    Neglectful facility; mixed compassionate staff

I had a mostly negative experience. Inside was reasonably clean but the outside is dirty, the building is very old, cluttered, and in need of repair and better organization. Staff were a mixed bag - several nurses and aides were compassionate and went above and beyond, but others (including the DON) were rude, dismissive and at times discriminatory; chronic short-staffing caused care gaps and neglect (my relative was left unattended for hours, stomach pain wasn't properly addressed, and later died of sepsis). Communication and management were poor; I plan to escalate my concerns and cannot recommend this facility.

    Summary review

Overall impression: Reviews of Hazelhurst Court Care And Rehabilitation Center are mixed but trend toward concern due to a number of serious complaints. Multiple reviewers praise individual staff members for being caring, compassionate, and helpful; several accounts describe staff who went above and beyond and who made families feel included. At the same time, a significant number of reviews raise urgent and specific concerns about inconsistent care, short-staffing, neglect, and poor management response. The overall sentiment is therefore polarized: strong praise for particular caregivers alongside troubling reports of neglect and systemic issues.

Care quality and safety: The dominant theme in negative reviews is inconsistent and sometimes dangerously poor clinical care. Reviewers report care gaps caused by short-staffing (explicitly noted on at least one hall), leaving residents unattended for extended periods, failure to address acute complaints (for example, stomach pain), and even an allegation of death from sepsis attributed to negligence. While some nurses and aides were explicitly called out as providing good care, other staff were described as neglectful, rude, or intimidating. These accounts suggest variability in clinical competence and oversight, with some residents receiving attentive care while others experience serious lapses.

Staff behavior and culture: Staff behavior is a major divisive factor. Positive comments emphasize compassion, hospitality, friendliness, and staff who integrate families into care. Negative comments describe rudeness from nurses and aides, a reportedly rude Director of Nursing (DON), intimidation of patients, and allegations of racial bias/discrimination. Several reviewers felt complaints were ignored, and some described mistreatment of family members and a general perceived lack of compassion. This pattern points to uneven staff training or morale and indicates possible cultural and leadership problems that affect interpersonal care.

Staffing levels and operations: Short-staffing is repeatedly cited as a root cause of many problems—care gaps, patients left unattended, and staff being stretched too thin to respond appropriately. At least one review specifically attributes patient suffering to staffing shortages. Reviewers also call out disorganization, clutter, and a need for clearer instructions from management. These operational shortcomings likely compound clinical and interpersonal problems and suggest management needs to address scheduling, staffing ratios, supervision, and workflow.

Facility condition and environment: Physical conditions are described as mixed. Some reviewers note that the inside of the facility is clean, while others report unpleasant odors, clutter, and disorganization. The exterior and outdoor areas were described as dirty, and multiple reviewers stated the facility is very old and needs cleaning and repair. Overall, the physical environment appears dated and unevenly maintained, which can negatively impact residents’ comfort and families’ perceptions.

Communication and management: Communication problems and poor responsiveness from management are recurring themes. Reviewers say complaints were ignored or not addressed and that management needs clearer instructions and better oversight. One reviewer mentioned plans to discuss issues with management, indicating at least some families try to escalate concerns. The presence of both highly praised staff and serious unresolved complaints suggests inconsistent management follow-through and possible deficiencies in complaint-handling processes and leadership accountability.

Other notable patterns: There is strong variability across halls and staff: some halls or shifts appear to provide very good care while others do not. A few reviewers explicitly warn others to avoid the facility, citing severe outcomes; others call it “the best place I’ve been,” demonstrating how polarizing the experiences can be. There is little to no specific information in these summaries about dining, activities, or clinical specialties, so conclusions in those areas cannot be drawn from the provided reviews.

Conclusion and recommendation: The reviews indicate Hazelhurst Court has caring individuals and pockets of excellent service, but also systemic issues—short-staffing, inconsistent care, communication failures, facility maintenance needs, and serious allegations of neglect and discrimination. Prospective residents and families should weigh the positive accounts of compassionate staff against the risk of variable care and operational problems. If considering this facility, visitors should ask management about staffing levels, complaint resolution processes, staff training on bias and de-escalation, clinical oversight (especially infection control and acute care response), and plans for facility repairs and odor control. Families already using the facility should document concerns, escalate them in writing, and monitor care closely, particularly on halls or shifts that have been singled out for problems.

About Hazelhurst Court Care And Rehabilitation Center

Hazelhurst Court Care And Rehabilitation Center is a nursing home with 73 certified beds that takes care of people who need either short-term rehabilitation or long-term residential care, so if someone can't manage safely at home anymore, this place will help with daily activities like bathing, dressing, eating, and moving around, and they'll make sure to provide 24-hour nursing supervision with on-site physician support. They've got the Pathways to Active Living program, which aims to help folks stay as active and independent as possible, offering all sorts of therapy, from physical and occupational to speech therapy, plus cognitive skills training, balance and strength programs, and care for stroke recovery or post-surgery. Residents get both housekeeping and laundry services, and there's Wi-Fi, cable TV, a range of daily activities, and a life enrichment program that encourages mental, social, and physical engagement, so nobody feels left out. You'll also find wound care, nutritional support (including tube feeding and special diets), pain management, fall prevention, respite care, and hospice services, and the staff includes nurses, therapists, and many support professionals such as a dietitian, pharmacist, psychiatrist, social services, and more. The center is managed by Beacon Health Management and has Beacon staff who are known for friendly care, but it's important to know that the facility has a one-star rating from the Centers for Medicare & Medicaid Services, with several deficiencies noted in government inspections, most linked to infection control and food handling - these did not cause direct harm but could have. Nurse staffing hours run a bit lower than the state average, and nurse turnover is higher than usual, which sometimes happens in busy healthcare places. With a mix of short stay and long-term patients, and amenities like cable TV, Wi-Fi, and regular activities, Hazelhurst Court fits the needs of people recovering from hospital stays or needing ongoing nursing care, and while it's a for-profit, privately owned center, it does meet federal standards to accept Medicare and Medicaid, so families might want to ask more about care practices and staff stability before making decisions.
