    Excellent skilled nursing and rehab

I'm very pleased with the care my father received - skilled nursing, excellent wound care and strong rehab helped him regain mobility and color. The team was attentive and proactive (CNA Lynne, Mike in maintenance and Pam in management stood out), dietary offered many choices, communication was good, the facility is clean with private baths and plenty of activities; I'm grateful for their compassionate, responsive care.

    Summary of reviews

The reviews for Granite Nursing & Rehab LLC present a mixed picture, with distinct areas of strong performance alongside recurring operational weaknesses. Positive accounts highlight competent clinical teams, especially in rehabilitation and wound care, and several families credited staff for measurable functional improvement and compassionate end-of-life support. Multiple reviewers singled out individual caregivers, therapy staff, and certain managers for attentive, respectful treatment and good communication. Facility features that are noted favorably include private bathrooms, a generally clean appearance in common areas, an active programming schedule, and a dietary department willing to offer choices and special meals.

Clinical care quality appears uneven. Many reviewers described effective therapy-led recoveries and capable wound-care interventions, while others described delayed recognition of acute illness, missed treatments, and medication errors. These patterns suggest variability in clinical-assessment processes and escalation protocols: when clinical teams and therapy staff are engaged, outcomes improve; when staffing or supervision lapses occur, families perceived significant care gaps. Several accounts referenced serious consequences following delayed action, which points to a need for more consistent monitoring, clearer escalation pathways, and stronger medication-administration controls.

Staffing, supervision, and conduct were recurring themes. Positive reviews emphasize skilled, compassionate nurses, CNAs, and helpful support staff. Conversely, many reviewers described inconsistent staffing levels, slow responses to call signals, insufficient night coverage, and perceived lapses in bedside care. There are also allegations concerning staff conduct and discriminatory behaviors; these are serious if accurate and indicate a need for stronger human-resources oversight, training in person-centered care, and a reinforced incident-reporting and investigation process.

Dining, housekeeping, and environmental services show similar variability. Some families praised varied menus, special dietary accommodations, and clean common areas, while others reported inconsistent meal delivery, temperature problems, sanitation concerns in kitchen areas, and pest-control issues. Linen supply and incontinence-care processes were flagged as inconsistent by multiple reviewers. These operational areas appear tied to staffing and management oversight and would benefit from standardized service protocols, routine environmental audits, and clearer accountability for supply inventory.

Management and communication receive mixed evaluations. Several reviewers named managers and social-work staff who were proactive, responsive, and helpful in coordinating care and special events. Other families experienced poor documentation, difficulty contacting staff, billing disputes, and inaccurate medical records. These communication and administrative gaps can exacerbate clinical concerns; strengthening interdisciplinary handoffs, family-notification procedures, and billing transparency would likely address many complaints.

Patterns across reviews suggest the facility can deliver strong rehabilitative and compassionate care in many cases, but inconsistent staffing, uneven clinical processes, and lapses in environmental services create variability in resident experience. Prospective residents and families should weigh the positive reports of therapy outcomes and individual caregivers against the noted operational weaknesses. For current leadership, priorities would be stabilizing staffing, improving clinical escalation and medication controls, enforcing sanitation and pest-control standards, and enhancing communication and billing practices to reduce variability and improve overall reliability.

About Granite Nursing & Rehab LLC

Granite Nursing & Rehab LLC sits in Pontoon Beach and Granite City, Illinois, and it mainly focuses on nursing and rehabilitation services, and you'll notice they've got unique care choices and programs only found there, which means residents get help that fits their needs, whether they're staying a long time or just for a quick recovery. The place has a private ownership, and the team there sticks with patient-centered care, so folks see therapy professionals working with evidence-based routines and modern equipment like Accelerated Care Plus, which is often used in sports. They've got skilled nursing, plus physical, occupational, and speech therapies, and they offer therapy six days a week in a dedicated gym, which makes it easier for those rehabbing after stroke, illness, surgery, or injury, and helps with wound care, orthopedic, pulmonary, and even sports medicine rehab, alongside IV therapy and hospice care when needed. There's a memory care unit for those with special memory needs, and they've set up a small personal care section, so anyone who needs a safer, quieter space can feel at ease.

The Pathway Home Wing, fitted with modern rapid recovery suites, serves people who only need short-term rehab, with private rooms, TV, Wi-Fi, and a comfortable spot to rest and heal, while longer-term residents can enjoy the facility's sitting areas and outdoor lounge to help with comfort and connection, both physically and emotionally. Amenities support well-being, with things like cable TV in rooms and a supportive environment that's built to help people heal or manage their conditions a little better. Outpatient therapy, home health, and memory care options also give folks different choices, and the therapy staff aim for personal recovery goals over pushing one-size-fits-all care.
