Legacy AFC, LLC is an independently owned, nurse-owned, and family-operated adult foster care home in Ithaca, Michigan. Licensed by the State of Michigan as an Adult Small Group Home, we provide 24- hour staffing, supervision, and personalized assistance in a comfortable family setting. All resident bedrooms are private, and three include an half bath. Services include personal care assistance, support with daily living and mobility, medication management, meals, housekeeping, laundry, activities, and coordination with health care providers and outside services. Legacy AFC accepts private pay and Medicaid Waiver Residents, along with PACE for eligible participants. Transportation is not provided directly, but staff do assist residents with arranging transportation. Please call 989-388-5000 for current room availability and rates
