designation, but the staff continue to meet all required health care standards and the building license is verified monthly. Fountains isn't located in a hospital or part of a continuing care community, and it's operated as a for-profit corporation by Purple Door, LLC, serving seniors in Boca Raton with a mix of experience, practical support, and a stable environment for those who need both daily living help and specialized medical attention.
