    Upbeat therapy and caring staff

I stayed here for a month after a femur replacement and left doing pretty well-therapy was upbeat and first-class, and the CNAs and nurses were caring, attentive, and made my family feel welcome. My private room (with a lift chair) was clean and home-like, meals and activities kept me engaged, and the social worker and staff were incredibly helpful; overall, a very good experience.

    Summary of reviews

Envive Healthcare of Sullivan receives a mix of positive and critical feedback. Many families and residents praise the facility’s rehabilitative services, noting strong therapy programs and measurable functional improvement after stays. Frontline caregivers — particularly CNAs and certain nurses — are frequently described as compassionate, encouraging, and effective in helping residents regain strength and mobility. Several accounts emphasize a warm, home-like atmosphere, private rooms with adaptive equipment, reliable transportation for dialysis, and a good selection of meals with variety including lighter sandwich options.

Activities and social programming are commonly mentioned as a strength. Reviewers cite regular exercise sessions, bingo, daily physical therapy for rehab patients, and ongoing entertainment that supports social interaction. Admissions staff and the social worker receive positive mention for guidance and follow-through, and some families found the environment safe, clean, and welcoming. Outpatient therapy access nearby and attentive rehab staff contribute to positive outcomes for post-surgical and recovery stays.

At the same time, there are recurring operational concerns. Staffing consistency is a notable pattern: reviewers describe high turnover, sporadic responsiveness to call lights, and situations where some staff members appear less engaged than others. These staffing issues are linked to delays in attending to residents, variable professionalism, and inconsistent supervision across shifts. Several families also raised sanitation and cleanliness inconsistencies in certain areas and expressed concerns about medication-management practices and clinical escalation in at least one serious case; these remarks indicate a need for clearer clinical oversight and communication during deteriorations in condition.

Communication and administration are additional areas of mixed experience. While some families felt well informed at every step, others described gaps in communication, difficulty getting concerns addressed, and billing disputes. A few reviewers suggested management is reluctant to expand CNA and nurse staffing levels despite demand, which may exacerbate responsiveness and workload issues for existing staff.

Overall, the facility demonstrates clear strengths in rehabilitative care, many compassionate caregiving relationships, and an active program of meals and activities that support resident engagement. However, prospective residents and families should be aware of variability in staffing consistency, responsiveness, cleanliness in certain areas, and administrative communication. Those considering this facility may want to ask specific questions about current staffing levels, medication oversight policies, sanitation audits, care-plan communication practices, and how the facility handles clinical escalations to ensure those operational areas meet their expectations.

    About Envive Healthcare of Sullivan

Envive Healthcare of Sullivan sits on 325 West Northwood Drive in Sullivan, Indiana, and provides 77 certified beds, usually serving about 39 residents each day, where folks get a mix of care options depending on what each person really needs, so if someone's looking for nursing care, post-surgical rehab, respite care, or hospice care, the place tries to cover all those bases, offering things like wound care, tracheotomy care, colostomy care, bariatric support, IV therapy, physical, occupational and speech therapies, and even a post cerebrovascular accident (CVA) program that helps people who've had strokes, plus there's 24/7 nursing care with staff available when needed, though the nurse staffing here is about 2.56 hours per resident per day, which is below the state average, and nurse turnover is a bit higher than in other places, running 73.7% yearly compared to the state's 48.5%. Chef-prepared meals help keep the menu varied and nutritious, with a registered dietician putting together therapeutic diets for those who need them, though it's worth mentioning some inspection reports have noted nutrition and dietary concerns, along with pharmacy service and nursing staff deficiencies, none causing actual harm but raising concerns for possible problems, with pharmaceutical service issues affecting a few people and nursing care issues affecting many more. The building's got beautiful grounds, private and semiprivate rooms for comfort and privacy, a fully stocked therapy room, and transportation to help residents get out to appointments, while a full activities and social calendar keeps everyone engaged, offering structured programs that help residents stay active and social, sometimes including educational classes or just friendly gatherings that make it easier to build friendships. Residents can expect individualized care plans and support shaped around their changing needs, with the main goal being to help folks stay as strong and independent as possible, and since Envive Nursing Holdings, LLC took over in January 2022, the place has kept its focus on creating a warm, family-like setting where staff encourage open communication and residents feel like they belong, striving to balance comprehensive medical care and good company, even while working on addressing the challenges highlighted in recent inspections.
