    Excellent rehab care restored independence

I stayed here for rehab and had a very positive experience. The building is bright and spotless, meals are good, and housekeeping/laundry are reliable. Staff were friendly, attentive and professional - shoutout to Nurse Rhea, Donna, Norma and Mr. Craig on the rehab team - and social work/admissions made coordination and discharge seamless. Therapy restored my mobility and independence; I felt cared for and would recommend this well-run, rehab-focused facility.

    Summary of reviews

The reviews for Harborview Decatur describe a facility with sharply polarized experiences: many family members and patients praise strong clinical rehabilitation, dedicated individual staff, effective social work, and clean, well-maintained public spaces, while other accounts raise substantive concerns about basic daily care and safety practices. Positive themes center on successful short-term rehab outcomes, an organized admissions process, attentive social-work coordination, supportive housekeeping/laundry, and a roster of staff members who receive consistent individual praise for responsiveness and clinical skill.

Care quality appears inconsistent across shifts and patient stays. Several reviewers describe effective therapy programs and measurable functional gains; others describe gaps in ongoing clinical follow-up after intake, missed or incorrect medications, and delayed initiation of antibiotics or other therapies. There are also serious clinical-safety concerns cited, including medication-administration errors that resulted in hospital transfers and concerns following a resident's death. Those accounts point to weaknesses in medication checks, documentation practices, and handoffs between units.

Staffing and conduct are recurring themes. Many individual caregivers, nurses, therapists, and social workers are singled out as compassionate and capable, and some management and admissions staff receive praise for clear communication. At the same time, reviewers report inconsistent professionalism, poor phone responsiveness, and variable performance on night shifts. Staffing shortages and task-passing behaviors are described as contributing to delays in assistance with activities of daily living and to gaps in supervision.

Operational areas highlighted for improvement include dining, transportation, and property security. Multiple accounts note inconsistent meal service (timing, temperature, and dietary appropriateness), difficulties arranging transport to medical appointments, and incidents involving missing packages and other property-control problems. Facility-level cleanliness and sanitation are another important axis of variability: some visitors find the building clean and bright, while others report persistent odor concerns, incomplete room cleaning, and lapses in personal-care schedules.

Management and administration likewise receive mixed feedback. Several reviewers commend accessible leadership, effective case management, and coordinated discharge planning; others cite slow administrative responsiveness, trouble obtaining records or prescriptions, and inconsistent communication with families. There are also mentions of alleged criminal incidents involving staff; such accounts underscore the importance of verifying the facility's current staffing vetting, security procedures, and incident-response policies.

For prospective residents and families: the facility demonstrates clear strengths in short-term rehabilitation, social-work support, and in the efforts of many individual staff members. However, the pattern of highly divergent experiences suggests operational inconsistency. When evaluating this facility, prioritize an in-person visit across different shifts, ask specific questions about medication-safety protocols, staffing ratios (including night coverage), sanitation schedules, transportation arrangements, package-security procedures, and how the facility documents and communicates clinical incidents to families.

    About Harborview Decatur

Harborview Decatur sits at 2787 N Decatur Rd in North Decatur, GA, within the 30033 ZIP code, and the place fills up with residents needing different levels of support, since it covers independent living, assisted living, memory care, short-term rehab, and full nursing home services, and I've seen people come for help right after a hospital stay and then others who need more care over time, like skilled nursing or support for dementia in their special Fountainview Center for Alzheimer's Disease. The building holds up to about 73 residents on a daily basis, and the rooms come already furnished, with private bathrooms and kitchenettes, and residents get things like cable TV, phones, and internet, plus air conditioning, and though there's no mention of things like pools or patios, the place has outdoor areas and walking paths, gardens, and common spaces set up for group activities. They take care of lining up daily activities, with movie nights, game and activity rooms, music programs, and fitness classes held in a fitness room, and there's even a library for folks who like some quiet with their books; for those who need spiritual support, pastoral care comes around the clock, and there's always at least a 12-16 hour nursing presence, with a 24-hour call system for emergencies. Meals get cooked by chefs, covering special diets for allergies or diabetes, and they serve them in a dining room, plus you'll find laundry, housekeeping, move-in coordination, and concierge services, so most daily needs get covered, and transportation and some parking are available for those who still go out and about. For medical care, Harborview Decatur is licensed for skilled nursing with certified beds, accepting Medicaid and Medicare, though recent inspection reports found a number of deficiencies, including issues with safety, pharmacy services, and respiratory care, as well as some infection control items, and their nurse turnover is higher than average, which means a fair bit of staff change. There are dedicated spaces and programs for memory care, including dementia support, and they offer short-term rehabilitation between a hospital discharge and returning home, as well as long-term nursing care for frailer folks, plus help with bathing, dressing, transfers, and non-ambulatory care, and the team includes physician assistants, nurse practitioners, geriatric psychiatrists, and links to hospice for when someone needs that kind of support. Harborview Decatur doesn't say anything about allowing pets, but there are other rentals nearby if a pet is important, and the building is off market, meaning you can't buy or lease it like a regular apartment, but they do offer a variety of flexible terms for those moving in. Residents get legal protections regardless of gender identity or orientation, and some personal care services and mental health support are available, with a community-sponsored activity program and scheduled events to keep up social life. Ownership ties back to Ga Nc 14, LLC, with some indirect backers, and Harborview Health Systems runs the parent company; so while Harborview Decatur covers a big range of needs and has a good setup for activities and support, people may want to look over the inspection history and staffing before making any big decisions.
