    Attentive staff, great rehab progress

I'm very pleased with my experience - the nurses and CNAs were attentive, friendly, and professional, and the rehab/PT team helped my dad make real progress. The facility is clean and welcoming, meals are good, and staff made check-in, visits, and discharge smooth. I felt at home and grateful for the genuine, caring team and would recommend this place for rehab or longer stays.

    Summary of reviews

Fernandina Beach Rehab and Nursing Center elicits a strongly mixed response. Many families and residents commend the facility’s rehabilitation program, front-line caregivers and several administrative staff: therapy services are frequently described as skilled and effective, with named leaders and therapists praised for supervision, instruction and measurable progress. Nursing assistants and nurses are often characterized as compassionate and professional, and multiple accounts highlight prompt call-button responses, helpful social services, a secure and efficient visitor check-in, and on-site amenities such as a salon, activity center and community programming that contribute to resident engagement.

Care quality appears variable across the facility. A consistent pattern in the feedback is uneven performance between shifts and units: daytime staffing and responsiveness are generally viewed more positively than after-hours care. Operational themes include staffing shortages, inconsistent medication and treatment follow-through, and delays in attending to some resident needs. Clinical concerns raised include wound-care and pressure-injury management gaps and incontinence-care delays; some reviewers also described transfers to higher-acuity care without clear communication. These items suggest opportunities to strengthen clinical oversight, staffing reliability and family communication pathways.

Dining and activities receive mixed marks. Several reviewers praise the meals, dietary accommodations and attentive dietary staff, while others describe inconsistent food temperature and quality. The facility’s activity program, volunteer involvement, live music and social offerings are viewed as assets that support resident well-being. Physical spaces are often described as clean and inviting, with spacious resident rooms and cable TV; at the same time, the building shows signs of age and ongoing renovation needs, and reviewers noted parking limitations and intermittent odor and pest-control concerns in some areas.

Management and communication show divergence in experiences. Some families cite responsive administrators who resolve billing and care-plan issues, conduct productive care meetings and facilitate smooth admissions and discharges. Conversely, other accounts describe poor communication, delayed follow-up, uneven complaint handling and limited family notification for incidents. Infection-control and environmental maintenance were also flagged in a subset of reports; given the potential clinical implications, these items warrant direct inquiry and verification with facility leadership and regulators.

Bottom line: prospective residents and families will find notable strengths at this center—particularly its rehabilitation services, many attentive caregivers, on-site amenities and active programs—but should also be aware of variability in staffing, clinical follow-through and environmental maintenance. When evaluating this facility, consider visiting across different shifts, reviewing recent inspection and infection-control records, asking for details about wound-care and medication protocols, and discussing specific staffing plans and communication processes with administration to assess fit for your needs.

About Fernandina Beach Rehab and Nursing Center

Fernandina Beach Rehab and Nursing Center sits on Amelia Island and has been the only skilled nursing facility in the area since 1984, providing different levels of care like skilled nursing and assisted living for folks needing short-term recovery or long-term support, and the building has a modern style that tries to make people feel at home and at ease, with semi-private rooms and common areas where residents can gather or relax, and those features include physical, occupational, and speech therapies, along with specialized help for memory care, wound care, and post-surgical recovery, plus a secured memory care unit for those who need extra protection. The facility has social activities and recreation programs to help residents stay active and engaged, and the promise of a supportive, safe place for people and their families to find peace of mind is something the center aims for, while providing 24/7 nursing care, palliative care, therapeutic recreation, respite care, and social services for folks who need that little bit extra. The staff makes use of specialized equipment and facilities meant to help recovery and daily life, and the center runs as a for-profit business under Citadel Care Centers, and the place reports a nurse staffing of about 3.52 hours per resident each day, but also has a high nurse turnover rate of 76.9%, with 120 certified beds and about 105 residents most days. The center's been accredited by the Joint Commission, which means it meets some recognized standards for quality, but inspection reports show 18 total deficiencies, including 2 infection-related ones, and there have been citations for not preventing abuse, neglect, or exploitation, along with issues giving enough food and fluids to some residents and not always providing safe dialysis care for those who need it. Even so, staff at Fernandina Beach Rehab and Nursing Center take a clinical and compassionate approach, trying to make routines personal, with programs aimed at a better quality of life and social support, tailoring rehabilitation and nursing experiences for each resident's needs as part of their mission to help people regain health or find comfort during long-term care.
