    Compassionate staff and homey community

I'm very pleased with the compassionate, attentive staff who treat residents like family - nursing and therapy were top-notch and helped with mobility improvements. The facility is clean and homey, full of activities and community events (the kids visiting is a delight), families are happy, and I highly recommend this warm, well-run place.

    Summary of reviews

Reviews of Maysville Nursing and Rehabilitation present a markedly mixed picture. Many accounts highlight strong, compassionate caregiving, an active activity schedule, and effective rehabilitation services that produced measurable mobility improvements for residents. Multiple reviewers praised a clean facility, personable interactions between staff and residents, intergenerational programming (for example, visits from local Head Start children), and a family-like atmosphere that some families found comforting.

At the same time, a consistent set of operational concerns appears across reviews. Staffing levels and staff-to-resident ratios were frequently described as insufficient, which reviewers link to delays in responding to resident needs and long hold times on telephone inquiries. Several accounts raise concerns about medication-administration consistency — including timing and insulin management — and more generally about clinical monitoring and hydration oversight. Variability in staff professionalism and tone is a recurrent theme: some families describe attentive, respectful care, while others describe brusque or unresponsive interactions.

Dining and dietary management are also areas of notable variability. Reviewers referenced inconsistent food quality, untimely meal delivery, mismatches between prescribed diets (for example, diabetic meal plans) and what was served, and situations where residents missed or slept through meal opportunities. One theme is a perceived discrepancy between how food and services are presented in external communications and the day-to-day dining experience on the unit.

Activities and social programming are frequently praised for being engaging and well-run, with outdoor events and community connections highlighted as strengths. However, a few reviewers noted that some residents spent extended periods without meaningful activity access, suggesting uneven implementation of programming across the facility. Facility maintenance and safety issues were also mentioned in some reports — for example, problematic ramps/steps and temporary safety markings — indicating localized upkeep concerns. Additionally, there are complaints about missing personal items and inadequate controls for resident belongings, which families should consider probing further.

Management and administrative responsiveness appear uneven: some families commend managers and therapy staff for good communication and follow-through, while others describe slow or unsatisfactory responses to incidents and concerns. Given the polarized feedback, prospective residents and families should conduct an on-site visit, ask direct questions about current staffing ratios, medication and dietary protocols (including insulin administration and diabetic menu planning), personal-item safeguards, and recent maintenance or safety work. Observing meal service, asking for therapy outcome metrics, and speaking with both day and night staff can help clarify whether the facility’s strengths align with an individual’s care priorities.

    About Maysville Nursing and Rehabilitation

Maysville Nursing and Rehabilitation sits at 620 Parker Road in Maysville, Kentucky, and offers a place for those needing skilled nursing, rehabilitation, long-term care, or respite care, and while many people do call it home-an average of about 121 residents a day out of the 130 certified beds-they try to keep things comfortable and focus on what each person needs, though nurse staffing hours come to about 3.8 per resident each day with a nurse turnover rate of 44.4 percent, which is something to think about. Managed by Valley Stream Operator LLC since May 2020, and with Ms. Tia Osbourne as the administrator, the facility is owned by several parties, including Benjamin Landa and KY Equity Partners LLC, so the leadership has changed over time. You'll find memory care, palliative care, short-term rehab, physical therapy, occupational therapy, and speech therapy offered, and the staff often works together to support each person's well-being, offering personalized care in a setting designed for comfort and community, and they try to create a warm and inviting atmosphere by providing different activities and simple amenities for daily living. The state inspections show some challenges, with 14 deficiencies noted overall, and the standard inspection on October 7, 2021 listed four deficiencies, including issues with care planning-like F0655 for not completing care plans within 48 hours and F0656 for not developing thorough plans-as well as a quality of life deficiency labeled as F0689, and one infection-related problem that didn't meet federal standards, so it's important to read those reports with care. Everything is overseen on site, and the building has options for people needing different lengths of stay or varying support levels, and they say they strive to treat everyone with compassion and respect, guided by a non-discrimination policy, and there's a virtual tour online if you want to get a closer look without visiting in person.
