    Spotless compassionate rehab rebuilt confidence

I had a very good rehab stay at Evergreen - the place was spotless, I felt safe and comfortable, and therapy (OT Keysha stood out) rebuilt my confidence and mobility. Nurses, CNAs and aides were kind, professional and attentive, and activities/front-desk customer service made the days pleasant. I left stronger and would recommend Evergreen for rehab and compassionate care.

    Summary of reviews

Reviewers present a consistent duality: Evergreenโ€™s rehabilitation services and many frontline staff are frequently described as excellent, while operational and clinical continuity issues recur in many accounts. Physical, occupational and speech therapy teams receive repeated praise for their skills, motivation, and measurable progress toward discharge goals. Recreational programming and communal amenities โ€” ice cream socials, live music, bingo and wellโ€‘kept outdoor courtyards โ€” are commonly noted as meaningful contributors to resident morale. The facilityโ€™s campus, dining room and public areas are often described as clean and contemporary, and several frontโ€‘line customerโ€‘service staff are singled out for helpfulness and warmth.

Clinical care quality has two distinct patterns in the reviews. On the positive side, individual nurses, CNAs, and therapists are regularly credited with compassionate, handsโ€‘on care and proactive problemโ€‘solving; families and residents describe strong therapeutic outcomes and attentive rehabilitation. On the other hand, reviewers repeatedly identify systemic weaknesses: staffing levels that strain RN coverage, frequent reliance on LPNs and aides, inconsistent nightโ€‘shift staffing, and delays in responding to call lights and personalโ€‘care needs. Several accounts describe medication administration, wound care, and diabetes/vitalโ€‘sign monitoring problems that required family advocacy or clinician escalation. A small but serious subset of comments raises concerns following hospital readmission and a resident death, and there are isolated allegations of theft and mealโ€‘handling errors including allergen mishaps; these indicate a need for careful clinical oversight and riskโ€‘management review.

Operational and management themes recur across reviews. Discharge planning and paperwork are frequently described as delayed or incomplete, and caseโ€‘management/socialโ€‘work coordination is characterized as uneven. Laundry handling and tracking of personal belongings surface as repeat operational problems. Communication gaps between shifts and between clinical teams and families contribute to frustration; reviewers report improvements when unit managers or administrators intervene, but note that responsiveness is inconsistent. Night and weekend performance is a particular area of concern, with multiple accounts of slower response times and reduced availability of therapy or personalโ€‘care services during those periods.

Dining and activities show mixed but actionable patterns. Many reviews praise an accommodating kitchen and an engaged dietitian or chef who can tailor meals; others describe cold or repetitive meals and at least one serious allergyโ€‘related meal error. Activities and communal programming are a strong point, offering social engagement that residents value. Facility cleanliness and landscaping receive positive comments overall, but some reviews flag sanitation and pestโ€‘control concerns in specific rooms or areas, suggesting inconsistency in environmental services.

What this pattern means for prospective residents and families: Evergreen appears to be a strong choice for goalโ€‘oriented shortโ€‘term rehabilitation where therapy is the priority and where individual caregivers excel. For longerโ€‘term skilled nursing needs or for patients requiring close medical oversight, families should verify current nurseโ€‘toโ€‘patient ratios, ask about nightโ€‘shift staffing and physician availability, confirm woundโ€‘care and medicationโ€‘management protocols, and clarify discharge and laundry procedures in advance. Bringing an advocate and establishing frequent checkโ€‘ins with unit leadership can help address variability in responsiveness and ensure care plans are followed consistently.

About Evergreen Health & Rehabilitation Center

Evergreen Health & Rehabilitation Center sits at 19933 W 13 Mile Rd in Southfield, MI, and covers a lot of ground all in one single-story building, so you're not dealing with stairs or elevators getting in the way, and since the place opened in 2019 it still feels fairly new with a clean environment and well-kept features. This facility takes up to 172 residents and offers skilled nursing, long-term care, memory care, assisted living amenities, and all sorts of rehabilitation like physical, occupational, and speech therapy, and the therapy gym is up-to-date for rehab and therapy sessions. The center cares for folks recovering from cancer, broken foot injuries, foot surgery, and even has special therapy programs like the Rivetus iHeart Rehabilitation Program, Lymphedema Therapy, VitalStimยฎ, LSVT Big and Loud, as well as falls prevention, so you see they handle all kinds of situations, including respiratory therapy and support after you need it, and the nurses and CNAs are known for being friendly and courteous. Anna's Place, the memory care unit, gives people with dementia or Alzheimer's some specialized programming and activities, and the staff works on individualized care plans but also emphasizes safety features and expertise specifically aimed at memory care. For care needs that change, Evergreen provides things like hospice care for a comfortable end-of-life experience, respite care for when caregivers need relief, and supports both short stays and longer terms, even discharge planning and initial assessments. They also accept Medicaid and several insurance plans like Medicare Advantage, and if you're a veteran, the place offers contracted and preferred provider status, which means more options for coverage.

The building has private and semi-private suites with private baths, memory care studio apartments, and all units come standard with amenities such as housekeeping, washers and dryers, cable TV, and safety features like sprinkler systems. Housekeeping and laundry services are under Jason Quantrell's careful direction, while Chef Marvin Pittman oversees dining services, making sure meals are served three times a day in their dedicated dining room. You've got outdoor spaces that the residents say are wonderful, a salon/barbershop, game and activities room, fitness center, WiFi, kitchenettes, and more for folks who like to stay busy or just relax in good company. The activities team, led by Angela Croci, offers social events, arts and crafts, health and wellness programs, and even education sessions, and for anyone with questions about benefits, costs, or contracts, staff members will sit with prospective residents and family members to lay everything out. Bob Clark keeps the environment running smoothly, Beth Seelbach leads social work, Ruby Sherman-Brown handles guest registration, and the therapy director, Heather Wild, coordinates rehab, so there's a whole team making sure things operate as they should. Medical care, led by Dr. Kristina Hyde, includes support like medication management, wound care, podiatry, and ongoing nursing round the clock, and Maggie Demock, the registered dietitian, sees to specialized dietary needs.

Memory care, skilled nursing, and assisted living services all come with their own price points, so for a nursing home setting, private rooms run $9,000 to $12,000 per month, semi-private are about $8,000 to $10,000, and memory care studio apartments usually cost between $5,000 and $6,500 per month, but families can ask about Medicaid if they need help covering costs. Room layouts meet most needs-private spaces, shared spaces, and details that help folks stay comfortable and as independent as possible. Transportation services, in-house maintenance, and an on-site concierge help take the load off daily worries. Evergreen invites families to come tour the community in person or do a virtual visit, so people can see the staff in action and get a real feel for the programs, activities, and everyday life, which is always the best way to get answers and really know if the care and comforts line up with what someone needs.
