    Treated like family, excellent care

I'm very pleased overall with this facility. The staff treated my loved one like family - friendly front desk, proactive CNAs, attentive nurses and a strong therapy team. Activities (arts & crafts, cooking, gardening and outings) keep residents engaged, and the rehab and dietary teams provide quality, homemade meals. Check-in was smooth, the place felt clean and well-managed under new leadership. I would highly recommend it.

    Summary of reviews

Overall impression: Reviews describe a facility with evident strengths in person-centered services alongside recurring operational inconsistencies. Many families and visitors praised the clinical rehabilitation program, therapy staff, and the general attentiveness of nursing and CNA teams. Admissions and front-desk staff are frequently characterized as welcoming and helpful, and several reviewers highlighted a family-oriented culture, pet visits, and a lively activities program. At the same time, there are repeated concerns about clinical processes, cleanliness in specific areas, and variability in day-to-day operations.

Care quality and clinical issues: The facility is noted for strong restorative and skilled nursing services in many accounts, with therapy staff and some nurses described as respectful and competent. However, reviews also indicate inconsistent clinical monitoring (including glucose and blood-pressure checks) and delays in medication administration and pain control. There are specific concerns about medication handling practices and psychotropic medication use that suggest the need for clearer medication-management protocols and oversight. Nutrition and hydration monitoring and mouth-care follow-up were also flagged in a subset of reviews as areas needing closer attention.

Staff and communication: Staffing strengths include compassionate, resident-focused caregivers, helpful social-work support, and accessible administration in many cases. Nevertheless, several reviews point to variability in staff responsivenessโ€”particularly on weekends and during off shiftsโ€”and to gaps in communication with families and between clinical teams. Instances of dismissive or uncoordinated communication were raised, indicating that familyโ€“staff communication protocols and care-coordination handoffs could benefit from reinforcement.

Dining and activities: The activities program is a clear positive, with a wide range of offerings (arts and crafts, cooking lessons, gardening, seasonal events, and outings) that several families reported as highly engaging and meaningful for residents. The dietary department receives praise for homemade-style meals and specific positive dining experiences; however, other reviews describe bland or cold meals. This suggests some inconsistency in meal quality and temperature control that the kitchen could address through standardization and monitoring.

Facilities, cleanliness, and maintenance: Common areas, grounds, and the lobby are frequently described as well-kept and pleasant, and many reviewers praised housekeeping and maintenance staff. Concurrently, there are mixed reports at the room level and in some shared bathrooms, including sanitation and odor concerns and reports of pest-control issues. Room size and the limited availability of private bathrooms were noted as persistent constraints. Several reviews referenced aging infrastructure and ongoing or promised renovations; follow-through on those projects and on equipment provisioning has been uneven.

Management and patterns over time: Multiple reviewers described a positive shift under new managementโ€”improved morale, more activities, and better appearanceโ€”while others recounted negative experiences connected to prior management or specific shifts. This pattern suggests operational variability that may correlate with leadership, staffing assignments, or time of day/week. Prospective families should consider asking about recent changes in management, staffing ratios across shifts, and the status of renovation projects.

Recommendations for prospective residents and families: Schedule an in-person tour that includes resident rooms, shared bathrooms, dining service during a meal, and observation of an activity. Ask for specifics on clinical-monitoring protocols (blood sugar/BP checks), medication administration procedures, staffing ratios by shift (including weekends), bathing and incontinence-care schedules, laundry processes, pest-control measures, and the timeline/status of any promised renovations or equipment orders. Request recent survey or inspection results and inquire how the facility tracks and resolves family concerns. Doing so will help assess whether the facilityโ€™s strong rehabilitation and activity offerings align with your expectations for clinical oversight and daily operational consistency.

About Santa Rosa Center for Rehabilitation and Healing

Santa Rosa Center for Rehabilitation and Healing sits on Broad Street in Milton, Florida, and offers 110 certified beds for folks needing care, focusing mostly on rehabilitation and healing, so if someone's been through surgery or has a medical condition like heart or lung trouble, they've got specific programs like orthopedic rehabilitation, cardiopulmonary rehabilitation, and even respiratory therapy to help with recovery and better breathing, and they've also set things up to help people who need long-term nursing care or just some help for a short time between the hospital and home; each resident gets a plan of care that's meant to fit with their health and social needs, and the staff, who are known for being both attentive and experienced in the Pensacola region, include dietary aides like Javier Ramos and a Director of Rehabilitation named Christine Fisher, while Samantha Pierce, an LNHA, runs the nursing home. Families can count on the team to try hard to treat every resident as a unique person, working to restore as much health or independence as possible, and they're committed to personal and considerate care even though they're a for-profit center, owned by Santa Rosa SNF Operations Holdings LLC and affiliated with Infinite Care. The staff puts in about 3.58 nurse hours on average per resident per day, which is a bit less than the Florida average, and the nurse turnover rate is on the higher side, but those who stay are trained to figure out what each resident and their family needs and to respond to that. The place has some handy amenities for healing and recovery, offers both inpatient and outpatient care, and has a range of therapy options meant to help folks get better or stay comfortable, though like many facilities, they've had a few noted problems over the years, like needing to keep up with proper mental health screenings and better management of certain medications. The management has been steady since February 2020 under Summit Care II Inc, and while the place is definitely geared for rehabilitation and healing, it's also a spot where residents can get thoughtful long-term care in a setting that's designed to balance medical care with the importance of feeling at home.
