    Attentive staff, clean facility, recommended

I placed my mother here for an 8-month rehab stay and was consistently impressed by the friendly, caring staff who treated her like family. The homey facility is extremely clean with beautiful grounds, good meals on time, and plenty of activities-arts & crafts, bingo, Bible study, movie nights, salon and volunteers. The therapy team and CNAs were attentive and went above and beyond; my family felt reassured and I highly recommend a visit.

    Summary of reviews

Pleasant Meadows Senior Living elicits strongly mixed impressions. Many reviewers highlight the staff as the facility’s principal strength: caregivers are described as compassionate, attentive, and family-oriented, with long-tenured team members who engage residents in a broad calendar of activities. The facility offers a varied activity program (arts and crafts, bingo, worship services, movie nights, spa and salon amenities) and several accounts emphasize a homelike atmosphere and attractive grounds. Several families also praised on-site rehabilitation services and therapy staff who helped residents meet discharge goals.

At the same time, reviews indicate recurring operational weaknesses that prospective families should weigh. Cleanliness and housekeeping standards appear inconsistent across units and timeframes; some accounts praise clean common areas while others cite dust, mold, pest-control concerns, and leaky windows in resident rooms. Food service was another area of divergence — reviewers alternately described satisfactory, timely meals and meals of inconsistent quality that raised concerns about adequate nutrition for some residents.

Clinical care and safety-related concerns appear in multiple reviews and tend to cluster around process and continuity issues rather than uniform practice. Specific operational patterns described include gaps in catheter and wound-care follow-up, inconsistencies in medication/consent documentation, transfer-safety and fall-prevention weaknesses, and lapses in equipment maintenance. Several families reported delays or barriers in referral and intake processing as well as uneven rehabilitation continuity between stays. There are also reports suggesting inconsistent infection-control practices during periods of heightened risk.

Management and communication received mixed feedback. Some reviewers commended proactive administrators and clear communication; others experienced lengthy paperwork timelines, perceived reluctance around certain referrals, and unresponsive family communication when clinical events occurred. A subset of accounts raised serious concerns about property security and financial handling; these were described as allegations and would merit direct follow-up with the provider or regulators for verification.

Overall, Pleasant Meadows appears to offer strong interpersonal care and an active, homey environment when staffing and operational controls align. However, variability in housekeeping, dining, clinical-process rigor, and administrative responsiveness suggest that families should conduct targeted questions and on-site observations before placement. Recommended pre-admission checks include inspecting resident rooms for maintenance and sanitation, reviewing recent inspection or survey findings, clarifying nursing and therapy staffing levels and handoff practices, and obtaining written policies on medication consent, catheter/wound care, infection control, and family communication during clinical incidents.

    About Pleasant Meadows Senior Living

Pleasant Meadows Senior Living in Chrisman, Illinois, at 400 W Washington Ave, has a big healthcare center with more than 100 beds and offers many care options for seniors, like independent living, assisted living, long term care, memory care, respite care, hospice care, and skilled nursing, so folks can stay through different stages as their needs change and there's a special locked unit for residents with Alzheimer's or dementia which makes it safer if memory is a challenge, and besides, there are short-term rehab services for people getting back on their feet after surgery or illness, and you'll find both garden apartments and private rooms with their own bathroom, air conditioning, cable TV, Wi-Fi, and other comforts, so it really can feel like home.

This place isn't huge, with about 11-50 staff members, but they go out of their way to help everyone live as well as possible and the staff are there 12-16 hours a day with a 24-hour call system if someone needs help quick, and there's an on-site chaplain providing worship services, so spiritual needs get met together with daily care, and families can rely on support services to help them through any worries or adjustments, making transitions a little easier. At Pleasant Meadows, there are four garden apartments near the front door, plus 10 senior apartments, and laundry, cleaning, and meal services are part of daily life, with fine dining options including meals for allergies or diabetes, and meals get served by a professional chef in a restaurant-style dining room where you can eat at flexible hours.

When it comes to enjoying time, residents stop by the community room, activity center, and library, plus there's a movie room, arts area, and fitness room, and folks can join in group fitness, movie nights, music time, or faith gatherings, and when the weather's good, many take advantage of outdoor walking paths and gardens. Beauty salon and barbershop services are available for those wanting a trim or some pampering, and a spa, sauna, whirlpool, and wellness space provide calm and relaxation when someone needs to unwind.

Families and visitors often use the scheduled transportation services, and if a resident wants to run errands or get to an appointment, that's all arranged, while move-in coordinators and concierge staff help with the little things and new beginnings, making each step seem less daunting. The setup's always aiming to keep older adults safe with fast emergency alert systems, with telehealth options for medical needs without always leaving the building.

There's a rating of 2.9 from 11 reviews, so while it's got positives, some people notice things to improve, though Pleasant Meadows does provide many choices including rehab, skilled nursing, independent living, and memory care, along with activities to help residents feel connected, and overall it works hard to be a supportive place for seniors searching for a secure, caring place to live.
