    Clean, calm, affordable; supportive staff

I've been very pleased with this facility - clean, calm, and affordable. The staff are supportive, courteous, and attentive; nurses are excellent and Rasheed is punctual, caring, and on point. There's good social interaction; only minor imperfections keep it from being perfect.

    Summary of reviews

Reviewer feedback for Sheridan Village Nursing and Rehabilitation Center is mixed, with clear positive impressions of some frontline staff and the facility’s affordability alongside repeated operational and safety concerns. Several families praise specific nurses and caregiving staff as attentive, courteous, and punctual; reviewers also describe the facility as calm with social opportunities and generally clean interior spaces. Affordability is commonly cited as a strength, and a few staff members receive strong individual praise for responsiveness and compassion.

At the same time, there are recurring operational issues that prospective families should investigate. Multiple reviewers describe inconsistent supervision and staffing patterns that they associate with falls, emergency-room transfers, and delays in responding to resident needs. Relatedly, reviewers raise concerns about medication-management practices, including reports of excessive or inappropriate medication administration and the use of discontinued drugs. There are also consistent reports of inconsistent bathing, grooming, and other personal-care practices that suggest variability in daily-care routines.

Facility upkeep and safety present additional mixed signals. While some reviewers find the environment clean and well maintained, others describe sanitation concerns in personal-care and common areas, as well as deferred maintenance needs (for example, elevator repair). Transportation safety is noted as a specific area of concern, with commentary about van-driver practices. Activity offerings appear to include social interaction opportunities, but some families report limited assistance for outdoor access and restricted resident mobility for recreation.

Communication and management practices emerge as another pattern. Several reviewers describe poor administrative communication, abrupt phone interactions, and an apparent prioritization of financial considerations over individualized care. There are also serious and specific allegations — including theft and inappropriate staff conduct such as sexual harassment — that warrant direct follow-up with the facility and, if appropriate, regulatory authorities. A few reviewers compare care unfavorably to other local providers or state hospitals.

In sum, Sheridan Village presents a split profile: tangible positives in staff members who provide reliable, compassionate care and an affordable, calm environment, contrasted with systemic concerns around supervision, medication management, personal-care consistency, maintenance, transportation safety, and administrative communication. Families considering this facility should visit in person, ask targeted questions about staffing levels and training, review medication-administration and incident-response protocols, inspect maintenance and cleanliness, and inquire about any reported regulatory actions or formal complaints before making a placement decision.

About Sheridan Village Nursing and Rehabilitation Center

Sheridan Village Nursing and Rehabilitation Center sits right along Lake Michigan in Chicago, and while it doesn't look fancy from the outside, the building holds 191 beds for people needing skilled nursing, psychiatric care, and specialized programs, with rooms facing the lake for a nice view when the weather's clear, and they try to give each person personal and goal-based care, whether for recovery or long-term help. The place is open all day, every day, with an experienced staff offering a wide range of services, such as skilled nursing, short-term and long-term rehab, physical, speech, and occupational therapy, plus behavioral care like psychological testing, and even special programs called Adapt and Moving On to help people settle into the community, and they say their team keeps a close eye on both medical and mental progress, making sure folks get what they need as they recover or adjust. They offer memory care and palliative care for those who need extra help, along with medical services like IV antibiotic therapy, wound care, diabetes care, pain management, colostomy and catheter help, dialysis transportation, and more, and there's a big focus on therapies that go beyond the basics, including recreational therapy, nutrition counseling, and even a monthly activity list to keep spirits up. Staff offer social services to help residents plan for going home when they're ready, support with daily life skills, and help connecting with the wider community, while daily living gets some comfort with small touches like an in-house salon offering free haircuts, community councils for both residents and family, lakefront sitting areas, and transportation for appointments. The facility accepts Medicare and Medicaid, provides both personal and medication care, and hosts tours for new residents and families curious about life there; yet, despite the many services, it does face its own challenges, as shown in recent CMS inspection reports where inspectors found 46 total deficiencies, four of them involving infection control, with some citations for not protecting residents from infection, taking too long to report concerns of abuse or neglect, and not responding properly to all reported violations. Ownership is split between individual owners like Yaacov Mashiach, Rita Lipshitz, Rhonda Mashiach, Yechiel Mashiach, and a company called Atied Associates, and while the nurse turnover rate is lower than the state average at 31.5%, the nurse staffing hours per resident per day fall below the average, with 2.33 hours compared to the state's 3.4, and while folks have said staff are compassionate and amenities decent, average online reviews tend to be mixed, with the last check showing a 2.8 out of 5. The place is known for focusing on rehab and recovery through a proactive approach, but it's had both B-minus and C grades in inspections over the years-not without issues, but with some above-average performance in other areas.
