    Spotless facility with caring staff

I placed my mom here and feel comfortable leaving her - the facility is spotless, well-maintained, tastefully decorated and odor-free. Staff and management are kind, caring, attentive and responsive; nursing and therapy teams (physical and rehab) are outstanding and the social worker Regina truly cares. Memory-care supports are strong with good doctor/psychiatry involvement, clear family communication, and lots of activities (bingo, church, outings, crafts) that keep her happy and social. Meals are appealing and the overall atmosphere is welcoming and professional; I'd recommend this facility while noting there's always room for small improvements.

    Summary of reviews

Reviews for Brickyard Healthcare - Portage Care Center present a mixed picture with clear strengths in rehabilitation, activities, and pockets of compassionate caregiving, alongside recurring operational weaknesses that families should evaluate closely.

Care quality: Multiple reviewers praised the therapy/rehabilitation teams for skill, personalization of plans, and measurable functional gains. Memory-care programming and roommate matching were also described positively in several accounts. At the same time, there are repeated concerns about clinical follow-through: delayed or missed medication administration, inconsistent attention to pain management and respiratory treatments, and occasional failures in timely clinical response. These issues indicate variability in nursing/clinical performance across units and shifts.

Staff: Staff are frequently described as caring, polite, and responsive by many families; several reviews single out individual nurses, aides, and social work staff for exceptional compassion and follow-up. However, staffing levels and turnover are recurring operational themes. Reviewers report understaffing, busy call lines, and aides or nurses who appear overextended; this operational pressure correlates with reports of delayed care, less frequent checks, and inconsistent documentation.

Dining: Dining impressions are mixed. Some families note appealing meals, fruit and special offerings, and generally good food, while other accounts describe cold meals, inconsistent preparation, limited menu/allergy accommodations, and timing issues. Meal-service reliability and temperature control appear to vary by shift or day.

Activities and social life: Activity programming is a clear strength for many residents. Church services, outings, bingo, crafts, decorating, and social events receive positive mention; reviewers describe an engaged activities department that contributes to social well-being and community connection.

Facilities and environment: Opinions on the physical environment are divided. Numerous reviews describe clean, well-maintained, tastefully decorated common areas and shining floors, while other reviews cite older fixtures, worn furnishings, and sanitation concerns in specific units or rooms. These conflicting impressions suggest variability in housekeeping and maintenance consistency across different areas of the facility.

Management and communications: Management performance receives mixed feedback. Some families praise organized therapy leadership, proactive social workers, and responsive administrators. Conversely, other reviewers report frequent management changes, incomplete paperwork transfers at admission or discharge, and delays or gaps in communication after clinical incidents. There are also multiple accounts of problems with laundry, missing personal items, and concerns about how possessions are tracked.

Notable patterns and guidance: The overall pattern is one of pronounced variability — strong clinical rehabilitation and engaging activities coexist with operational weaknesses in staffing consistency, medication and treatment reliability, sanitation in localized areas, and personal-belongings management. A small number of reviewers described serious clinical and sanitation allegations; these are serious and warrant direct inquiry during a visit. Prospective residents and families should prioritize on-site assessment of staffing levels on different shifts, medication-administration protocols, incident-notification practices, laundry and personal-item procedures, and infection/pest-control processes. Asking for recent survey reports, staffing ratios, ACLS/clinical coverage details, and examples of discharge planning can help clarify whether the facility’s strengths are consistent with an individual resident’s needs.

About Brickyard Healthcare - Portage Care Center

Brickyard Healthcare - Portage Care Center, known locally as Portage Care Center, is a nursing home in Portage, Indiana, that offers a wide range of care services for seniors, including long-term nursing care, short-term rehabilitation, skilled nursing, memory care, and assisted living support, so people can find the type of help they need, whether for a short stay after surgery or for longer support with daily life or memory problems, and it's part of the larger Brickyard Healthcare network. The building has 186 certified beds and keeps skilled nurses on staff 24 hours a day, while it also provides comprehensive medical services like therapy, pulmonary care, IV therapy, pain management, dialysis, diabetic care, and even VA contract care and respite care, which is useful for families in need of a temporary break or help after a hospital stay.

Families can schedule tours or request more information, and there's a map to help with location guidance, making it easier for people to visit and get a feel for the place, and the center accepts Medicaid for eligible residents, which can help with the cost. There are private suites, family lounges, phone services, Wi-Fi, television options, and transportation to local shops, restaurants, and activities in Portage, along with everyday things like a barber and beauty shop, housekeeping, and laundry services, so people can feel comfortable and have their wants taken care of. The staff offers structured social, educational, and entertainment activities and they have programs for people with memory issues like Alzheimer's or dementia, and they also make sure each person has care suited for their needs, whether incontinence care, pressure ulcer care, stroke recovery, restorative care, or support staying safe and healthy day to day.

Brickyard Healthcare - Portage Care Center is run by Fountainview Portage Operating LLC since 2012 with Latoya Haggard serving as Executive Director, and ownership comes through Hendricks County Hospital. They've reported 40 deficiencies in inspection reports, including infection-related issues, and some problems with things like safe respiratory care, proper catheter care, and pressure ulcer care were noted, though the state marked that as potential for more than minimal harm but no actual harm, so it's wise to look over inspection reports before deciding. The nurse turnover rate is 46.1%, which sits a bit below the state average, but nurse hours per resident per day are slightly under the state average. Whether someone needs help with bathing, meals, or medication management, or something specialized like dialysis, diabetic support, transportation, pain, and pharmacy services, Brickyard Healthcare - Portage Care Center has a variety of health services, friendly trained staff, and programs to promote independence. Tours are available if families want to see the center and amenities in person.
