    Compassionate staff, excellent wound care

I was very pleased with The Gardens - compassionate, attentive staff who treated my loved one like family. Lori the wound care nurse and the PT/OT rehab teams were exceptional, the facility was clean and welcoming, administration responsive, and I would happily recommend it.

    Summary of reviews

The Gardens Health and Rehab Center elicits strongly polarized impressions. Many families and patients praised clinical strengths: a highly regarded wound-care team, robust physical and occupational therapy programs that often produce measurable functional gains, and numerous individual staff members (nurses, CNAs, therapists, and certain administrators) who are described as compassionate and attentive. The facility's exterior, courtyard and some common areas are frequently described as clean and welcoming, and several accounts highlight effective coordination for transportation, discharge planning and therapy-related follow-up.

At the same time, there is a recurring pattern of operational weaknesses that families flagged. Staffing levels and skill mix appear inconsistent across shifts: daytime therapy and nursing coverage often receives positive comments, while overnight and later shifts are described as understaffed or less responsive. This uneven staffing correlates with delays in assistance response times, slow attention to urgent needs, and inconsistent implementation of care plans. Several reviews cite missed or delayed medications and other coordination gaps between clinical staff and pharmacy or families.

Dining and nutrition are another mixed area. Some reviewers note improvements and acceptable meals, but others cite inconsistent food quality and failure to adhere to prescribed diets (for example, diabetes-specific meal requirements). Nutritionist involvement is perceived as variable. Families frequently say they bring supplemental foods or medications from home to ensure continuity.

Facility- and equipment-related issues appear in multiple accounts. While the grounds and some public areas are well maintained, interior conditions and equipment are described as uneven: peeling paint, holes in walls, aging beds and medical equipment, and occasional shortages of basic supplies. Sanitation and incontinence-care concerns are raised repeatedly, as are specific gaps in catheter and ostomy care training and supply availability. Noise controlโ€”especially persistent televisions and nighttime disturbancesโ€”is another environmental complaint.

Management and communication patterns are mixed. Several reviews commend responsive administrators, a supportive director of nursing and staff who engage families; other accounts describe poor phone etiquette, limited communication about plans of care, and instances where families felt they had to advocate vigorously to secure appropriate attention. Documentation and written treatment plans are sometimes absent or inconsistent, and discharge medication coordination has been a point of friction. A subset of reviews also raise administrative concerns such as billing irregularities and insurance-switch confusion.

Notable patterns to weigh: the facility shows clear operational strengths around wound care and rehabilitative therapy, and many individual staff deliver commendable, family-centered care. However, recurring themesโ€”shift-to-shift inconsistency, response-time delays, medication and documentation gaps, sanitation and supply shortages, and variable management follow-throughโ€”indicate areas of systemic vulnerability. Prospective residents and families should consider visiting at different times of day, observing multiple shifts, asking for written care and medication plans, and clarifying dietary and supply responsibilities before admission. If clinical complexity or high acuity care is needed, families may want to confirm physician/APRN engagement and on-site clinical oversight in advance.

About The Gardens Health and Rehab Center

The Gardens Health and Rehab Center, found at 1704 Huntington Village Circle in Daytona Beach, Florida, is a nursing home that provides skilled nursing, rehabilitation, and memory care services to seniors who need high-quality support and medical attention, and with its team of compassionate staff working around the clock, family members often say they notice how the staff pays attention to individual needs, making sure each resident is safe, comfortable, and as independent as possible. The center has 26 private suites and 41 companion suites, which allow people to choose between privacy and a shared room, and each suite offers enough space so residents don't feel crowded or overlooked, which can matter to many folks who still want their own sense of home. The place also has a clean, friendly environment with large, welcoming dining areas and plenty of gathering spaces, like an enclosed courtyard and expansive community rooms, because being able to meet, talk, and do things with others really matters for people living here, and that's why they offer activities like games, arts and crafts, movies, and light exercise classes such as chair yoga and morning stretching, since staying active-mentally and physically-can make a big difference in how people feel each day.

You'll find a modern rehab gym here that's equipped with state-of-the-art machines, and there's even an "ADL suite"-which stands for Activities of Daily Living-where people recovering from illness or surgery can practice the basic skills they need to return to daily life, so folks who've had strokes or orthopedic trouble can work on regaining strength through physical, occupational, and speech therapy, all without leaving the building, and trained staff are always nearby to guide and support residents at every step. Transportation to appointments and outside activities is available onsite, which helps people get to where they need to be, and the facility's location allows easy access to local medical centers and hospitals for any extra care that might come up. Aside from short-term rehab and long-term care, The Gardens also offers hospice and palliative care, as well as respite stays for families who need a break, so there's a wide range of services under one roof, which can be a relief to families looking for continuity of care.

Families often find comfort knowing the center holds a 5-star rating from the Center for Medicare and Medicaid Services, has received industry accreditation, and meets state license standards, so the quality of healthcare is recognized. There's a focus on supporting health through family engagement and educational resources, and residents can use onsite amenities like beauty and barber services, while staff, including experienced CNAs, remain dedicated to making sure everyone's needs are met as best as possible, whether that's help with mobility, eating, or just enjoying a game or a movie. The Gardens Health and Rehab Center is owned by a reputable health system and keeps up with its legal filings, including state filings in Delaware and annual reports for several years, and it stays registered as GARDENS HEALTHCARE & REHABILITATION CENTER LLC with VSTATE FILINGS LLC acting as the registered agent. All in all, this is a facility that works hard to provide a safe, supportive, and caring setting for seniors who need extra help with daily living and rehabilitation.
