    Trusted, compassionate care near home

I moved my wife here because it's close to home and the small, homelike setting felt right. The nurses, CNAs and therapy team are attentive, compassionate and truly resident-focused - therapy even helped my wife regain movement - and long-term staff plus strong leadership make the care feel trusted and family-like. The secure memory unit, fenced garden, transportation and deficiency-free/5-star Medicare rating give me extra peace of mind; I'm very satisfied overall with a few minor areas that could be tightened.

    Summary of reviews

Review sentiments for Haven of St. Elmo are polarized: many families and residents praise the facility for hands-on rehabilitation, a caring day-to-day nursing presence, and a small, family-like culture, while other reviews raise substantial operational and clinical concerns. The facility's therapy department and CNAs receive frequent commendation for encouraging recovery and teamwork; several accounts describe meaningful functional improvement, reliable transportation to appointments, and an engaging activities program. The secure Memory Care unit and fenced garden, combined with a long-tenured leadership team and a deficiency-free survey / 5-star Medicare rating, are often cited as strengths that support resident stability and regulatory compliance.

Care-quality patterns are mixed. Positive patterns include attentive rehabilitation services and compassionate bedside care from many front-line staff. Negative patterns reported involve clinical-process gaps: wound management and pressure-injury prevention, oxygen supply and distilled-water handling, and occasional delays in medical attention. Reviewers also describe medication and supply constraints linked to funding or administrative processes. Safety concerns around transfers and mobility assistance appear as a recurring operational theme; some families reported incidents that point to weaknesses in fall-prevention and transfer-safety protocols.

Staff and culture present a complex picture. Numerous reviews describe respectful, family-oriented nurses and aides with strong teamwork and resident focus. At the same time, there are reports of concerning staff conduct and communication tone, instances of unresponsiveness overnight, and privacy or property-control issues that suggest inconsistent staff behavior or supervision. Administrative responsiveness is described both positively and negatively: some families find leadership accessible and helpful, while others note billing, insurance, and documentation problems that required escalation.

Dining and activities: the activities program and transportation are consistently praised as well run and resident-centered. Food quality and meal reliability are more problematic; reviewers mention inconsistent food quality and occasional missed meals, indicating a need for improved meal-service continuity and oversight.

Facilities and maintenance: the site is described as small and homelike but shows signs of aging. Reviewers note outdated fixtures, hand-cranked beds that are difficult to operate, bathroom maintenance needs, limited shower availability, and sanitation concerns in certain areas. These infrastructure issues can affect resident comfort and the delivery of care, particularly for residents with higher physical needs.

Notable patterns and guidance for prospective residents and families: weigh the facility's strong therapy program, family-like culture, memory-care resources, and regulatory ratings against the operational weaknesses some reviewers identified. Before placement, families should ask detailed questions about wound and oxygen protocols, night-shift staffing and response procedures, property and documentation safeguards, medication and supply funding contingencies, meal-service schedules, and routine maintenance plans. Where possible, request recent survey and incident records, tour resident rooms and bathrooms, and speak directly with therapy and nursing leadership about individualized care plans and safety practices.

    About Haven of St. Elmo

Haven of St. Elmo provides skilled nursing, short-term rehabilitation, and long-term care, and the staff there tries hard to make everybody comfortable and well looked after, because the caregivers you meet every day are the ones who set the tone for the place with their kindness and know-how. The facility offers services that help people recover from illness or surgery, with physical therapy to build strength, occupational therapy to help with daily tasks, and speech therapy to work on communication and swallowing, and all these treatments come from folks who know what they're doing and pay real attention to what each resident needs. There are specialty programs for memory care, psychiatric rehabilitation, cardiac rehab, post-stroke rehab, wound care, pain management, and orthopedic and joint replacement, and they've got licensed medical team members working to help people manage things like diabetes, heart failure, and medication, day and night if that's what it takes. Residents get their own care plans made just for them, and the staff really tries to understand each person's goals and motivations, which means care feels more natural and less like a routine. Folks who need long-term skilled nursing have a dedicated nursing team for regular check-ups and ongoing care, while social services can step in to help families make sense of what's next. Amenities like massage therapy, reflexology, an on-site beauty salon, and specialized meal plans add a bit of comfort, and there are lounges, Wi-Fi, cable TV, and activities that can brighten up the day. Residents also have housekeeping and laundry taken care of, and health products, like Plexus Slim or pain relief medicines, are available when needed. The facility's got a peaceful atmosphere where people can work toward getting strong enough to go home, or just settle into daily routines with dignity and support, plus they keep up care even after a resident leaves, guiding them so life can be fuller and more rewarding. Staff members put effort into connecting with both residents and their families, so everybody feels like they're understood, and the programs and rooms have their own names to keep things clear and organized for whoever walks through the door.
