    Warm family-like care, highly recommended

I live here and I'm very pleased - the staff are excellent and genuinely caring, creating a warm, family-like atmosphere where residents are prioritized. It's clean, has great food, attentive, well-staffed care with a central nurse's station, and a quaint rural feel; they accept Medicaid and I'd recommend it as a very good choice.

    Summary of reviews

Overall impression: Reviews present a mixed but clear pattern. Many comments emphasize a caring, attentive workforce, positive first impressions on entry, and a warm, family-like environment with appealing meals and generally clean, odor-free common areas. At the same time, there are substantial concerns about operational consistency, physical plant condition, and management oversight that prospective residents and families should evaluate during a tour.

Care and staff: Strengths cited include attentive, knowledgeable nursing and caregiving staff and a high visible staff presence; these attributes contribute to a perception of residents being prioritized and receiving personal attention. Counterbalancing that, reviewers also describe inconsistent responsiveness, delays in attending to residents, and concerns about the tone and conduct of some staff members. Those descriptions point to variability in day-to-day care delivery and to potential gaps in training, supervision, or staff coordination rather than universal practice failures.

Dining and activities: Dining receives consistently positive mention — meals are described as good and appealing. The facility atmosphere is characterized as warm and family-like, and reviewers report happy residents in a small, rural community setting. There is limited specific information about structured activity programming in the summaries provided; the social environment appears generally positive but activity offerings should be confirmed during follow-up inquiries.

Facilities and safety: The building is described as older and in need of cosmetic updating; reviewers used language indicating worn finishes and deferred maintenance. At the same time, common areas were described as clean and without strong odors. More operational concerns arise around equipment and room-level safety (for example, broken beds and other maintenance items were referenced), indicating a need for stronger equipment upkeep and safety-monitoring processes. There are also comments suggesting weaknesses in safeguarding resident property.

Management and notable patterns: Management-level issues are implied by the combination of positive staffing statements and reports of staff dysfunction, inconsistent responsiveness, equipment failures, and property-safeguarding weaknesses. Some reviewers raised serious allegations (including loss of resident property and safety problems) that suggest gaps in incident response, documentation, and family communication. The facility does accept Medicaid and has a centralized nurse station that may aid oversight, but the contrasting praise and severe concerns recommend targeted questions about staffing ratios, turnover, maintenance schedules, incident-tracking, and complaint-resolution processes.

Recommendation for prospective families: Given the polarized feedback, families should conduct an in-person visit, review recent inspection or complaint records if available, ask management for details on staffing patterns and training, inquire about maintenance and equipment-repair routines, and request examples of how the facility investigates and communicates about resident concerns. This balanced approach will help verify the consistent strengths (staff warmth, dining, cleanliness of common areas) while clarifying the extent and remediation of the operational weaknesses described.

About Eastview Healthcare and Senior Living

Eastview Healthcare and Senior Living sits in a quiet, residential part of Sullivan, Illinois, at 100 Eastview Place, with landscaped grounds and two outdoor patios where folks can sit and enjoy the outdoors if the weather's right, and there's plenty of comfortable living spaces inside, too, as well as handy extras like a beauty salon. The facility fits into Petersen Health Care's larger network and they call it both Eastview Healthcare & Senior Rehabilitation and Eastview Terrace for their nursing home operations. There are 63 certified beds, and on most days about 39 residents live there, getting a mix of skilled nursing, rehabilitation, and memory care-especially to help folks with dementia or Alzheimer's-with a staff that aims to treat everyone with respect, warmth, and dignity, always looking to see what each resident needs. The team offers physical, occupational, and speech therapy seven days a week, focusing on recovery, rehabilitation, and helping people keep their strength and some independence, and they try to include activities like community-sponsored gatherings and arrange shopping outings with the therapists if someone wants to work on real-world skills like cooking or cleaning, or just get out for a while, with scheduled transportation for these non-medical trips. The environment feels warm and friendly, and the staff provides care with a holistic perspective-caring about emotional, social, and spiritual well-being along with physical health. Memory care services get particular attention, so those with dementia and Alzheimer's receive personalized care plans and support every day. Eastview Healthcare and Senior Living also takes pride in family caregiving resources and guidance for long-term care and insurance needs. The care team here includes registered nurses and they do their best, but there are issues that have come up, such as a nurse turnover rate of 46.7%, and they've been cited for not always having a registered nurse on duty and lacking a full-time director of nurses. There have been inspection reports listing problems with infection control, nurse staffing, administration, and repeated citations for not having a proper infection prevention program and not meeting requirements for their Quality Assessment group, so there's room for improvement when it comes to oversight and safety. Even with these challenges, Eastview Healthcare and Senior Living remains a place where seniors get skilled nursing, rehabilitation, memory care, and a supportive environment, with a focus on trying to help residents feel at home and stay active as life changes.
