    Excellent rehab care, attentive leadership

I stayed here for short-term rehab and was very pleased - new management (Raelyn Covington) and a kind, professional staff made me feel welcome and handled any issues quickly. The therapy team was outstanding, CNAs and nurses were compassionate, activities were engaging, and the facility was clean with flavorful dining. Attentive leadership and above-and-beyond care gave me confidence and comfort; I highly recommend it for rehab or long-term care.

    Summary of reviews

Reviews of this nursing and rehabilitation center present a mixed but repeatable picture: the facility demonstrates clear strengths in short-term rehabilitation and therapy, while operational and environmental inconsistencies recur across reviews. The therapy department receives frequent, specific praise for helping residents regain function and return home; many families and former patients cited measurable gains and credited therapists and rehab staff with professional, attentive care. Several reviews also highlight compassionate CNAs and nursing staff in many units, warm admissions interactions, and on-site physician availability that supports clinical follow-up.

Clinical care quality appears to vary by unit and shift. Positive accounts describe timely vital checks, attentive IV and wound care, and respectful bedside manner. However, reviewers also described inconsistent staffing coverage, especially on weekends and evenings, which contributed to delayed responses to call lights, missed meal delivery, and inconsistent bathing and incontinence-care practices. Medication processes were another recurrent concern: examples include documentation gaps, missed medication deliveries at discharge, and specific allegations of narcotics diversion and medication mishandling. These issues point to weaknesses in medication-administration controls and discharge coordination that families should verify during tours.

Dining and activities are described unevenly. Some reviewers praised the dining room updates, flavorful meals, creative events, and active programming (karaoke, seasonal themes, social events). Others cited inconsistent meal timing, perceived small portions, and variability in culinary quality across stays. Activity staff and social programming received substantial positive feedback overall, with many residents described as engaged and supported by the activity team.

Facility condition and environmental management also vary. Multiple accounts praise cleanliness and well-kept sections, separate units for rehab, long-term care, and memory care, and effective housekeeping in those areas. Contrasting accounts raise sanitation and pest-control concerns, odor issues in some common areas and vents, limited shower availability, and occasional plumbing or maintenance problems. These descriptions suggest uneven facility upkeep that may be unit-specific rather than facility-wide.

Leadership and administrative performance are described as inconsistent. Several families named administrators and nursing leadership who were accessible, responsive, and helpful during admissions and transitions. Conversely, other reviewers described difficulty reaching administration, slow responses to complaints, billing disputes, and allegations of retaliation or problematic staff conduct that triggered external reports or investigations. Given this variability, prospective residents should ask about recent inspection findings, complaint-resolution processes, and leadership turnover during a visit.

Notable patterns for prospective residents and families: the center is often recommended for short-term rehabilitation because of strong therapy outcomes, on-site medical availability, and staff who facilitate discharge home. However, there are persistent operational concerns—staffing variability, medication-management controls, sanitation and pest-control issues in certain areas, and uneven communication from administration—that warrant targeted questions and verification. When considering placement, families should tour the specific unit, ask about staffing ratios and weekend coverage, review recent inspection reports, inquire about pest-control and housekeeping protocols, and clarify medication and discharge procedures.
