    Compassionate professional staff, welcoming facility

I placed my mom here and overall had a very positive experience. Sally Garcia, the on-site administrator, is hands-on and available 24/7; staff are compassionate, professional, patient and knowledgeable, and the rehab team helped her improve. The home is clean, warm and welcoming with lovely common rooms - I highly recommend this caring, organized facility.

    Summary of reviews

Maple Grove Senior Living presents a mix of strongly positive caregiving experiences and several serious operational concerns. Many family accounts emphasize compassionate, attentive caregivers and an effective therapy/rehabilitation program; therapy staff and certain leaders onsite are frequently singled out for professionalism and for producing measurable functional gains. The facility’s common areas are described as warm and residential — large formal living rooms, comfortable seating, fireplaces and audiovisual amenities contribute to a home-like environment, and overall cleanliness and organization are commonly mentioned.

Care quality appears to be uneven. Multiple families praised nursing and direct-care staff for kindness, bedside attention, and supportive end-of-life interactions; others described delays or gaps in clinical oversight, including missed condition monitoring and incomplete handoffs from hospital to rehab. Specific clinical concerns cited include delayed treatment for acute conditions and scenarios that point to weaknesses in medication or monitoring workflows. Rehabilitation services are a relative strength, though access and scheduling of therapy were described as inconsistent for some residents.

Infection-control and environmental health represent recurring patterns of concern. Reviewers noted pest-control and mold issues and described episodes of contagious conditions that raise questions about isolation practices and family notification. These accounts suggest that infection-prevention procedures and environmental maintenance are areas requiring clarification and improvement. Cleanliness of public spaces is generally viewed positively, but environmental and sanitation-related issues in resident units were also reported.

Communication and management practices show both strengths and weaknesses. The on-site administrator and certain directors are frequently praised for hands-on leadership, accessibility, and compassionate handling of difficult situations. At the same time, families reported inconsistent notification about hospital transfers or significant clinical events, examples of administrative denial or minimization of issues, and at least one pattern of staff apprehension about raising concerns. There are also mentions of billing or administrative-process problems and allegations regarding online presentation that merit verification.

Activities and dining receive limited, mostly neutral mention in the available feedback; amenities and social spaces are appreciated, but specific information about structured activities and meal quality is sparse. Staffing patterns appear to vary: many reviewers noted dedicated, knowledgeable caregivers, while others described confrontational interactions or understaffing-related delays that affected care responsiveness.

Overall, Maple Grove offers clear strengths in individualized caregiving, therapy outcomes, and a welcoming physical environment, supported by visible on-site leadership. However, prospective residents and families should perform targeted due diligence on operational areas highlighted by reviewers: pest and building-environment maintenance, infection-prevention protocols, clinical handoff and monitoring procedures, family-notification policies, and billing/administrative practices. Asking for written policies, recent inspection or maintenance records, and examples of how the facility addressed past incidents may help assess whether the facility’s operational controls align with a family’s expectations and risk tolerance.

About Maple Grove Senior Living, LLC

Maple Grove Senior Living, LLC sits in the countryside around Shelbyville, Kentucky, with a stately front porch, gazebos, courtyards, and beautifully landscaped outdoor areas giving it a peaceful feel, while the building's recent renovations help make things more comfortable and bring the residents together. This place is a non-profit skilled nursing home, licensed for 117 beds, with about 80 people living there every day. The staff, ranging from 51 to 200 employees, includes experienced professionals, therapists, and skilled nursing care professionals who work to meet both medical and emotional needs, which is something you do notice-since many stay long-term, there's a sense of familiarity here. Nurse aides provide about 2 hours and 37 minutes of care per resident daily, with licensed nurses averaging 4 hours and 33 minutes per resident each day, which is well above the Kentucky average, and nurse turnover's also a bit lower than in most places. You see about 3 minutes of physical therapy per person each day, so therapy's available though not for long periods.

Maple Grove has won several awards for long-term care and's recognized as the most awarded senior living and long-term care facility in Kentucky, plus it sticks close to the old Masonic tradition in its values. It's not joined to a hospital or a big retirement community, so it stays a bit quieter and more focused, which matches well with what people look for in the area. The building passed its last fire inspection in July 2024, has had no Medicare payment denials or federal fines in three years, and hasn't gotten complaints that led to citations. The most recent inspection showed four deficiencies-which is about average-including a couple for infection control and one about food sourcing, but there was no actual harm, just the potential for things to get worse if not fixed. It has a health inspections quality rating of 4, with an overall rating of 4 out of 5, but for resident care quality the score's around 2 out of 5 for short-stay and 3 for long-stay residents, which is something to think about.

The community offers several kinds of care: skilled nursing, long-term stays, assisted and personal care, respite services if someone stops in for a short stay, rehab services, and memory care programs for people who need help with memory loss. Residents and families can join councils to share their thoughts and help with decisions. There are activities to keep people engaged, and the place encourages connection and trust, trying to make it homelike as much as possible. Maple Grove is nonprofit and privately held and puts an emphasis on compassionate care and community involvement. For those who'd like to look around before coming, it features a virtual tour and photos of the community. Overall, Maple Grove Senior Living, LLC focuses on steady care, safety, and a sense of belonging, with a mix of practical support and attention to comfort, though it's smart to check what features and services are currently offered since there's not a lot of detailed information about them out there.
