    Excellent month long rehab care

I had a month-long rehab stay at SKLD Leonard and had a very positive experience. The therapy team helped me regain mobility, staff were kind, responsive and respectful, rooms and common areas were immaculate, and meals and activities were enjoyable - I felt well cared for and would recommend them.

    Summary of reviews

SKLD Leonard elicits strongly mixed impressions. Many reviewers praise the facility's rehabilitation services and individual caregivers: physical therapy and rehab staff are frequently cited as effective, motivating, and instrumental in returning residents home. Housekeeping and cleanliness receive consistent positive mention, and numerous accounts describe friendly, attentive aides and support staff who create a supportive, family-like atmosphere in this smaller building.

At the same time, a pattern of operational weaknesses appears across reviews. Medication management and timely administration are recurrent concerns, and slow response to call lights or requests for assistance is repeatedly described. These issues are often attributed to staffing shortfalls; reviewers indicate that inadequate staffing levels negatively affect responsiveness and routine personal care tasks, including assistance with toileting and linen changes. Reviewers also describe inconsistent clinical monitoring practices, which suggests variability in how vital signs and other routine checks are handled.

Dining and therapeutic services show divergence: while many residents find the food acceptable and some praise the kitchen, others report inconsistent meal quality and limited accommodation for special diets. Therapy is more uniformly praised for improving mobility and helping patients return home, but communication between therapy, nursing, and families is sometimes reported as uneven. Activities and educational programming are generally viewed positively and contribute to a constructive daily environment for many residents.

Facility condition and management practices form a third area of concern. Several comments point to an older, sometimes outdated building with room-condition variability (temperature control and mattress comfort). Administrative issues include gaps in discharge planning, billing coordination including Medicaid-related processes, and delays or missed follow-up on ancillary services such as dental care. There are also serious individual claims — including allegations of missing personal belongings and worries about medication-consent practices — that families should investigate further with administrators and, if warranted, with oversight authorities.

In summary, SKLD Leonard demonstrates clear strengths in rehabilitation, housekeeping, and the interpersonal skills of many staff members. Prospective residents and families should weigh those strengths against documented operational problems: inconsistent medication and clinical monitoring, responsiveness tied to staffing levels, variable dining accommodations, and some management-level coordination issues. Those considering the facility would benefit from targeted questions about staffing ratios, medication protocols, dietary accommodations, discharge and billing processes, property-security measures, and how the facility coordinates with hospice and external providers.

    About SKLD Leonard

SKLD Leonard sits about 2.5 miles outside Grand Rapids, Michigan, and serves older adults who need various kinds of support, offering skilled nursing, rehabilitation, and a nursing home environment. Residents get around-the-clock care, with help for daily tasks, medication management, and personalized plans based on their unique needs. They've got both private and semi-private studios, and each room features a TV, phone, and internet so folks can keep in touch and stay comfortable. There's chef-prepared meals, full-service dining, and nutrition supervision, but some inspection reports have found problems with food handling and infection control-31 deficiencies total, including 5 about infections.

SKLD Leonard's got specialized care for memory loss, cancer, tracheostomy, IV therapy, and orthopedic needs, and there's a state-of-the-art therapy gym for physical, occupational, speech, respiratory, and neurological therapy. Residents can join community activities, relax in lounge areas, or use the Wi-Fi and gadgets provided to connect with others, all in a place that tries to be welcoming and homey. There are about 63 residents on average, and the facility has 69 certified beds. The nurse staffing level is a bit below the Michigan average, with a turnover rate higher than most, but there are still dedicated staff around who manage care, laundry, housekeeping, and even respite or hospice care for those who need it.

Owned by Zihuatanejo Holdings LLC and managed by Illuminate Healthcare Network LLC since 2018, the facility accepts managed care options like Medicare Advantage and Medicaid. There are options for inpatient and outpatient rehab, long-term care, assisted and independent living, and important things like memory and continuing care. There's always a chance to take a tour, meet other residents, see how things work, and experience that sense of community and support they strive for, but it's always good to keep in mind those inspection results when making a decision.
