    Improved care and compassionate therapy

I moved my mom to Chestertown Nursing and Rehab under new management in November and have seen clear improvements. The nursing and PT/OT teams are professional, compassionate, and personalize care - wounds healed and first steps regained. Staff are attentive, friendly, work well together, and the home-like atmosphere, therapy, activities and food all support recovery. I'm grateful for their dedication and would recommend this facility.

    Summary of reviews

The reviews present a strongly mixed picture of Chestertown Nursing and Rehab, with clear clusters of both high praise and serious operational concerns. Positive accounts emphasize strong therapy services and clinically focused recovery outcomes: families describe effective PT/OT teams, measurable functional gains, and attentive rehabilitation plans that appear to support healing and discharge goals. Several reviews also highlight compassionate, friendly nursing and ancillary staff who produce a welcoming, home-like atmosphere; activities (including pet therapy) and food quality were cited as contributors to resident morale.

Conversely, many reviewers described operational and care-delivery weaknesses that affect daily resident experience. Staffing variability and coverage gaps are recurring themes, frequently linked to delayed responses to call bells, delayed assistance with personal care, and missed or late meals. These delays were also tied to concerns about medication administration and monitoring in a few accounts, and to lapses in privacy and dignity practices. Housekeeping and sanitation inconsistencies โ€” including odor concerns and incontinence-care issues โ€” were cited alongside pest-control concerns and episodic cleanliness problems in resident rooms and common areas.

Communication and management practices appear to be a dividing line. Multiple families reported poor follow-up on phone calls, limited transparency around incidents, and inconsistent accountability for adverse events. At the same time, several more recent reviews describe a change in ownership/management and clearer, more proactive leadership: these note improved staff responsiveness, better coordination, and visible process improvements that families found reassuring. This suggests the facility may be in transition, with improvements visible to some but legacy problems persisting for others.

For prospective residents and families: weigh the strong rehabilitative and therapy capabilities and the positive reports of compassionate care against the operational risks tied to staffing, housekeeping, and communication. When evaluating the facility, consider direct observation of staffing levels during care times, review of recent inspection or complaint history, inquiry about medication-administration protocols and incident reporting, questions about pest-control and housekeeping routines, and discussion of financial/consent procedures. Visiting during typical meal and activity times and speaking with the therapy team and recent family contacts can help provide a clearer, current picture of daily operations.

    About Chestertown Nursing and Rehab

Chestertown Nursing and Rehab sits on Morgnec Road in Chestertown, Maryland, with certified beds for ninety-two residents and an average of seventy people staying at one time, and the place is open around the clock, every day of the week, even Sundays, so folks are never left alone and always have nearby care, and meals are provided, and there's indoor common areas where you can sit and talk, with activities planned on-site and devotional services for those who want them, and if someone gets hurt or there's a big change in health, staff will tell doctors, families, and the residents right away. Residents can choose long-term care, short-term rehab, respite care, or hospice services, with skilled nursing staff and rehab therapists working together to match the care to what each person needs, and they try to focus on dignity and respect, making people feel comfortable with care plans built just for them. The team says they're dedicated to safety and check their protocols often, offering a warm atmosphere with scenic views, and they have activities and programs that try to help with physical, emotional, and spiritual needs. Some details to bear in mind, though, are that the facility has had forty-eight inspection deficiencies, including two for infection control, and has been cited for medication errors, problems with keeping the place clean or safe, and not setting up a steady quality assessment group. The nurse turnover rate is 63.7%, higher than the Maryland average, and there are fewer nursing hours per resident per day compared to other places in the state, which means nurses may be stretched thin. The business side is handled by a for-profit company called MD4 Holdco, LLC, owned by Yechiel Hirth, and management staff information isn't made public, which some folks may wonder about. There are family and resident councils, and the home accepts both Medicare and Medicaid, and the building is fully sprinklered for fire safety. While there's no published price list and no information on whether it's part of a Quality Indicator Survey, and it's not inside a hospital or a retirement community, many people like the compassionate community feeling and the reliable meal service.
