    Pleasant setting but dangerous care

I had a very mixed experience. The place is plush with a beautiful lake view, friendly caregivers, engaging activities, and some genuinely attentive staff - my family liked the room and atmosphere. But care was inconsistent and at times dangerous: residents ignored for hours, oxygen/medical needs not monitored or provided, wandering and injury incidents, medication problems, and reports of abuse/neglect. Cleanliness and maintenance were hit-or-miss (dirty rooms, roaches, slow or no fixes, no weekend maintenance), kitchen/sanitation issues, and administration was dismissive and unresponsive. Because of those serious safety and management concerns, I can't confidently recommend it despite the pleasant setting and some caring employees.

    About Leslie Lakes Retirement Center

Leslie Lakes Retirement Center sits on 50 quiet acres and offers a blend of living options like independent apartments, assisted living, memory care, and a dedicated skilled nursing and rehab center, so you're likely to see folks fishing for bass and bream, eating in the buffet dining room or the short-order grill, or taking part in social outings, painting, dancing, and even regular church services in the chapel, and you'll find the place keeps a small-town feel thanks to its large private and semi-private suites, outdoor patios for family picnics, and special spaces like the Gingerbread House Gift Shop and a nicely decorated hospice suite, plus there's an Alzheimer's Neighborhood on Wisteria Lane with unit color choices and a secure setup with staff trained for memory care, while people needing short or long-term nursing have bariatric suites and skilled tracheostomy care if needed, and there's 24/7 licensed nurses and CNAs, a Certified Activity Director, and year-round recreational activities for everyone, and for folks looking for therapy, the in-house rehab staff provides physical, occupational, and speech therapy, and there's post-acute care for those recovering from surgery or illness with the help of a physician-led team, while unique programs like Oak Woods Rehab & Wellness give specialized care, and services cover things like social services, professional therapy, and home health care through several providers, with hospice care offered under names like Ascend Hospice Care and Legacy Hospice, now the average daily census is around 86 residents but there are 150 certified beds, and some residents prefer large suites, some independent apartments, and those who need steady routines or extra support with dementia have a safe environment and outdoor activities to help, and you'll notice the community tries to set a homelike tone that honors family spirit and resident dignity, and meals are nutritious, routines are simple for those with Alzheimer's, and the team takes a flexible approach, but past inspection reports do show 17 deficiencies including failure to fully assess all residents within the time required and some resident rights not being reasonably accommodated, and management comes from Paramount Healthcare Consultants LLC since 2008, with ownership involving Dcz Healthcare Investments LLC, Pkc Investments LLC, Dawne Smith, and Paul Coburn at a 73% stake, and you'll see a nurse turnover rate of about 35.2% with a staffing ratio of 3.71 nursing hours per resident per day, and while there are good amenities and social opportunities, the facility's records show ongoing areas for improvement related to care and resident rights, but staff do make efforts to provide a supportive, safe, and respectful environment with services tailored to seniors' needs and preferences, and with links to multiple health service providers and a focus on letting people make the most of each day, Leslie Lakes Retirement Center tries to create a place where older adults can live with comfort, care, and choices.
