    Friendly, professional, clean rehab experience

My dad has been here for rehab and loves it - the staff are consistently friendly, professional, and attentive, nursing and therapy are excellent, and dietary and laundry services are exceptional. The facility is very clean and homey in a pleasant rural setting with activities and smooth check-in/out; aside from a few minor maintenance projects underway, I feel comfortable recommending it.

    Summary of reviews

The reviews for Ridgeview Health and Rehab Center describe a facility with clear strengths in direct resident services and communal environment, alongside persistent administrative and clinical consistency concerns. Many accounts emphasize the quality of hands-on care: caregivers, CNAs and nurses are frequently described as personable, attentive, and resident-focused. Rehabilitation and skilled-therapy services are also praised by multiple sources, and several families highlighted a functional admissions and check-in/check-out experience suited to short-term rehab stays. The building and grounds are commonly characterized as clean, homey, and set in a pleasant rural environment, with engaging activities such as bingo and an overall welcoming atmosphere.

Dining and support services receive consistently positive mentions. The dietary department and meal service are singled out as strong points, and laundry and housekeeping are described as reliable. Reviewers also note ongoing maintenance and repairs, suggesting a commitment to facility upkeep. Collectively these elements contribute to a setting that many describe as comfortable for residents and visitors alike.

Counterbalancing the positives are recurring operational concerns. There is a pattern of variability in staff professionalism and tone: while many direct-care staff are praised, other accounts describe condescending or rude interactions with both residents and family members. Administrative transparency and complaint-handling emerge as significant issues — examples include disputes about edited records or altered comments and dissatisfaction with how concerns are processed. Documentation gaps and errors (for example, missing emergency-contact entries or misplaced items) were cited and point to weaknesses in administrative workflows.

Clinical and programmatic gaps are another theme. Several reviews cite limited mental-health support and instances where therapy or rehabilitative interventions were discouraged, indicating inconsistent access to or prioritization of those services. Infection-control and sanitation process concerns were noted by some families, which contrasts with many other comments praising cleanliness; this suggests variability between units or shifts. There are also serious, singular allegations related to theft and financial misconduct; such claims warrant careful verification through formal channels.

For prospective residents and families, the pattern is one of a facility that offers strong direct-care and ancillary services in a pleasant setting, but with real variability in administrative responsiveness, clinical consistency, and staff conduct. Recommended areas to probe during a tour or intake include current staffing levels and supervision, mental-health and therapy availability, the facility’s complaint-resolution procedures and record-keeping practices, infection-control protocols, and any recent regulatory or state survey findings. These checks will help determine whether the facility’s praised strengths are reliably delivered for an individual resident’s needs.

About Ridgeview Health and Rehab Center

Ridgeview Health and Rehab Center in Oblong, Illinois, sits on 413 Ridge Ln and runs with about 74.9% occupancy, offering both short-term and long-term nursing care along with a range of therapies like physical, occupational, speech, and respiratory therapy, plus wound care, memory care, respite care, cardiac and orthopedic rehab, stroke recovery, and even a discharge concierge to help residents adjust after leaving care. The center has 55 certified beds and usually has around 48 residents each day, and while they have a modern building with a homely atmosphere and an "always available" menu, nurse staffing comes in below state average at 2.89 nurse hours per resident per day and a nurse turnover rate of 53.7%, higher than the state average. Ridgeview accepts Medicare and Medicaid, brings in registered nurses, LPNs, an RN MDS Coordinator, medical directors, and Certified Nursing Assistants, and is managed under Crest Illinois Holdco III LLC and Shalom Lichtman since 2021, as part of Crest Healthcare Consulting-operating as a for-profit partnership with 51 to 200 staff members. There's a history of variable quality with a C in long-term care and a D grade in short-term care, and while recent facility inspections earned an A-, reports show 38 total deficiencies, some of which cover areas like resident rights (not informing families or doctors quickly), daily nursing staff limits, pressure ulcer care, and infection prevention. Residents and families get to join councils and have access to a visitor-friendly website where you can take a virtual tour or view menus and brochures, and the facility tries to build a sense of belonging through community engagement activities and by working closely with both residents and their families to set up care plans, aiming to help everyone feel part of the place. Ridgeview wants staff to feel valued and treats them like family, which they believe leads to more caring service, and the goal is to support both medical and emotional needs, focusing on comfort, safety, and helping people recover or live as well as they can, though challenges with staffing and quality have been noted in their public reports. The center is licensed and provides most standard nursing home services including homemaking, personal care, transportation, and recreational programs, though some deficiencies and quality grades suggest it's important for families to review reports and decide if this approach meets their specific needs or preferences.
