    Attentive staff, clean modern facility

I spent a month in rehab and had a very positive experience. The nursing and therapy teams - especially Nurse Donna Davis - were attentive, ensured timely meds and meticulous wound care, and helped me recover in a clean, home-like facility with modern private rooms, onsite dialysis access, activities, and caring, responsive staff. Management was visible, call lights were answered promptly, and little touches (birthday cupcakes, fresh air in roomy rooms) made my stay comfortable and reassuring.

    Summary of reviews

The reviews portray Evergreen Crossing & The Lofts as a facility with strong physical-therapy resources, attractive modern rooms, and an active social program, alongside persistent operational inconsistencies. Positive experiences highlight an engaged therapy team, some standout nursing and aide staff who clearly advocate for residents, convenient on-site dialysis, a hair/beauty service, and frequent recreational offerings. The building itself is often described as clean, modern, and comfortable, with private rooms, natural light, and pleasant common spaces that support recovery for a number of residents.

Conversely, a substantial portion of feedback describes variable care quality that appears linked to staffing and organizational processes. Common operational concerns include inconsistent staffing coverage, long call-light delays, gaps in toileting and bathing assistance, and examples of pressure-injury care needs that families felt were not consistently addressed. Several accounts point to medication-administration and documentation problems, admission/discharge paperwork errors, and breakdowns in coordination between nursing, therapy, and kitchen teams. These process weaknesses have translated for some families into delayed clinical responses and the need for repeated requests to obtain basic care actions.

Dining and daily-living services are described unevenly: some families praise chef-prepared meals and celebratory events, while others report cold meals, missed feeds, and brown-bagging by families. Laundry, belongings inventory, and phone/front-desk responsiveness are additional areas of inconsistency. A number of reviews flagged sanitation and pest-control concerns, and others raised safety issues such as room placement for fall-risk residents and incidents of unassisted falls that suggest weaknesses in fall-risk management and bed/transfer safety practices.

Management and leadership impressions vary across accounts. Several reviews commend visible, responsive managers and individual leaders who improved outcomes and responsiveness; others describe perceived dismissiveness, unhelpful administrative interactions, and lapses in investigating family concerns. Taken together, the pattern is highly mixed: some residents experience strong, recovery-oriented care and active engagement, while others encounter significant service and safety gaps. Prospective families should tour multiple times, take specific notes on staffing levels, call-response times, clinical documentation practices, and sanitation measures, and ask for written policies on medication administration, wound care, fall prevention, and admission/discharge procedures before making placement decisions.

About Evergreen Crossing & The Lofts

Evergreen Crossing & The Lofts in Indianapolis, Indiana, brings together a team that works closely with healthcare professionals to look after each resident's needs, and there's staff available day and night to help with things like moving between bed and wheelchair or checking insulin levels, which makes it a place where people feel supported and safe, especially since they have things like fire sprinklers and big windows that let in plenty of natural light. Folks can choose from different living spaces, with the assisted living apartments coming as studios or one-bedrooms with kitchenettes, while The Lofts offer independent apartments with safety features like walk-in showers, grab bars, and emergency systems, and there are also semi-private suites and private suites that have things like in-suite dining, mounted televisions, wireless internet, private bathrooms, and independent heating and cooling, so there's really an option to suit different needs. Meals are taken care of for everyone, and the facility has restaurant café dining, a pub, a gym, a theater, a lounge, and even a full-service salon for residents, plus common areas where people can meet and chat, and they offer activities both indoors and out, whether that's walking paths or an outdoor fire pit, spiritual services, outings to things like concerts or dinner theater, or even brain games and senior flex classes in the Ante Up game room. As part of the CommuniCare Family of Companies, Evergreen Crossing & The Lofts provides a lot of care services, including assisted living, skilled nursing, memory care, rehabilitation services, wound care, hospice and palliative care, therapy for speech, physical and occupational needs, high-frequency rehab, and even help with incontinence and diabetes. They accept many types of payment, including private pay, insurance, veterans' benefits, Medicaid, and Medicare, but there are extra fees for things like moving in, respite care, or extra care levels. Housekeeping and transportation are available, with short-term or long-term stays, and the management believes in building close relationships with both residents and families, working together to make sure care plans fit each person, always trying to create a calm and supportive place where people feel at home and families can visit with ease.
