    Compassionate, clean, comfortable, attentive care

I'm very pleased with this place - clean, not fancy but comfortable, with friendly, patient, and helpful staff who provided excellent, compassionate care for my grandmother in her final days. My dad (who shares a room) enjoys the fun activities and attentive, conscientious team, and our family feels well cared for.

    Summary of reviews

The reviews show a highly divided experience at Elmwood Nursing & Rehab, with a mix of clearly positive interactions and substantial operational concerns. On the positive side, multiple accounts highlight compassionate direct-care staff, a skilled physical therapy team, friendly front-line employees, and periods of engaging activity programming. Several families described satisfactory personal interactions with staff and occasions when meals and common areas were acceptable or well-maintained.

However, a large set of operational weaknesses recurs across reviews. Facility maintenance and infrastructure problemsโ€”including roof and ceiling issues, water intrusion, and areas of visible degradationโ€”are described alongside sanitation and odor-control deficiencies. Reviewers describe broken equipment, intermittent hot-water and shower problems, and unreliable internet access; those issues point to weaknesses in preventive maintenance and building systems management. These infrastructure problems have implications for resident comfort and safety.

Care-quality and staffing concerns are a prominent theme. Reports indicate inconsistent staffing levels, frequent staff turnover, and leadership instability, which reviewers link to delays in care, missed or inconsistent personal-care routines (including management of incontinence and pressure wound prevention), and occasional unsafe handling or transfer practices. Conversely, some direct-care employees and therapists are described as conscientious and effective, creating a mixed picture where strong individual caregivers coexist with systemic gaps.

Family communication and administrative responsiveness are frequent trouble spots. Reviewers describe difficulty reaching staff by phone, delayed responses to requests, frequent room moves that disrupt continuity, and perceived lapses in property management. There are also references to regulatory scrutiny and instances where documentation or staff conduct raised compliance questions; these indicate potential gaps in clinical oversight, recordkeeping, and staff supervision.

Dining and activities appear variable. Some families report acceptable meals and meaningful social programming, including activities that residents enjoy, while others cite limited dementia-tailored programming and insufficient engagement for residents with cognitive impairment. Prospective families should verify activity schedules and dementia-care offerings during a visit.

In summary, Elmwood presents a polarized profile: there are clear strengths among individual caregivers and therapy services, but persistent facility-level problems in maintenance, sanitation, staffing consistency, clinical oversight, and communication create risk for inconsistent resident experiences. Prospective families should perform an in-person tour, observe multiple units and meal times, ask for recent state inspection and staffing records, inquire about maintenance and infection-control protocols, review transfer and wound-prevention procedures, and speak with current families about day-to-day responsiveness before making placement decisions.

    About Elmwood Nursing & Rehab

Elmwood Nursing & Rehab Center is a medium-sized skilled nursing facility with a capacity of 104 beds operating under a for-profit, limited liability company structure. This care center offers both short-term rehabilitation and long-term care services and accepts residents covered by Medicare and Medicaid. Elmwood Nursing & Rehab Center is not included within a continuing care retirement community and functions as an independent nursing and rehabilitation facility. Its services are designed to meet the needs of individuals requiring daily assistance for medical and non-medical tasks, including activities such as dressing, eating, bathroom use, medication administration, and post-hospital rehabilitation.

Short-term rehabilitation outcomes at the facility are rated as below average, with about a third of discharged residents able to return home following their stay. The percentage of residents experiencing emergency room visits during short-term care is over 20%, indicating that medical oversight is an important component of resident health management. Residents who remain for longer periods also receive below-average ratings for care, with a notable proportion maintaining the ability to move, eat, and carry out other activities independently. The annual influenza vaccination rate is close to three-quarters of all residents. Nurse staffing at Elmwood Nursing & Rehab Center averages 2 hours and 30 minutes per resident, per day, a figure that receives particular scrutiny regarding its consistency on weekends compared to weekdays.

Recent health and safety inspection reports provide insight into the daily environment and services. There have been infractions related to providing a safe, clean, and comfortable living environment, sufficient pressure ulcer care, appropriate nutrition and hydration, and maintaining rooms of regulatory standard sizes. There were also findings involving resident safety, accident prevention, pain management, and the requirement to ensure protection from all forms of abuse and neglect. Inspections found that issues of varying severity affected both a small number and a large group of residents in different circumstances. Elmwood Nursing & Rehab Center has received multiple fines over the past three years, reflecting regulatory engagement to address these issues.

While helping a significant number of residents maintain their ability to care for themselves with minimal support, the center remains under close oversight to ensure that safety, care quality, and resident well-being are prioritized. Elmwood Nursing & Rehab Centerโ€™s participation in both Medicare and Medicaid programs makes it accessible to a wide range of individuals with different payment needs. The facilityโ€™s goal is to provide medical rehabilitation and daily support for older adults and those requiring long-term care, balancing its role in post-acute recovery and ongoing life assistance.
