    Attentive, supportive care and therapy

I'm very satisfied with Maple Manor - the staff were attentive, friendly and family-like (special thanks to Paula, Karen, Cameron, Liza and Louie), the owner/administration stayed involved, and the facility was clean and homey. Twice-daily PT/OT and dedicated therapists noticeably improved mobility, meals always included fresh vegetables, and there were plenty of activities and thoughtful touches (a window picnic let him see great-grandchildren). My family felt supported, communication was consistent, and overall the care, respect and value were excellent.

    Summary of reviews

Maple Manor of Wayne elicits strongly polarized impressions. Many families and residents praise the facilityโ€™s strengths in rehabilitation: an experienced physical and occupational therapy team, frequent therapy sessions, a well-equipped therapy room, and demonstrable improvement in mobility and function for a number of patients. Direct-care staff are frequently described as personable, compassionate, and family-focused; reviewers highlight consistent caregiver assignments, helpful patient-liaison and office staff, and staff who know residents by name. The facilityโ€™s communal spaces and therapy areas are commonly noted as clean and well-maintained, and the activity program โ€” including a transportation van and community trips โ€” is a recurring positive. Dining receives a lot of favorable comment as well, with varied menus and fresh-vegetable options often cited.

Counterbalancing those positives are recurring operational weaknesses that affect safety and confidence for some families. Staffing consistency is a prominent concern, with reviewers noting diminished coverage on nights and weekends and slower response times at the nursesโ€™ station or front desk. Related to staffing and oversight, reviewers described gaps in medication management and clinical coordination, along with delays in supplying essential equipment such as walkers and pressure-relief mattresses. Several accounts point to sanitation and incontinence-care issues and to inconsistent housekeeping and room maintenance practices, which can undermine the otherwise clean appearance of common areas and therapy spaces.

Administration and communication emerge as mixed. Some families report accessible, responsive leadership and owner involvement; others describe front-desk unresponsiveness, poor phone communication, ineffective social work follow-up, and billing or insurance-processing problems. Dietary accommodations are another area of variability: while many residents report good meals and dietary variety, others cite unmet allergy needs or inconsistent food quality. Safety-monitoring and fall-prevention were raised as concerns in a subset of reviews, sometimes accompanied by rehospitalization or wound-care issues; these represent serious clinical concerns when they occur and contribute to the polarized overall impression.

In practical terms, Maple Manor appears to perform strongly as a rehab-focused center for patients needing intensive therapy and supportive, personable caregiving. Prospective residents and families prioritizing rehabilitation outcomes, active programming, and an engaged therapy team may find it a good fit. Families emphasizing consistent night-and-weekend staffing, ironclad medication controls, reliable equipment provisioning, or flawless administrative processes should probe those areas during tours and conversations with management. Asking about weekend staffing ratios, medication-administration protocols, equipment-delivery timelines, dietary-accommodation processes, and recent quality or regulatory reviews will help clarify whether the facilityโ€™s operational practices align with a particular residentโ€™s needs.

    About Maple Manor of Wayne

Maple Manor of Wayne sits on Venoy Road in Wayne, Michigan, and it's known as a senior living community that's family-owned by physicians Stella and Marcus Evangelista, with Stella holding most of the ownership, and with a focus on providing skilled nursing, subacute care, and rehabilitation to older adults who need daily help or are working to recover their abilities so they can move home or to assisted or independent living. The facility has 59 certified beds and a daily average of 38 residents, with a range of care options including 24-hour skilled nursing, medication management, therapy services seven days a week, and assistance with daily living tasks, and it's got nurses, board-certified doctors, and respiratory therapists on-site around the clock, striving to keep care personalized by watching over residents closely. Residents can live in either private or semi-private apartments, with 28 assisted or independent living units and 59 beds for rehab, and there's plenty of space, as the building covers about 70,000 square feet, so folks have room for activities and comfort.

Maple Manor of Wayne has earned several top awards, including Medicare Five Star ratings and Perfect State Surveys in years past, and it's accredited by CARF International and the Joint Commission, which shows a level of quality recognized by outside organizations. The home uses a person-centered approach, aiming to support each resident's needs, with specially trained staff focusing on rehabilitation, long-term care, and making sure residents are looked after. The nurse staffing ratio is almost double state and national averages at 4.75 nurse hours per resident per day, which sounds nice on paper, though there's a high nurse turnover rate at 66.7%, and that can sometimes mean changes in staff.

Inspection reports from the Centers for Medicare and Medicaid Services show a total of 14 deficiencies, including issues with food procurement and meal services, infection prevention, and following resident rights. One complaint from February 5, 2025, found Maple Manor failed to prepare a resident safely for transfer or discharge, and inspectors also documented a violation of federal infection control standards. So, while the facility has strong points in family supervision, therapy programs, and reliable on-site staff, it's also had several federal and state reports highlight areas where improvement is needed, especially around infection control, nutrition, and resident protections. The owners and staff have worked to focus on safe, respectful care, and Maple Manor of Wayne continues to operate as part of the larger Maple Manor Rehab and Neuro Centers, serving seniors who need varying levels of support.
