    Caring staff, excellent therapy, spotless

I placed my mom at Encore for rehab and was very pleased - the staff were warm, attentive, and felt like family, concierge and front-desk were helpful, and the facility and rooms were spotless and bright. Therapy was outstanding (Mitch and the PT/OT team were exceptional) and nursing/CNAs like Hermithe provided truly personal, high-quality care; the food was good and meals convenient. Overall a very positive, reassuring experience that I'd gladly recommend.

    Summary of reviews

The collected reviews portray Encore at Boca Raton Rehabilitation and Nursing Center as a facility with strong strengths in rehabilitation, patient engagement, and a welcoming physical environment, while also exhibiting persistent operational weaknesses. Many families and residents praised the therapy teams (physical and occupational therapists named frequently), attentive CNAs, and concierge/admissions staff; these strengths are associated with measurable rehabilitation gains, effective discharge planning, and a broadly warm, family-like culture. The facility’s public areas, updated interior design, and landscaped outdoor spaces are repeatedly described as bright, hotel-like, and conducive to socialization and recovery.

Care quality shows a dual pattern. On the positive side, numerous accounts highlight skilled, compassionate nursing and therapy personnel, prompt initiation of therapy, individualized treatment plans, and strong wound- and post-acute expertise in many cases. Activity programming and live entertainment received consistent praise for improving resident morale and engagement. Conversely, a substantial set of reviews raise concerns about staffing levels and responsiveness, particularly overnight and on weekends: delayed call-bell responses, slow assistance for toileting or transfers, and missed or delayed medication administration are recurring themes. Reviewers also described lapses in continuity of care tied to staff turnover and uneven handoffs between shifts.

Dining and activities are generally reliable contributors to resident satisfaction: the on‑site café, special meals, and a varied activities calendar are strengths that support quality of life. However, dining quality and portion consistency were uneven across reports; some families supplemented meals or expressed a desire for more consistent protein choices and larger portions. The activity program — including music, outings, and rehabilitative sessions — is widely regarded as robust and beneficial for social and functional recovery.

Facility condition and maintenance are mixed. The building, public rooms, and grounds are often described as clean, modern, and well-kept, with multiple therapy rooms and accessible outdoor spaces. At the same time, reviewers cited localized maintenance problems — water intrusion, visible repair needs, mold or vent-related concerns, elevator outages, and room discrepancies compared with online listings — that suggest inconsistent facilities management in certain areas. A number of families reported odor concerns and other sanitation-related issues in specific units or common areas rather than across the entire campus.

Management and systems issues are a notable pattern in the reviews. Several families reported difficulty reaching nursing leadership or the director of nursing during urgent situations, as well as inconsistent communication from social work and administrative staff about discharge, insurance, and medication changes. There are also serious, though not uniformly described, accusations in some reviews — including concerns following resident deaths, missing personal items, and alleged medication-safety problems — that prompted families to request greater transparency and accountability. Use of the words “allegation” or similar is warranted in a small number of cases; these items indicate a need for thorough incident review and clearer family communication protocols.

Overall, prospective residents and families are likely to find Encore Boca Raton a strong option when rehabilitation goals and a welcoming environment are the primary criteria, particularly if the specific therapy team and frontline caregivers are available and engaged. Prospective purchasers should, however, ask targeted questions about overnight staffing ratios, medication-administration safeguards, maintenance response processes, family communication workflows, language-access services, and inventory/mail handling procedures. An in-person tour that verifies room condition, observes staff responsiveness, and clarifies leadership availability during off-hours will help balance the facility’s substantial rehabilitative strengths against the operational gaps noted by reviewers.

About The Encore at Boca Raton

The Encore at Boca Raton Rehabilitation and Nursing Center sits at 7300 Del Prado Cir S, Boca Raton, serving folks in Palm Beach, Broward, and Dade Counties, and you'll find it's open all day, every day, with a staff who speak English and care for people needing both short-term and long-term help, so whether someone's coming in for rehabilitation after surgery or they're staying longer for memory support or long-term nursing, there are services to match. You'll see they offer private and semi-private rooms, each with their own bathroom, and folks can ask about rates if interested, but right now the center's not taking new patients.

The center holds 154 beds, so it's a sizable place, and the environment is known to be clean and staff are described as kind and skilled, though a few reviews say that some rooms feel a bit like an asylum, which is something to keep in mind if you're thinking about the atmosphere. The Encore offers both skilled and intermediate nursing care, looking after people who need IVs, post-surgical care, wound care, or psychiatric and psychoanalytic support, and there's a memory care staff for those who need extra help with dementia-related issues. There's also a Director of Nursing named Brandy, along with teams of nurses and nursing assistants on duty at all hours.

For folks needing therapy, you'll get occupational, physical, and speech therapy here, and the center has expanded its rehabilitation programs, offering things like orthopedic, cardiac, pulmonary, and even amputee rehabilitation, with specialized programs for veterans too. While staying, people have access to community spaces and can use amenities like a beauty salon, computer access, courtyard, elevators, internet/Wi-Fi, laundry, residents lounge, restaurant-style dining, and services like meals, room service, housekeeping, transportation, and even social outings, plus walking paths for fresh air. There are recreational programs and the Urban Zen program, magazines, books, DVDs, computers, and iPads for leisure time.

You'll find some luxury options, like suites, and concierge and patient experience services, which is something offered since the place is part of CareRite Centers. Some say the food service is good and the team keeps things spotless. The center's earned an A+ rating from BBB and has been listed with its business file open since 2019, run as a limited liability company, and though it doesn't take new patients at the moment, the directory gets updated monthly. There's a warm and welcoming feeling to the place for many, and it's a close-knit community with staff who genuinely aim to meet care needs.
