    Personalized compassionate memory care community

I had a very positive experience with Sunflower Communities - small, personable, and truly focused on one-on-one, tailored care. Carol Kreuser-Bade (and staff like Janelle K.) were compassionate, knowledgeable, quick to respond, and excellent advocates who smoothed our move-in and connected us with helpful resources. The facility offers strong health and memory-care services, fresh meals, engaging activities (great sensory room), rare single rooms, and a community-minded team that volunteers and supports families. I'm pleased with the care we received and would recommend them as a dependable, caring option.

    Summary of reviews

The reviews present a mixed picture of Nagel Assisted Living & Memory Care: many families and partners praise the facility for its compassionate, hands-on staff and strong family advocacy, while recurring operational concerns suggest areas that require managerial attention. Strengths center on person-focused care for residents with complex medical and memory needs, along with individualized attention made possible by the facility’s small scale and one-on-one approaches. Several reviewers singled out particular team members and care coordinators as responsive and helpful, and the facility’s memory-care offerings, sensory-room resources, and continuing-education opportunities for staff were viewed positively.

Care quality shows two distinct patterns. On the positive side, staff are frequently described as kind, attentive, and effective at coordinating moves and transitions; reviewers noted supportive interactions, smooth move-ins, and good follow-up by central staff via phone. The facility’s ability to support higher medical and behavioral-health needs was highlighted, and some families recommended the program for private-pay and waivered clients. On the other hand, consistent concerns relate to medication management, staff inexperience, and training delays. These issues manifest as missed or incomplete med passes, unclear medication controls, and general gaps in clinical-process reliability — operational weaknesses that affect perceived safety and trust.

Dining and activities drew mixed feedback. Some reviewers appreciated fresh food, an active dining experience, and varied menus; others found the kitchen small and were dissatisfied with meal quality. Activity programming and engagement are strengths where they occur — tailored programs, outings, and a well-regarded sensory room were mentioned positively — but reviewers also described irregular scheduling and instances where field trips or outings did not take place as expected.

Facility design and practical operations present additional considerations. Positive notes include the availability of single rooms and a personable, small-community atmosphere. Concerns include limited accessibility features (no ramps mentioned), shared living-space constraints, and a perception that parts of the building feel institutional. Communication infrastructure gaps — notably a lack of phones on resident floors or at nursing stations — were cited as creating barriers for residents to contact loved ones and for timely family communication.

Management and culture show both capability and inconsistency. Several reviewers praised the leadership’s problem-solving, community involvement, and staff-development orientation, including CEU opportunities and expansion plans. Conversely, others reported organizational disorganization, privacy-protection lapses (staff speaking aloud about resident identities or conditions), and episodes of unprofessional conduct that raise questions about staff professionalism and cultural sensitivity. There were also mentions of safety and regulatory concerns that suggest the facility should be reviewed for compliance and process improvements.

For prospective residents and families: Nagel offers strengths in compassionate, individualized memory-care and close family advocacy, but the pattern of operational concerns — particularly around medication management, consistent staff training, privacy protections, communication systems, and accessibility — warrants careful inquiry. Before transitioning a loved one, ask for current staffing ratios and turnover data, the facility’s medication-administration protocols and audit results, documentation of safety/regulatory compliance, examples of activity schedules and outing plans, and a chance to meet the core care team who will be involved day to day.

About Nagel Assisted Living & Memory Care

Nagel Assisted Living & Memory Care is set in the historic Nagel Hospital building in downtown Waconia, Minnesota, a place with a nostalgic feel that goes back to 1940, where residents enjoy a mix of modern comforts and the charm of days gone by, and with its location you're close to restaurants, shopping centers, and antique shops, along with the convenience of being not far from downtown Minneapolis. The community offers both assisted living and memory care services, with a focus on providing personalized attention, and there's staff available 24 hours a day, including nurses and caregivers trained to help with medication management, bathing, dressing, and support for high medical needs or complex behaviors. Memory care residents have a supervised environment designed to reduce confusion and prevent wandering, with their own dedicated areas and 16 units, and there's a strong focus on meaningful activities to keep their minds and lives active.

Residents can choose from private or semi-private rooms, furnished and designed to be comfortable and homey, with features like kitchenettes, lounge spaces, phone desks, and libraries, and there are even private cottages for folks who want a bit more independence. For meals, there's all-day and anytime dining, meal delivery to rooms, special menus with options like no-sugar or guest meals, and dining rooms that encourage socializing. The building has places to gather like communal living rooms, kitchens, decks, and outdoor spaces, and there's always something going on, from movie nights and scheduled daily activities to resident-run clubs and community-sponsored events. There's a garden and walking paths, plus a barber and salon on-site, and for those who love to keep busy with arts or gardening, there are rooms for hobbies and space outdoors.

Other services provided include housekeeping, laundry and dry cleaning, move-in help, transportation, and parking, so day-to-day life is easier for everyone. Residents also get support from visiting doctors, nurses, physical therapists, and a licensed psychotherapist who comes weekly. Health services cover non-ambulatory care, incontinence care, diabetic care, and help with activities like bathing and dressing. There's a strong safety presence, with emergency alert systems and a robust call system in place to keep people secure, and the staff is friendly, easy to talk to, and always close by. The community is pet-friendly, accepts CADI, EW, and BI waivers, and offers respite or short-term stays as well.

Nagel Assisted Living & Memory Care is part of Sunflower Communities and mainly aims to help folks live simply without giving up quality or support, focusing on providing modest, affordable housing in a small, home-like setting, where residents can go about their lives with the help they need, but still keep as much independence as possible-kind of like being on a quiet, friendly main street where people look out for each other and always have something to do if they want to join in. The building and its daily life encourage making new friends, staying mentally active, and connecting with the local community, while professional staff takes care to help every resident feel safe and valued. The community's strong reputation shows, too, with a rating of 4.7 out of 53 reviews.
