    Consistently caring, clean, excellent therapy

During multiple stays I found the staff consistently caring, welcoming, and accommodating - therapists and rehab services were excellent and the facility felt clean and home-like. Meals were good and attentive to needs; overall I'm very satisfied and would recommend their compassionate, attentive care.

    Summary of reviews

Reviewer feedback on Brickyard Healthcare - Woodlands Care Center is mixed, with distinct strengths in rehabilitation services and interpersonal care alongside recurring operational concerns. Many families and residents praise the therapy teams, describing strong physical, occupational and speech therapy offerings, extensive hallway mobility activities, and well-equipped therapy spaces. Short-term rehabilitation patients and those needing diet modifications (including swallowing supervision and soft-food accommodations) commonly noted individualized clinical attention and favorable therapy outcomes.

Staff interpersonal qualities are another repeated strength: multiple accounts describe welcoming, compassionate caregivers who create a home-like environment and consistent care across repeat stays. Dining is frequently cited positively, with specific mention of varied, enjoyable meals and attention to dietary needs. These elements together support a perception of effective rehab-focused care and a social atmosphere that some families find reassuring.

Counterbalancing these positives are operational concerns that appear across reviews. The most prominent themes are staffing shortages and slow responses to nurse-call signals, which families link to delays in routine care and perceived inattentiveness at times. Several comments indicate variability in staff conduct and responsiveness; while some staff are described as very caring, others are characterized as less communicative or difficult to engage. Reviewers also raised sanitation and housekeeping inconsistencies in resident rooms and common areas, and described small or outdated resident rooms and bathrooms in parts of the building.

Clinical continuity and leadership communication are additional areas of concern. Some families noted gaps in how hospital discharge instructions and prescribed treatments were followed, and a few referenced significant care-coordination issues that prompted requests for investigation. There are also recurring statements about personal-item security and coordination with outside providers (for example, podiatry), suggesting process weaknesses around property-control and external-provider access. Finally, several reviewers perceived corporate or financial pressures affecting staffing and resourcing decisions.

In summary, prospective residents and families should weigh the facility's evident strengths in rehabilitation, therapy staffing, personalized diet accommodations and a generally welcoming atmosphere against operational risks tied to staffing levels, inconsistent housekeeping, clinical handoff processes, and communication from leadership. Those prioritizing intensive, therapy-driven short stays may find the facility's services strong; families for whom consistent staffing, robust property safeguards, and clear leadership communication are critical may wish to investigate those areas further during a tour and by asking for specific policies and recent quality metrics.

About Brickyard Healthcare - Woodlands Care Center

Brickyard Healthcare - Woodlands Care Center sits at 4088 Frame Rd in Newburgh, IN, and has 120 certified beds, though about 105 residents stay there on an average day, so you do get a mix of long-term and short-term folks coming in and out, and the place has been managed by Ggnsc Newburgh LLC since September 2012, with Executive Director Maribeth Donaldson leading the team, bringing over three decades of skilled nursing experience, and she's been with Brickyard Healthcare for 20 years. The care center offers a wide range of services, including skilled nursing, short-term rehab, long-term care, memory care set up for people with Alzheimer's or dementia, outpatient therapy, dialysis, and VA contract care, and the building is also set up for independent living, assisted living, adult day services, home health care, non-medical home care, and hospice care, though details about apartment style or assisted living features aren't heavily listed. The place's daily staffing boils down to about 3.36 nurse hours per resident, but nurse turnover runs a bit high at 39.2%, and the place has been through 17 reported inspection deficiencies, with a few related to infection standards-such as citation F0880 for infection prevention programs and one about pharmacy storage and labeling, though these didn't cause actual harm but did have the potential for issues, even if only a few people were directly affected. For residents, there are private suites, phone and Wi-Fi, and TV options, plus laundry and housekeeping, and the staff handle a variety of clinical tasks such as IV therapy, pain management, cognitive training, diabetes care, and more serious rehabilitation for folks after strokes, along with regular doctor, dental, eye, and audiology visits. The community provides church services, daily activities, arts and crafts, nutritional snacks, and has barber and beauty shops, along with regular transportation services, and folks can visit in family lounges or the secure outdoor area, especially in the memory care section where extra safety measures and trained staff step in for residents who might wander or need a little extra care because of dementia. There's access for wheelchairs, but not much is told about the general building layout or features like parking, entrances, floors, or exterior space, and while the care staff handles a lot of health and personal needs, there's not much information on luxury amenities or unique rental setups, but you do get a full range of therapy and nursing for the folks who need it, and the facility stays connected with doctors and hospitals like St. Mary's nearby to make sure residents have medical support when they need it.
