    Short-term rehab with compassionate staff

I placed my parent here for short-term rehab after a hip replacement and we were very pleased - the therapy team helped her recover and she went home in three weeks. The facility is clean, welcoming and feels like family; staff are compassionate, professional, and run engaging activities residents enjoy. Overall, I would recommend this caring, community-focused place.

    Summary of reviews

Overall impression: Irvine Nursing & Rehabilitation is frequently described as a clean, well-maintained, community-focused facility with a strong emphasis on resident activities and short-term rehabilitation. Many families and former residents highlight a warm, family-like atmosphere, affectionate staffโ€“resident interactions, and a robust activity program that keeps residents engaged. The therapy and rehabilitation teams are commonly praised for helping residents meet goals and return home, and several accounts note successful short stays with timely discharge following procedures such as hip replacement.

Care and staffing: A consistent strength cited is the compassionate approach of nursing, therapy, and CNA staff โ€” reviewers often describe staff who are attentive, affectionate, and personally invested in residents' well-being. At the same time, the facility appears to have operational variability. Reported concerns include inconsistent staffing levels (with weekend coverage called out), delays in nursing responsiveness to calls, and intermittent gaps in basic-care tasks. There are also specific clinical concerns raised about wound-care and skin-integrity management that indicate the need for stronger clinical oversight in some cases.

Dining: Dining receives mixed commentary. Many reviewers appreciate menu variety and the availability of home-style meal choices, while others report inconsistent execution โ€” small portions, and meals described as undercooked or overcooked. Prospective residents and families should inquire directly about current menu planning, portioning practices, and how dietary issues are handled.

Activities and community life: This is a clear area of strength. The facility is repeatedly praised for an active calendar of daytime and evening activities, family-inclusive events, and a strong activity director presence. Community involvement and purposeful programming appear to contribute to a home-like atmosphere that many residents and families value.

Facilities and management: The physical environment is generally described as clean, safe, and welcoming. Management strengths cited include professional, friendly staff and a focus on resident-centric care. Areas for managerial attention include communication processes with families (timely updates and phone responsiveness), consistency in staffing patterns, transparent billing/pricing relative to perceived value, and clarity around staff vaccination policies that some observers found concerning.

Notable patterns and guidance for families: The overall pattern is of a facility with substantial strengths in rehabilitation, activities, and a caring staff culture, tempered by operational inconsistencies that can affect individual experiences. When considering Irvine Nursing & Rehabilitation, families may wish to ask about staffing ratios by shift (weekdays vs. weekends), protocols for wound care and clinical oversight, typical therapy success metrics, current dining procedures, how family communication and incident updates are handled, vaccination policies for staff, and a clear explanation of fees and billing practices. These targeted questions can help assess whether the facilityโ€™s strengths align with an individual residentโ€™s needs and whether the noted operational concerns have been addressed.

    About Irvine Nursing & Rehabilitation

Irvine Nursing & Rehabilitation sits at 411 Bertha Wallace Drive in Irvine, Kentucky, right in Estill County, and the place has licensed beds for 86 residents, with about 81 people living there on any given day, and you'll find a mix of skilled nursing care and many types of rehabilitation services. Residents get 24-hour nursing, with licensed nurses always on site, and you'll see a big focus on wound care, medication management, hospice, tracheotomy management, cancer recovery, and all the basics like help with daily living activities, housekeeping, and laundry. The place also covers longer stays and short-term rehab, like if someone's coming back from a hospital with a need for stroke recovery, post-surgery care, or needs therapy for physical, occupational, or speech challenges, which they do both as inpatient and outpatient, and they offer home health care if you'd rather recover at home. Irvine Nursing & Rehabilitation has special programs for people with Alzheimer's and dementia, plus they handle digestive and kidney problems, provide infusion services, handle palliative care, and make space for respiratory and vision or dental needs. The team works on reducing hospital readmissions by catching problems early, and every resident gets a care plan focused on their own needs, including visits from doctors and dietary counseling. You'll notice a supportive setting that's had some recent renovations, and there's a good range of social activities and recreational therapy to keep people active. Staffing shows a nurse turnover rate of 52.9%, with nurse staffing averaging about 3.47 hours per resident daily, which is important to keep in mind. The management team includes Thomas Tackett, Moshe Kelman, and Mica Collins, while Benjamin Landa is the affiliated owner, and the company runs as a for-profit limited liability company. Facilities here get regular inspections, with a recent one in February 2025 showing 4 deficiencies, including 2 tied to infection control and a total of 10 over past visits, so you may want to look at these reports just to know more about the environment. Irvine Nursing & Rehabilitation has won the Best of Kentucky in Nursing and Rehabilitation award for 2024, though like most places, it's good to check the details and see if their care matches what someone needs.
