    Clean peaceful compassionate memory-care facility

I placed my brother here and felt immediately reassured by the clean, peaceful facility and knowledgeable, compassionate nursing and rehab staff who supported a safe transition and focused on recovery. The Memory Springs memory-care unit (all-female) is tailored and home-like, with engaging activities and attentive dementia care. Admissions and leadership (Mrs. Khari Collins, Raven) were informative and accommodating-dedicated, friendly staff and clear communication made me confident recommending this place.

    Summary of reviews

Overview: Reviews present a polarized picture of Continental Nursing & Rehabilitation Center. Several accounts highlight committed individual caregivers, a strong memory-care unit (Memory Springs), and capable clinical staff in areas such as wound care and discharge planning. At the same time, a number of consistent operational concerns appear across reviews that would be important for prospective residents and families to consider. Care quality and clinical oversight: Clinical strengths include praised wound-care clinicians and staff who have supported successful rehabilitation and safe transitions home. However, recurring themes indicate inconsistent medication administration, occasional medication-supply shortfalls, and examples of missed or delayed medication dosing. Families also describe delays in physician access and in escalation to higher-level medical care, which has at times required external hospital visits. These patterns suggest variability in clinical oversight and handoff processes across shifts and units. Staffing and staff conduct: Many reviewers identify individual staff members as compassionate, attentive, and effective; several remarks single out admissions, social-work, and front-line caregivers as helpful. Conversely, staffing levels and responsiveness are frequently described as inadequate: long call-response times, limited CNA availability, and delayed assistance with toileting and hygiene. Reviewers also raise concerns about staff professionalism and communication tone, including instances of disrespectful interactions and discriminatory remarks. These issues point to uneven staff training, supervision, and accountability. Dining and activities: Meal service elicits mixed feedback. Some families found the food acceptable, while others reported limited menu variety and insufficient accommodation of individualized dietary needs. Activity programming receives generally positive comments within the memory-care unit — Memory Springs is repeatedly noted for engaged, tailored programming — but general activity offerings outside that unit are described as limited or insufficiently stimulating, and pandemic-era accommodations were noted as lacking by some families. Facilities and safety: Facility reports are inconsistent: parts of the building and the memory-care unit are described as clean, bright, and well-maintained, while other areas raise sanitation and odor-control concerns. Reviewers also cite equipment and maintenance shortfalls (broken beds, overcrowded rooms, driveway/parking blockages) and mobility/access restrictions (lockdown/pass systems and limits on outdoor access). Several accounts raise serious safety-related issues, such as delayed transfers and problematic hygiene processes; a subset of reviews references incidents that involved police or required external investigation. Management and communication: A common concern is ineffective communication from administration: unanswered calls and emails, delayed return of funds after a resident's death, and perceived refusal to engage with family requests or corporate contacts. Some families described management as prioritizing administrative or financial operations over timely clinical follow-up. There are also allegations concerning personal-property security and financial handling that suggest gaps in property controls and billing/benefit processes. Notable patterns and considerations: The overall pattern is one of uneven performance: strong pockets of clinical and programmatic quality—particularly in memory care and among certain staff members—exist alongside systemic operational weaknesses in staffing, medication management, sanitation control, family communication, and maintenance. Prospective residents and families who prioritize memory-care programming or who can identify and partner with specific, trusted staff may have a more positive experience. Families should ask targeted questions during tours and admissions about staffing ratios, medication-administration protocols, physician access, sanitation audits, property/financial safeguards, outdoor-access policies, and how management handles complaints and incident escalation. If considering this facility, an on-site visit focused on the specific unit of interest and direct conversations with clinical leadership are advisable.
